    Full-time Description Summary/Objective: The Clinician is responsible for the provision of comprehensive clinical services to clients and their families. This includes the provision of individual, family, group therapies, crisis intervention, advocacy, assessment, planning, discharge planning and interdisciplinary teamwork. Clinicians are responsible for formulating and implementing appropriate treatment strategies from clients and their families on their assigned caseload and for timely and satisfactory documentation of all treatment efforts. Clinical therapy involves responsibility for providing psychotherapeutic services to patients with behavioral healthcare needs to improve their level of functioning. The duties are performed as part of an interdisciplinary mental health team. The work is performed under the supervision of a senior licensed staff person who provides the incumbent with oversight and guidance in: assessment and evaluation, diagnosing utilizing DSM, collaborative and individualized treatment planning, completing psychosocial histories and progress notes, individual and group psychotherapy and consultation. The incumbent does related work as required. Essential Functions: Performs intakes, interviews new clients to obtain information and identify behavioral healthcare needs and refers to psychiatric medication management staff if appropriate. Participates in interdisciplinary staff meetings to discuss new cases and formulate treatment plans. Provides psychotherapy to individuals, couples and groups using evidence-based therapeutic methods, to assist patients in treatment mental health symptoms, increasing level of functioning in multiple domains, and achieving more satisfying lives. Makes referrals to a variety of health, social, vocational and educational agencies and facilities to arrange for supportive services for patients. Provides emotional support needed by clients involved in the process of growth or change. Refers patients to psychiatric medication management staff for medication review as needed. Prepares and maintains a variety of records and reports such as patient treatment plans, case progress notes, referrals, letters and treatment and discharge summaries. Contacts client families and other collaterals to obtain information to assess and monitor client progress and treatment as appropriate. Provide crisis intervention and services as necessary. Required to have a valid NYS driver's license and ability to drive legally. Compliance with OMH MHOTRS regulations. Actively develops clinical skills through clinical supervision and CEU trainings. Demonstrates adaptability in response to change and effectively manages additional responsibilities as needed. Participates in clinical on-call rotation. Performs additional duties as assigned to support CHJC and ensure the effective operation of the Community Clinic. Special Requirements: Work Environment: Office environment will require occasional travel to conferences and/or meetings. May have contact with service recipients that are agitated and/or are in crisis. Some risk involved working with service recipients with mental illness. Works in an office setting with a controlled temperature environment. Occasional exposure to inclement weather conditions may occur depending upon assignments. Equipment: This position is required to maintain a working knowledge of related office equipment including personal computers and printers, audio-visual equipment, telephone systems, copiers, fax machines, etc. Physical Demands: Must be able to sit, stand, walk, lift, carry, push/pull, climb, bend, and stoop. Must be able to perform fine motor skills, read, and type. Must be able to sit for long periods of time. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Position Type/Expected Hours of Work: Full-time, flexible working hours to meet the needs of the program. Due to the nature of the position, the Clinician will need to be on-site. Regular work-from-home arrangements are not permitted. Travel: Travel may occasionally be required throughout service area (Jefferson, Lewis, Oswego, and St. Lawerence County). Required Education and Experience: Licensure by the New York State Department of Education as a Licensed Mental Health Counselor (LMHC) or Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW or LCSW-R) or Licensed Psychologist. (Permit authorized with provided supervision.) Requirements Skills/Abilities/Knowledge: Strong organizational abilities and office skills are essential for this position. This person must also have excellent people skills and be a team player. Must be able to work with privileged information in an unbiased and ethical manner. Ability to work with diverse populations. Must demonstrate effective and excellent oral and written communication skills and the ability to work cooperatively with others. Must be detail-oriented and possess excellent time-management skills. Requires self-direction, tact, diplomacy, and the ability to be clear, courteous, and professional. Able to react to change productively and handle other duties as assigned. Additional Eligibility Qualifications: Must maintain privacy of confidential information. Successful completion of routine pre-employment requirements as mandated by the New York State Department of Health, Office of Mental Health and Office of Children and Family Services along with ongoing clearances and exclusion checks. These requirements are to include employment health requirements, reference checks, criminal background checks/fingerprinting, State Central Registry clearance and your eligibility to work in the United States. Summary/Objective: To help maintain the child/youth in the home, school, and community, the Home Based Crisis Intervention (HBCI) team delivers intensive crisis intervention to children and families within Jefferson County. The HBCI Program aims to serve children/youth ages 5 to 20 years at imminent risk of psychiatric hospitalization or admission to a group treatment program, or at risk for a rapid readmit to such settings. Under the supervision of the HBCI Clinical Supervisor, the HBCI Interventionist responds to referrals and provides treatment to address the clinical and complex needs of the child/youth and their family/caregivers. In addition to providing direct crisis intervention and stabilization, the Interventionist provides case management services linking the family with additional short and long-term available services. Essential Functions: The major responsibilities of this position include but are not limited to the following: Respond to each referral within 48-hours. Meet with each youth and family/guardian to discuss program options, service provision, and alternative service options. Conduct all admission paperwork and ensure all necessary consents/releases are signed. Collaborate with already established providers such as care managers, therapists, and psychiatric providers. Provide intensive crisis intervention and supportive counseling to the youth and family. Participate in team meetings. Work with community agencies, organizations, and systems for long-term planning and support of the youth and families served. Make referrals to mental health services and other programs as needed. Provide thorough documentation regarding all case related activities including but not limited to; face to face contacts, phone calls, case conferences, clinical interventions, noted incidents, etc. Represent CHJC as a liaison with various private as well as public service providers and resource agencies. Complete all CHJC and New York State mandated reports and forms in a timely fashion. Continuously promotes the program and provides community education. Participates in an after hours on-call rotation. Other duties as assigned or requested.     