    Job Description Intake Case Manager CSP (Full Time) Leads in the development of comprehensive health service plans, participates in the daily organizational and treatment /service planning meetings, and monitors client responses to health coordination and prevention activities. Provide case management services to psychiatrically disabled adults and their families in order to maintain and improve their level of functioning in the community. Orients new clients and their families to CSP (Community Suport and Recovery Services Program) and health home services. EDUCATION AND TRAINING REQUIRED: Bachelor's degree preferred in psychology, social work, or other human services related field. AA degree or equivalent education level in the human service field required. One-year experience in community mental health preferred. Ability to communicate effectively orally and written. Ability to work as a member of a multi-disciplinary team WORKING CONDITIONS: Competently and consistently completes work accurately and thoroughly. Ability to be a self-starter, attends to multiple tasks simultaneously, work with minimal supervision, and continually seeks new/better methods to organize/complete work. Effectively accomplishes duties by working with others, including team members, other CCA staff and/or outside agencies. Regularly available for work, reliable and on time for all things scheduled. Ability to move within the building to meet with clients, team members or accessing supplies to attend to client needs. Ability to transport clients and perform home visits. Demonstrates competency in clinical/technical or administrative skills, procedures, methods, and tools required to perform the job successfully. Able to perform in a fast-paced environment and meet or exceed expected service levels including health home targets. Position Summary: The Client Advocate will serve as the first and main point of contact for our disabled veteran clients and their families in the Veterans Practice Group. The role involves managing a high volume of client calls and correspondence (email & regular mail), providing timely updates on their cases, answering questions, and ensuring that all communications are clear, professional, and supportive. A successful candidate will be responsible for ensuring that forms and casework are completed, case notes are accurately updated in CCK's case management system, and all client questions and concerns are addressed in a timely fashion following CCK's policies. The 988 Call Center is a crisis answering service that responds to calls, chats, and texts from individuals experiencing mental health and/or substance use crises. This call center is connected to the BH Link - The state's walk-in behavioral health crisis center that serves Rhode Islanders statewide. As such, 988 Crisis Counselors can assist service recipients in facilitating entry to BH Link, to be seen in person by an experienced clinical team that provides comprehensive assessment and referral services. The 988 Crisis Call Center also coordinates with local mobile crisis response teams, dispatching available mobile crisis clinicians to the community in order to meet people where they are. The services provided emphasize client-centered care in the least restrictive possible settings. Roles and Responsibilities: Working on site at the 988 Call Center in East Providence. Answering calls, chats, and texts alongside Crisis Counselors (through the 988 Lifeline and the 414-LINK line). This will include: Modeling empathy and professionalism to Crisis Counselors while effectively establishing clinical rapport with service recipients in a brief period. Routinely assessing for risk of suicidality (and other risks to safety) with callers experiencing mental health and/or substance use crises. Identifying best outcomes, including the coordination of wellness checks and/or the dispatch of mobile crisis response teams.
