SUMMARY OF DUTIES
Provides short-term outreach and home visiting services, as well as self-sufficiency education classes to at-risk families. Engages families in services. Conducts intake/assessment with participants and create service plan and goals. Teaches and provides information on a wide variety of topics, including Kauai community resources, budget and finance, economic stability and vocational development, as indicated by participant needs. Provides linkage to community resources and services such as counseling, legal aid, and vocational assistance. Assists families, through advocacy or concrete help, to negotiate systems to receive needed services. Provides transportation to medical, legal and as needed appointments with consultation with supervisor. Performs administrative tasks related to caseload such as progress notes, service plans, mileage and miscellaneous reports in a timely manner. Maintains participant confidentiality at all times.
EDUCATION AND TRAINING REQUIREMENTS
Four-Year College Degree from a school accredited by a recognized accrediting agency - general major. Requires basic knowledge or training in a specialized or technical field or trade, such as social work, professional accounting, finance, business administration, and marketing.
EXPERIENCE
Over two years, up to and including four years.
Other (Specify): Experience working with families, who have abused or neglected their children, or high-risk families, preferred. Case management experience preferred.
SPECIAL KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED
Knowledge of child growth, development and behavior. Knowledge of positive parenting practices. Knowledge of case management and advocacy practices and techniques. Good written and verbal communication skills. Ability to work with families of diverse ethnic, cultural and socio-economic backgrounds. Ability to work flexible hours, including evenings and weekends as required. Valid driver's license, current no-fault insurance coverage, satisfactory driver's abstract, access to an insured vehicle.

Under the direct supervision of the Crisis Specialist / Clinical Supervisor the MHS-I conducts participant intake and initiates all forms required to open case. Performs crisis risk assessment and face-to-face stabilization services with youth in an active state of psychiatric crisis. Establishes participant rapport and trust and provides empathetic encouragement and support participants. Ensures participant safety by examining alternatives and providing options for alleviating the immediate situational threat in relation to the crisis. Maintains a list of collaborative contacts and community services pertinent to participant's needs. Develops a plan to energize participant to take action. Obtains agreement (from participant) as to the action required to stabilize the participant or crisis situation. Provides information and referral services to community resources and agencies. Provides participant transportation to hospital or TCH Home (with parental consent) as needed. Maintains participant confidentiality at all times. Participates in program staff meetings, meets with supervisor on a regular basis, and attends training and other staff development activities, especially on topics related to pertinent laws, the judicial process and cultural/ethnic diversity. Enters data into the computer and maintains case recording, mileage logs and other miscellaneous reports in a timely fashion.

SUMMARY OF DUTIES
Provide mentorship and support for justice-involved youth, helping them build a stable foundation for long-term success. Our services assist with probation and post-release supervision while offering re-entry support to promote positive life outcomes.
Key Responsibilities
Mentor Matching and Engagement
Oversee the matching of adult mentor/peer mentor-mentee relationships.
Engage with youth and promote positive youth development through mentoring guidance and support.
Conduct community outreach to recruit volunteers as peer mentors.
Case Management and Coordination
Provide case management and coordination services to youth referred by Judiciary, Family Court, and the Hawaii Youth Correctional Facility Juvenile Probation.
Conduct screenings and intakes.
Play a key role in the treatment team by scheduling and coordinating assessments and service planning meetings.
Develop youth- and family-driven service plans based on completed assessments.
Ensure smooth transitions during referrals and coordination of services.

SUMMARY OF DUTIES
The Family Counselor is responsible for collaborating with CFS programs and community programs on the implementation of program services, linking the assessment, design, implementation and evaluation of therapeutic interventions with the purpose of achieving successful case outcome. Flexible provision of services that accommodate the families' needs and adjust schedules to be available evenings and weekends. Under the supervisor's directive may be required to respond to on-call crisis response with 24/7 availability.

Provide mentorship and support for justice-involved youth, helping them build a stable foundation for long-term success. Our services assist with probation and post-release supervision while offering re-entry support to promote positive life outcomes.

Oversee the matching of adult mentor/peer mentor-mentee relationships.

Effectively communicates with program staff before, during and after shift changes. Administers medications to ensure compliance/accuracy as well as adverse reactions. May assist with life skills groups with youth. Takes youth to community outings and events as approved by Clinical Lead.
EDUCATION AND TRAINING REQUIREMENTS
High School Diploma and have completed or complete within three (3) months of hire a childcare worker training course that includes at least 50 hours of instruction. Requires ability to understand and carry out assigned duties effectively. 2+ years of experience minimum.

SUMMARY OF DUTIES
Plan, coordinate, and assist in the facilitation of TTS model across the state. Train staff to implement the TTS model with program participants via telehealth or in-person (where available). Gather and analyze data relating to training and effectiveness of the model in the programs.
Establish rapport and trust with participants/families, providing empathetic encouragement and support. Partner with staff and participants to deliver TTS model. Empower participants to create a family service plan focusing on their family's goals and dreams; and develop and maintain a list of collaborative contacts and community services pertinent to participant's' needs. Provide information and referral services to community resources and agencies and help to facilitate participant connection to needed resources. Attends case conference with other disciplines or team members. Maintains confidentiality at all times. Participates in program staff meetings, meets with supervisor on a regular basis, and attends training and other staff development activities especially on topics related to cultural/ethnic diversity, child abuse and neglect and child development/behavior management. Maintains case recording, mileage logs, and other miscellaneous reports in a timely fashion.
