Provides clinical leadership to promote increased compliance with a range of quality and cost measures and standards of care. Manages service delivery of inter-professional and para-professional team members working on an individual case or population of cases. Manages and provides clinical services in compliance with standards of Patient-Centered Medical Home standards, meaningful use of medical record data, HEDIS and QARR quality of care measurements. We are currently seeking a full-time physical therapist for our outpatient and multispecialty clinics in our Boston location Key Responsibilities: Performing complex diagnostic evaluations upon referral and by departmental and professional guidelines and standards. Verifying appropriateness of referral, contacting provider as necessary. Reviewing patient medical history/record and selecting, administering, and interpreting standardized and non-standardized tests and measurements to assess patients' functional status related to cardiopulmonary, developmental, musculoskeletal, neurological, and sensorimotor function. Provides direct psychosocial services to mentally ill patients in the community who are experiencing psychosocial difficulties. Provides linkage, coordination with, referral to and follow-up with appropriate ongoing service providers. About NYC Health + Hospitals New York City Health and Hospitals Corporation Outposted Therapeutic Housing Units Program (OTxHU) Since 2016, Correctional Health Services (CHS) has been the direct provider of health care in the New York City jails. Deeply committed to human dignity and patient rights, CHS is part of the NYC Health + Hospitals system and is a key partner in the City's efforts to reform the criminal-legal system. Our in-jail clinical services include medical, nursing, and mental health care; pharmacy services; substance-use treatment; social work; dental and vision care; discharge planning; and reentry support. Given the high visibility of this initiative, we are seeking the highest caliber health care professionals in key clinical services to staff our Outposted Therapeutic Housing Units (OTxHU). To be located in three NYC Health + Hospital acute care facilities, the OTxHU is a pioneering approach to safely increasing access to high quality clinical care for patients in custody who have complicated health conditions. OTxHUs will bridge the gap in the continuum between care provided in the jails and inpatient hospitalization, with admission to and discharge from the OTxHU in accordance with a patient's clinical needs. CHS will be the primary health care providers on these units and the NYC Department of Correction will provide security and custody management. The OTxHU at NYC Health + Hospitals/Bellevue in Manhattan will be the first of this unique, groundbreaking project to open with a planned completion date as early as the end of 2024. Facility: Bellevue-OTxHU and shifts on Rikers Island Duties & Responsibilities Under the supervision of the Clinical Supervisor, will provide quality mental health care to for individuals incarcerated in the New York City jail system in their assigned area of work. Responsibilities include: Conducts mental health evaluations, crisis intervention, assessments, and follow-up care. Collaborates on determining disposition of patients for appropriate level of mental health care. Coordinates with Reentry Social Workers for appropriate discharge planning services. Completes required statistical reports. Collaborates with the Department of Correction to ensure access to patients, proper housing, and treatment disposition. Coordinates patient care with other institutional services and programs. Facilitates or co-facilitates leading mental health group treatment. Completes all required documentation. Participates in clinical supervision with Clinical Supervisor at least once a week Participates in mental health and medical rounds as assigned. Participates in case conferences and treatment planning for patients receiving and being considered for mental health treatment. May conduct psychological testing as assigned, under supervision May supervise social work interns or student volunteers as assigned. Performs other duties as assigned by Senior/Clinical Supervisor. SUMMARY OF DUTIES The Family Service Worker (FSW) provides transportation and supervised visitation services in a safe setting between parents and their children in foster care. Provide transportation services to participants for case related activities to including medical appointments, therapy appointments, court hearings, unsupervised visits, or for any other case related activities that do not require monitoring or supervision. Provides childcare during the times participants are receiving CCSS related services as a way to address participation barriers. The FSW maintains participant confidentiality at all times, participates in program staff meetings, meets with supervisor on a regular basis and attends training and other staff development activities. The position maintains case recording, mileage logs and other miscellaneous reports in a timely fashion. EDUCATION AND TRAINING REQUIREMENTS Up to and including high school. Requires enough basic education to understand and follow standard practice or oral and written instructions, able to read and write, use simple arithmetic, keep simple records, or use simple office machines like a typewriter. EXPERIENCE Over one year, up to and including two years of relevant experience. Over two years, up to and including four years. SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED Experience with child abuse and neglect, domestic violence, permanency issues, and substance abuse required. Experience in working with parents/caretakers who harmed their children or threatened their children with harm and children who experienced harm or were threatened with harm and who experienced trauma and loss. Possess a willingness to work with others, including clients coping with multiple issues, families that present safety issues, and co-workers, as part of a team. OTHER POSITION REQUIREMENTS This job requires a valid Hawaii driver's license, a clear driver's abstract, and use of agency vehicle to transport participants, as needed, as well as a willingness to travel in personal vehicle on an as-needed basis throughout the island.
