As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW's work in a team - based structure and spend the majority of their time in the community engaging directly with members. Interesting in learning to work with medically complex patients who may be experiencing significant addiction and / or behavioral health conditions is important for this role. Expect to spend about 50% of your time in the field visiting our members in their homes or in long-term care facilities, in the King, Pierce, or Snohomish Counties, WA region. You'll need to be flexible, adaptable and, above all, patient in all types of situations. Scheduled: Monday - Friday with some evening and weekend hours to attend community events. If you reside near King, Pierce, or Snohomish counties, you will have the flexibility to telecommute* as you take on some tough challenges. Position Summary: As a Homeless Services RN at HealthPoint, you'll be a key member of our team, providing patient-centered care across various settings, such as permanent supportive housing (PSH) residence, clinic, via telehealth, or out in the community. Compensation is dependent on skills and experience. Your contribution to the team includes: * Conduct trauma-informed outreach to people, in-person, face-to-face. * Perform independent visits at PSH site, operating under standing orders. * Partner with providers to deliver high-quality, compassionate direct patient care. * Facilitate connection to care at primary and specialty health settings and coordinate patient care with clinic teams and social service organizations. * Collaborate with our integrated health team partners in social work, care coordination, mental health, dental, pharmacy, behavioral health, etc. to provide holistic support. * Promote substance use disorder (SUD) treatment in a client-centered manner. * Use EPIC EHR, follow protocols and guidelines, accurately document patient information. * Demonstrate active listening, empathy, and cultural humility in every interaction. * Critical thinking, problem solving, time management, organization and teamwork. Must have's you'll need to be successful: * Minimum BSN five (5) years of experience or 15 years equivalent and advanced degree or certification required. * Degree in Nursing from an accredited college or university. * Unencumbered Washington State Registered Nurse license required. * National Certification in Ambulatory Care or similar type certification desired. * Beginning level of Word, Excel and Outlook required. * Experience with electronic medical records required. Proof of vaccination for COVID-19 is required, prior to start. HealthPoint does not accept the Johnson & Johnson COVID-19 vaccine as proof of vaccination. Identify appropriate housing and complete housing applications. Perform street-based outreach services to people who are homeless and their families. Provide referrals to shelter and services. Distribute food, hygiene supplies, tents, tarps, sleeping bags, bus passes and gas vouchers. Identify needs and provide resources for mental health, substance recovery, educational programs, and financial and housing assistance services. Obtain program resources, all reports/record keeping, assessments to determine eligibility, and paperwork. Make multiple contacts as needed and provide basic needs items to engage clients. Maintain ongoing knowledge of resources available to the homeless within the community. Attend program meetings as assigned. Other duties as assigned. Qualifications Qualifications: Bachelor's or associate degree in social or behavioral sciences preferred. One-year relevant experience in mental health related field. Experience in the provision of case management services that demonstrate the ability to work with the mentally ill homeless population. WA State credential/license Strong analytical, interpersonal, written, and verbal communications skills. Ability to prioritize deadlines, work independently, take initiative, and maintain confidentiality. Valid driver's license and proof of insurance. A working vehicle may also be required.
