Job Description
PRIMARY FUNCTION/PURPOSE:
Provide direct casemanagement services including assessment, counseling, crisis intervention, referral, and advocacy to ensure an effective social service support plan is in place to enable the successful relocation of families to permanent housing.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Utilizing the Department of Homeless Services (DHS) - Client Assistance and Re-housing Enterprise System (CARES) complete family's intake assessment, service plan/independent living plan, housing, next step conferences/transfers, incident reports, progress notes, etc.
Utilizing the DHS Client Assistance and Re-housing Enterprise System (CARES), Review accessible information from Human Resource Administration, Administration for Children Services, Department of Probation, Department of Homeless Services, etc.
Utilizing the DHS Client Assistance and Re-housing Enterprise System (CARES) ensure that all information is updated in system in a timely fashion as required by DHS.
Provide advocacy and referral services to ensure clients receive appropriate benefits, entitlements and services thereby enhancing their transition to permanent housing while minimizing the length of time families are homeless.
Coordinate services, as needed, to stabilize the family unit, deal with problems of substance abuse, or improve housekeeping, parenting and money management skills so the goal of permanent housing may be realized.
Assess and identify the clients' needs, formulate appropriate plans of service, monitor and document progress of service plans and document all significant interactions with or on behalf of the clients to ensure the clear, effective delivery of service to families and the provision of high-quality social services that are in compliance with the State, City and Agency regulations.
Develop and maintain appropriate helping relationships with the families to assist them in making the most of their residency at the facility and facilitate their active involvement in the achievement of service goals.
Provide appropriate individual counseling and crisis intervention services to families to help stabilize and minimize the negative impact of the family's displacement.
Maintain familiarity with off-site and on-site program resources for homeless families and refer families to appropriate resources to ensure that they receive the most beneficial services.
Facilitate groups, as designated, to address family issues, enhance life skills, and assist families in achieving self-sufficiency and a successful transition to permanent housing.
Coordinate client services provided by other social services staff to ensure that families receive the necessary and appropriate services in a timely fashion and to ensure compliance with State and City regulations.
Facilitate effective communication among staff, both within the Casework Unit and with other staff members. Ensure the timely and accurate transfer of relevant information to administrative staff, such as client admission and discharge and changes in public assistance cases to ensure the smooth functioning of the family and compliance with numerous regulatory agencies.
Perform other duties and special projects as assigned and required including serving on-call for a one-week period each quarter.
Assist in the transferring clients in and out of the program, when required.
SECONDARY RESPONSIBILITIES:
Facilitate effective communication with other staff within an interdisciplinary context, participate in and contribute to staff meetings and the development of policies and procedures.
Participate in training and staff-development activities to acquire and maintain both interpersonal and operational skills necessary for successful job performance.
PRINCIPAL RELATIONSHIPS:
Frequent interaction with diverse clients to assess and respond to their needs.
Collaborative working relationships with and guidance to, volunteers who provide assistance in the facility.
Regular contact with other program and Social Services staff to ensure team delivery of consistent high-quality services.
MINIMUM QUALIFICATIONS:
Education: Bachelor's degree preferred and/or an Associated degree in Social Work, Sociology, Psychology, Counseling, Criminal Justice or Human Services.
Experience:
a) Bachelor's degree (preferred) with a minimum of one (l) year of experience in social services, counseling, crisis intervention, family services, preventive services or housing services preferred.
b) Associate degree (considered) with a minimum of two (2) experience in social services, counseling, crisis intervention, family services, preventive services or housing services preferred.
Experience facilitating groups for homeless adults, specialized knowledge of substance abuse issues and treatment, and family dynamics treatment is necessary.
Bilingual language ability highly desirable.
Excellent written and verbal communication skills.
Excellent interpersonal and computer literate in Microsoft-Office.
OTHER REQUIREMENTS (including Physical Demands):
May be required to work long hours and a varied schedule. • Travelling throughout various locations in the New York City area.
Climatic issues of temperature variance and ventilation association with resident facility impact delivery of services.
Lifting and moving objects weighing up to 30 pounds several times a week.

