Job Duties:
* Increase access to health services by identifying populations in need using various outreach methods
* Performs assessment based upon the initial reason provided for patient outreach (e.g., no care with primary care provider during the current year, health risk assessment completion, inappropriate emergency department use, recent hospitalization, a gap in routine care, and social need, etc.) and provides or connects to resources relevant to the identified needs.
* Coaches and motivates patients to engage in primary care to effectively manage their chronic disease, engage in preventive care as appropriate, and avoid inappropriate care settings (e.g., emergency department use for non-emergent concerns).
* Actively collaborates with other members of primary care teams to communicate patient needs, special patient arrangements, and areas of further support.
* Perform Social Determinants of Health (SDOH) screenings and address issues identified to eliminate barriers to care as appropriate.
* Provide outreach and engagement services remotely, on-site at Health Centers, or through other community-based settings, including emergency departments or other access points.
Education/Experience:
* Minimum of one year experience (two years preferred) in direct patient/client service exercising one or more CommunityHealthWorker (CHW) core roles (providing culturally appropriate health education, care coordination, coaching, and social support, individual and community assessment, conducting outreach, etc.)
* Completion of Louisiana's CommunityHealthWorker Training Program (CHWTP) and certification process or a substantially similar training/certification program, preferred. (May be completed after hire)

Job Summary:
The CommunityHealthWorker participates as a member of the inter-disciplinary care team (ICT) to coordinate care for members.
Essential Functions:
Engage with the member in a variety of settings to establish an effective, professional relationship. Settings for engagement include but are not limited to; hospital, provider office, community agency, member's home, telephonic or electronic communication
Accompany members to appointments and other social service encounters when necessary
Coordinate logistics to support members' care plan goals and interventions - reminders, transportation, and childcare arrangements
Verify eligibility, previous enrollment history, demographics and current health status of each member
Contribute to assessments by gathering information from the member, family, providers and other stakeholders
Contribute to the development and implementation of the individualized care plan based on member's needs and preferences, reporting information to the Case Manager

Assist with empowering the member to manage and improve their health, wellness, safety, adaptation, and self-care through effective care coordination
Assist with the provision of health education and wellness materials as directed by the Case Manager(s) or Team Lead

Education and Experience:
High School Diploma or General Education Diploma (GED), is required

What You'll Do

How You Will Fulfill Your Mission
Assist the Health Physics Manager in ensuring compliance with Federal, state, and local regulations by implementing and administering various aspects of the Radiation Protection Program.
Conduct periodic health physics surveys, hazard assessments, and calculations, for ionizing and non-ionizing radiation.
Evaluate radiation use requests to develop radiation use authorizations for all uses of radioactive material, X-ray machines, radiofrequency systems, optical/UV sources, and laser operations.
Prepare technical survey reports to document whether the radiation source is being used safely, the radiation use authorization is effective, and regulatory requirements are being met.
Collect, analyze, and report on the results associated with radiation exposure rates, contamination levels, and airborne radioactivity levels.
Ensure compliance with program and regulatory requirements for sealed and unsealed radioactive material, including inventory verifications, leak testing, storage, handling, and disposal.
Ensure compliance with requirements for machine-producing radiation equipment, including state registrations, if applicable, and shielding verification surveys.
Develop, update, and provide initial radiation safety and periodic training to radiation workers and ancillary staff. Ensuring that all radiation workers have the appropriate level of radiation safety training and refresher training.
Conduct quality assurance and control of analytical equipment operation and calibration.
Facilitate the dosimetry program by ensuring radiation badge application criteria is being met and that the handling, distribution, and receipt of badges are performed according to established procedures. This includes interpreting radiation exposure data and ALARA review for all monitored employees.
Provide radiation dose assessments for radiation incidents and assist with ALARA investigations.
Prepare, pick up, and transport radiation sources and perform radioactive waste management, including decay in storage.

Routinely visit and call referral sources to maintain quality relationships.
Inform referral sources of changes in services and contracts as well as achievements, building or equipment upgrades, goals met, and activities taking place at the nursing home.
Assist the Internal Admissions person in making the transition into our facility smooth and positive.
Assess possible admissions to deem appropriateness for placement.
Communicate with marketing team, referral sources, residents, residents' representatives, and the facility to coordinate smooth admission and/ or re-admission into our facility.
Work with nursing team for clinical approval or valid reason for denial. Request further information upon the request of the clinical team. Ex: wound care notes, therapy notes, respiratory notes, behavior details, etc.
Focus on facility census goals- meet and maintain them.

Log, track, and trend referrals in the appropriate application.
Address trends for denial and see if there is any accommodations or solutions the facility can come up with to expand the cliental, we are able to serve.
Ensure clinical and financial approval of referrals before notifying referring party of acceptance.
May assist with completion of admissions paperwork with the resident or their representative(s).
Inform the Interdisciplinary Team (IDT Team) of anticipated admission and obtain room number.
If in the facility, check resident's room for readiness prior to arrival: cleanliness, odors, bed made, working call light, water pitcher and overbed table present, all light bulbs working, soap and paper products stocked, walls, floors, and ceilings in good repair, etc.
May coordinate or participate in facility tours with potential admissions/referrals.
Follow up with new admissions or their responsible party to ensure satisfaction shortly after admission and thereafter (until resident becomes acquainted with new environment).
Maintain open communication lines with residents and their representatives after completion of the admissions process. Help refer them to the appropriate staff member for issues or concerns. Also, a friendly call to give an update on their loved one is impactful.

CHARACTERISTICS OF WORK
This is a professional level position involving the provision of social services to the seriously mentally ill. This individual is responsible for providing specific, measurable, and individualized services to each person they serve with a focus on maintaining the individual in the community and identifying and accessing needed services.
EXAMPLES OF RESPONSIBILITIES
Maintains documentation as evidence that all duties are completed in accordance with program requirements contained in the Department of Mental Health Operational Standards.
Maintains proper training, continuing education, qualifications, and other certifications required for this position as written in the Department of Mental Health Operational Standards.
Monitors consumer progress with face-to-face contact, according to consumer's level of functioning
Collects data and prepare consumer histories based on interview with consumer, family, and collateral contacts
Provides follow-up monitoring of psychotropic medications
Determines full range of social /medical /psychiatric needs
Locates and refers consumer to appropriate agency or facility for service
Cooperates with representative of agency to which referral is made
Maintains contact with other service agencies
Maintains case records of work assigned
Serves as public relations person for community support services
Provides home visits as necessary to monitor consumer
Provides a minimum of 6 hours /day of face-to-face direct service to consumers
Provides transportation for consumers as needed

QUALIFICATIONS
Bachelor's degree in a mental health related field.
Must receive DMH certification as a Community Support Specialist.
