    Job Description Clinician - Healthy Transitions (Full-Time) Starting Salary Range ($70k-$80k) The Healthy Transitions (HT) Team at Community Care Alliance is recruiting a clinician to support our transition-aged participants (15-26 years old) who struggle with serious emotional/ mental health concerns & co-occurring disorders, and who have not typically benefitted from traditional models of treatment and services. Priority populations for HT programming include young people experiencing their First Episode of Psychosis (FEP) or serious mood disorders, and those who have previously experienced residential placement/ youth aging out of the foster care system. The clinician works closely with other team members, including a case manager, nurse, vocational specialist, and psychiatrist. The HT clinician provides individual, group, and family therapy to a caseload of 15-25 people. The ideal candidate for this position is: Energetic, relentlessly hopeful, but grounded in reality Able to thrive in a dynamic team-based work environment Organized, and able to balance direct service and documentation tasks within a flexible weekly schedule Comfortable providing services in office/home/community settings (including transporting clients if needed) Knowledgeable about the continuum of healthcare and social services available to youth/ young adults Familiar with the needs and treatment of persons with serious mental illness Able to translate participants' needs and goals clearly into recovery plan goals and objectives and summarize participants' progress in communications with external providers. Recent MSW graduates, especially those with prior experience in bachelors-level mental health positions, are strongly encouraged to apply. Benefits: Flexible work schedule and environment.     Job Description CCBHC Mobile Crisis Bridge Clinician (Part time) Community Care Alliance is looking for a part-time master's level clinician, QMHP certification preferred, to work in a new cutting-edge program responding to crisis in the surrounding community. In this position you would have the opportunity to be working with an innovative team including, the program director, law enforcement, case managers, peer specialists and community mental health workers. This type of community based crisis triage is the new frontier of responding to mental health and substance use crises.     Clinician - RISEN (Full Time) Community Care Alliance's Social Health Services division is actively seeking a compassionate and skilled Clinician to join our RISEN (Rhode Island Systems, Engagement, and Navigation) Program. The RISEN Program is designed to stabilize individuals who are at risk or currently experiencing homelessness by connecting them to critical housing, behavioral health, and supportive services. Grounded in the Housing First model, RISEN emphasizes person-centered care to promote long-term housing stability and recovery. Position Summary:Coordinate with local housing authorities, shelters, community outreach programs, and other entities serving unhoused/unstably housed to connect to Home Stabilization/other services (e. g. , behavioral, basic needs, medical, legal, etc. ) QUALIFICATIONS:Independently licensed clinician - LICSW, LMHC, Master's level LCDPExperience working with individuals who have co-occurring mental health and substance use issues, as well as housing instability/homelessness. The employee will assume full responsibility for the cost and efforts of maintaining all forms of licensure, certification, and credentialing documentation as required in their job description or cited on any Action Form as a qualification for hiring, job promotion, or monetary increase. Support individuals to access the appropriate level of behavioral health treatment, including but not limited to outpatient, community-based, detox, inpatient, residential care, and crisis triage. For those engaged in behavioral health treatment, support individuals to adhere to recommendations. Connect individuals to recovery supports in the community, including recovery housing as appropriate Complete all required paperwork in HMIS and the Electronic Health Record. Work with the Housing Care Manager to create a culturally appropriate case plan for each individual, also focusing on diverse intersectionality of oppressed sub populations. Provide transportation as needed. Maintain a time allocation record Work collaboratively with a diverse team to achieve goals. Any other duties as assigned by supervisory staff. Maintain all Agency records under Agency, State, and Federal requirements. WORKING CONDITIONS:Approved driving record and ability to transport clients.