What You Will Do

We require:
MSW from an accredited School of Social Work; NYS licensure or permit as applicable.
Minimum 1 year in medical, geriatric, primary care, or community health social work experience with brief screening, goal‑setting, and resource navigation preferred.
Motivational interviewing or equivalent brief counseling; familiarity with evidence-based screening tools (eg, PHQ/GAD screening); knowledge of accessible and relevant caregiver resources (respite, benefits, mental health); EMR proficiency; strong communication and teamwork.

The Social Work Reentry Department, within the Division of Correctional Health Services (CHS), is seeking a Social Worker. Duties & Responsibilities
This full-time position reports either directly or through a Program Director to the Clinical Director of Psychiatry in the Division of Correctional Health Services and is responsible for the clinical oversight, management, and innovative development in specific mental health treatment areas, including standard mental observation (MO) units, specialty MO units and general population clinic settings in one of the 12 jail facilities in New York City. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city.
The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. Depending on the treatment area, s/he manages patient admissions and discharges, monitors and engages in treatment, oversees quality assurance, manages on-going training and education for treatment-area staff, and performs other managerial administrative tasks including liaising with DOC. In addition to direct supervisory and management responsibilities, the Clinical Supervisor also provides direct care to a small caseload of complex patients and is responsible for the accompanying medical record documentation. Responsibilities Include:
Formulate and implement procedures for operating, monitoring, and evaluating mental health programs and services in assigned area of responsibility
Develop and lead staff training programs and exercises to maintain a culture of learning and clinical excellence

Minimum Qualifications
1. Possession of a valid license and current registration to practice in a mental health discipline issued by the New York State Education Department (NYSED); and
2. Three (3) years of progressively responsible experience in a health care setting or regulatory agency administration, with an emphasis on development and evaluation of mental health delivery services; one (1) year of which must have been in an administrative, managerial or supervisory capacity.

MAJOR FUNCTIONS:
The CaseManager is responsible for providing counseling and casemanagement services related to substance abuse, legal, mental health, medical and vocational/educational issues. THE OPPORTUNITY
The Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court along with community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, casemanagement, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant's life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling.
SIJC seeks a CaseManagement Coordinator for the borough's Supervised Release Program (SRP). Reporting to the SRP Director of Practice, the CaseManagement Coordinator is responsible for employing quality assurance measures to ensure the program is adhering to the program model and employing best practices aligned with organizational values, including a commitment to Operational Excellence. The CaseManagement Coordinator will provide direct supervision to Supervising CaseManagers and group supervision to CaseManagers and Senior CaseManagers. Additionally, they will ensure that casemanagement staff regularly engage in program-specific training and receive consistent and appropriate task supervision. The CaseManagement Coordinator will facilitate effective communication between this team and across all other SRP teams to ensure that the program builds on successes and proactively addresses challenges.

THE OPPORTUNITY
The Staten Island Justice Center (SIJC) seeks to re-engineer the experience of criminal court in Staten Island, New York, by providing judges and attorneys meaningful alternatives to bail, fines, and jail sentences. Operating out of Richmond County Criminal Court along with community-based offices, SIJC is a team of social service providers, court-based resource coordinators, mental health practitioners, compliance specialists, and others who seek to improve the quality of justice. Supervised Release offers an alternative to jail by providing pretrial supervision, casemanagement, and voluntary social services to people charged with misdemeanor and felony offenses, and in doing so, uses an arrest as a window of opportunity to change the direction of a participant's life, avoiding the harmful effects of incarceration. Program participants are monitored to ensure their appearance at court dates and mandatory programming, and receive referrals to services like job training, drug treatment, and mental health counseling.
SIJC seeks a CaseManagement Coordinator for the borough's Supervised Release Program (SRP). Reporting to the SRP Director of Practice, the CaseManagement Coordinator is responsible for employing quality assurance measures to ensure the program is adhering to the program model and employing best practices aligned with organizational values, including a commitment to Operational Excellence. The CaseManagement Coordinator will provide direct supervision to Supervising CaseManagers and group supervision to CaseManagers and Senior CaseManagers. Additionally, they will ensure that casemanagement staff regularly engage in program-specific training and receive consistent and appropriate task supervision. The CaseManagement Coordinator will facilitate effective communication between this team and across all other SRP teams to ensure that the program builds on successes and proactively addresses challenges.
This position leverages significant direct practice and supervisory experience to support staff in the execution of their responsibilities.