SCOPE OF ROLE: The Licensed Senior Mental Health Clinician will play a pivotal role on one of the newly launched innovative Safe Options Support (SOS) teams, that will provide comprehensive care to street homeless or subway dwelling individuals. The multi‐disciplinary SOS team will consist of a Team leader, Licensed Senior Mental Health Clinician, Care Managers, a Registered Nurse, and a Peer Specialist. The team will support program participants in the community through the application of the highly acclaimed, Critical Time Intervention, evidence‐based, model of care. The Senior Mental Health Clinician's role will involve community outreach on the streets and subways, coordinating participants needs before and after their move from street to home, enhancing their daily living skills, providing supportive counselling, and advocating on their behalf when faced with discrimination or healthcare inequities. Member choice, harm reduction, non‐coercion, flexibility and person‐centered core elements are essential to this team. The SOS teams will continue to follow participants for several months after housing placement to ensure their stability, independence and wellbeing in their new community.     The Intensive Permanency Therapist will provide culturally competent, trauma competent clinical treatment services (individual, group and/or family) according to the highest level of professional standards and best practices for youth and children in care at HSVS. Base Rate of $23.26 per hour, plus fee for service. Session Rates: 60 minute session: $50.00 45 minute session: $37.50 30 minute session: $25.00 15 minute session: $12.50 Primary Responsibilities and Essential Functions of Positions: Provision of individual therapy to an identified caseload of children and youth in foster care or group homes; including: Counsels' individuals and/or families related to psychological or emotional stressors such as substance abuse, self-regulation, or family settings Establishes permanency and treatment goals for children and youth, utilizing historical and current available information in collaboration with the multi-disciplinary team. Assist children and youth to develop skills as defined by permanency and treatment goals. Provides emotional and behavioral support and therapy focused on healing and permanence. Provide individual therapy, substance abuse counseling and treatment, trauma-competent therapy, family and group therapy, transitional support with a focus on healing and permanence. Promoting integration with community resources and skill building. Providing psychoeducation, wellness education and all other assigned training presentations. Taking lead on mental health and trauma learning and training for the agency. Communicating with family, case planning staff, all relevant stakeholders and medical practitioners. Maintains contact with collateral resources where indicated, including but not limited to referral sources, schools, foster care providers, and hospitals. Explains the function of behaviors to group and foster home staff. Uses interventions drawn from evidence-informed behavioral health strategies. Structures interventions to understand maladaptive behavior and increase developmentally appropriate responses that lead to healing opportunities. Interviews clients to obtain information concerning mental health history or other pertinent information. Observes clients to build understandings and insights of children and youth's behaviors. Provides individualistic and holistic interventions Discusses progress toward goals and permanence with client, such as self-regulation, self-awareness, stress, or substance abuse. Consults with medical doctor or other specialists concerning treatment plan and amends plan as appropriate Responsible for the engagement, retention and outreach to clients. Completes all required documentation, including but not limited to session notes, treatment plans, psychosocial summaries, billing documentation, and client correspondence. Refers client to supportive services to supplement therapy and permanency plans. Conducts monthly family outreach searches using search platforms to help reconnect youth with family. Performs other related duties as assigned, within the scope of the role. Qualifications Master's degree with a minimum of 2 years' experience in children's mental health, addiction, and/or foster care/child welfare/juvenile justice. Required LMHC, MHC-LP or LMSW. Have a valid driver's license and dependable transportation. Have experience working with children (preference given to those with experience working with children with special needs).     Job Description Within the administrative structure of the SASi Article 16 clinic and the requirements of New York State for the profession of Licensed Clinical Social Worker, the Licensed Clinical Social Worker (LCSW) shall develop and deliver professional services to individuals who are Intellectually Disabled / Developmentally Disabled (ID/DD) who require and desire such services. Responsibilities and Illustrative Duties: Assesses each individual's level of function as related to their interpersonal relationships, feelings, self-esteem, stress, grief, socialization skills, support systems, and potential for participating in counseling. Develops a clinical impression and determines a mental health diagnosis in accordance with generally accepted standards of professional practice. Develops a Treatment Plan with specific goals and objectives that include the nature, frequency, duration and modalities of the recommended therapeutic interventions that are related to the Clinic Service Request and Assessment. Initiates a therapeutic program using counseling techniques to facilitate individual change, self-exploration, self-understanding and independence. Applies psychological and social theory to develop strategies for counseling services. Adjusts counseling approaches or styles to meet each individual's cognitive and personality traits. Initiates or attends team meetings for individuals on their case load as needed, to discuss areas of concern or to develop, modify, or discontinue therapy. Works with family members and significant others to best meet the individual's interests and needs. Documents all services provided following procedures determined by SASi administration and/or other certifying or oversight organizations. Maintains a working knowledge of developments in the field of social work counseling by attending conferences and in-service programs, and by maintaining appropriate membership in professional organizations. Meets with the supervising Licensed Clinical Psychologist to discuss clinical cases and treatment methodologies. Provides in-service training to staff, as needed, on topics that relate specifically to a diagnosis or emotional/social situation involving an individual or group of individuals. Familiarizes him or herself with the policies and procedures of OPWDD, the Article 16 clinic, and the Agency. Assists the Quality Assurance department as needed with completing assessments and writing reports pertaining to incidents of abuse Provides temporary grief counseling as needed, within the agency, for both staff and people supported in the event of a death. Qualifications: Candidates must have received a Master's Degree in Social Work from an accredited educational institution and be currently licensed in New York State as a Clinical Social Worker. Sasi will provide the supervisory oversight to candidates who do not currently hold the “R” certification, for Psychotherapy Privilege. The candidate must be willing to work toward this certification. Basic computer skills preferred.
