Job Title & Role Description:
The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes.
Skills Required:
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.).
High school diploma or GED required.
A valid drivers license, auto liability insurance, and reliable transportation to travel within the assigned territory.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Experience with motivational interviewing, trauma-informed care, and care coordination.
Strong interpersonal communication skills with the ability to engage patients and team members effectively.
Ability to prioritize tasks, manage schedules, and work independently in an unstructured environment.
Multi-lingual skills are a plus but not required.
Prior home care experience is beneficial.

The incumbent will assess for completion of suicide risk screening and assessments that include the assessment of suicidal/homicidal thoughts and behaviors.
* The incumbent may also be consulted regarding allegations of abuse and neglect and will follow the established protocols including reporting and educating interdisciplinary team members on mandatory reporting to Elder and Child Protective Service agencies as necessary and clinically appropriate.
* Acts as the subject matter expert on VA and community resources. The incumbent will collaborate with other service providers to ensure Veterans receive appropriate services. The incumbent is responsible for educating the Veteran and/or caregiver of available services and assisting them in establishing appropriate referrals based on the Veterans preference or that of their surrogate decision-maker.
* The incumbent will enter all Veteran/family contacts in the electronic record system using appropriate formats and templates and are to enter consults and make assessments based on their Advanced level of licensure.

The Children Therapist is an exciting position where no one day is the same! This staff member delivers solution-focused and strength-based services to a caseload of youth, kindergarten to 12
th
grade. Therapists have office space within the school setting, and also provide in-home services to the family unit. After hired, School-Based Therapists spend their first two weeks in a supportive training environment, then transition to shadowing peers in the field. Therapists provides supportive services include: intakes, assessments, treatment planning, individual counseling, and psychosocial group interventions. This position reports to a Program Director.
Location and Schedule:
The Children Therapists provide services during school hours and this is a Part Time 20 hour a week work schedule. We offer competitive wages!
Responsibilities:
Provides therapy and clinical case management for full caseload of students and/or families.
Develops and maintains intakes, assessments, and treatment plans.
Maintains accurate, thorough, and current documentation in accordance to SB&H clinical standards.
Collaborate with school administrators, social worker, and teaching personnel to meet the needs of individual clients and school programs.
Coordinate community resources alongside clinical staff and other human service professionals.
Leads psychosocial education groups and implements other effective prevention programs that reduce risk factors for students.
Qualifications:
Must be at least 21 years of age
Master's degree in the Behavioral Health field (Psychology, Counseling, or Social Work)
1 year of full-time behavioral health experience preferred
Level I fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire (as required per program requirements)

* Assumes responsibility for the provision of substance use and co-occurring services to Veterans who are referred for substance abuse specialty care treatment, including group, individual, couples, and family educational and therapeutic psycho-social and behavioral interventions.
* Provides telehealth services to Veterans throughout the NAVAHCS catchment area, including Clinical Video Telehealth CVT, VA Video Connect (VVC), and Telephone, in addition to face-to-face services.
* Provides a range of treatment services, as a trained and experienced professional in the treatment of patients who suffer from substance use disorders, either alone, or in combination with other DSM-5 disorders, including but not limited to individual, couples, and group treatment, which includes psycho educational as well as psychotherapeutic groups.
* Develops treatment plans, assessments, progress notes, and all other documentation collaboratively to meet The Joint Commission's standards adhere to SUDS/IOP Standard Operating Procedure.
* Facilitates referrals based upon Veteran's needs and eligibility, using general knowledge of Veterans' benefits and services relating to special programs, service-connected compensation, and non-service-connected pension.
* Serves as a consultant to other medical center staff and trainees and assist in their formulation of the psychological characteristics and appropriate treatment expectations regarding their patients.

At EntireCare, you will be guaranteed 1 on 1 treatment time with all patients and in most locations will have a technician to assist with any duties the therapist deems necessary. The practice of Physical Therapy shall include assessment, treatment, planning, implementing, education, communication and demonstration of leadership to address the needs of patients and their families as well as to foster a positive working relationship among professionals and ancillary staff. Responsibilities
Care Management
* Participates in patient care conferences with other related health services, patient, and family to review patient's progress and evaluate goals.
* Develops appropriate plan of care that include goals and home instructions in conjunction with other team members, patient and family.
* Delegates selected treatment to supportive personnel with assumption of responsibility for care of patient and continuing supervision of supportive personnel.
Documentation
* Plans specific measures to meet assessed learning needs and documents in patient record.
* Recognizes and addresses patient, family, and appropriate others in the learning needs of the individual.
* Presents information clearly and in a manner appropriate to the level of patient and family understanding.
* Implements the treatment plan and evaluates and records the patient responses to the treatment sessions.
* Participates in the development of individual discharge plans that are responsive to patient/family concerns.
* Able to revise/update plan based on assessment.
* Completes documentation per department policy.
Education
* Acts as a resource person to peers within the rehabilitation department, fielding questions, mentoring, or providing educational opportunities.
* Acts as a liaison/partners with other educational institutions/groups/organizations to enhance professional development.
Patient Care
* Evaluates the patient by performing chart review, interpreting tests and measurements appropriate to discipline and patient diagnosis.
* Performs treatments and patient care activities in a timely manner following appropriate protocols and guidelines.
Compliance/Safety
* Responsible for reporting any safety-related incident in a timely fashion through the Midas/RDE tool; attends all safety-related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.
* Stays current and complies with state and federal regulations/statutes and company policies that impact the employee's area of responsibility.
* If required for the position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.
* Completes all company mandatory modules and required job-specific training in the specified time frame.
* Completes charges in a timely manner and according to proper procedures.
* Meets productivity goals according to departmental standards.
* Responds to new orders promptly in compliance with department policy.
* Adheres to regulatory and organizational standards affecting patient care and safety as well as all hospital and departmental.
* Attend 75% of monthly department meetings; responsible for 100% of department meeting content including reviewing minutes, asking questions as needed, and reviewing handouts.

Education
Bachelor's degree- Required
Certification & Licensures
Current license in the state of Arizona- Required
BLS (American Heart Association) - Required upon date of hire.
Certification specific to profession- Preferred
Employee will obtain and maintain appropriate clinical competencies for the roles assigned by their departmental leader.
Experience
Clinical documentation experience- Required

Computers and other electronic devices are utilized across the organization and throughout each department.
