Under the supervision of the CommunityHealth Outreach Manager, and Certified Diabetes Care and Education Specialist or Registered Nurse, the CommunityHealthWorker (CHW) II is responsible for assisting patients navigate and access services within the community to address social determinants of health. Act as a bridge between the community and healthcare system to access preventive health services for disease prevention, disease progression, disability, and other health conditions. Promote health and well-being by providing culturally appropriate health education, such as assist patients in self-management of chronic disease conditions, and medication adherence.
This position will work at our Merced Suites and Northview Sites.
Full-time schedule, with required flexibility to work Monday - Saturday, 8:00am - 7:00pm, as they represent GVHC clinic outreach efforts and community events.
Compensation: $23.02 - $24.17 an hour
Essential Duties and Responsibilities
* Provide patients with culturally appropriate health information on health topics including but not limited to nutrition, chronic disease management, preventive care, and others, and assist patients in self-management of chronic disease conditions to prevent disease progression and co-morbidities.
* Perform basic health screenings, including, but not limited to blood pressure, and health risk assessments.
* Assist patients navigate the healthcare system by scheduling appointments, linkages to internal services and programs, coordinating with providers, and facilitating access to other necessary services.
* Assess patient's health needs and provide linkage to high quality healthcare, including addressing social determinants that impacts their health.
* Facilitate meetings on health care topics and provide education at clinic sites, and build partnerships with local organizations.
* Provide individual counseling, motivational interviewing, and support to help individuals manage their chronic conditions and health lifestyle.
* Model commitment to high quality healthcare by engaging in quality improvement initiatives and projects, such as identifying and addressing care gaps, and by identifying, developing, and testing best practices for an improved outcome.
* Ensures documentations are accurate and in compliance with regulatory requirements and accreditation standards. Assist in the collection and facilitation of information or data needed by programs, including but not limited to participant tracking and appointment scheduling.
* Follows department expectations regarding productivity and programmatic requirements.
* Participates in mandatory staff meeting, work groups, trainings and update meetings related to the program to build and maintain current knowledge base of awareness and prevention in the specific area of health addressed by the project.

Physical Demands
The employee must occasionally lift 40 pounds and/or move up to 100 pounds (on wheels). Specific vision abilities required by this job include close vision and the ability to adjust focus. Must be able to hear staff on the phone and those who are served in-person, and speak clearly in order to communicate information to clients and staff. Must have vision with or without lenses adequate to read memos, a computer screen, personnel forms and clinical and administrative documents. Must have high manual dexterity. Must be able to reach above the shoulder level to work, must be able to bend, squat and sit, stand, stoop, crouching, reaching, kneeling, and twisting/turning.

Education/Experience Requirements
Minimum Qualifications
* Strong knowledge of community resources in the patient's service area, related to patient health needs.
* Working knowledge of and experience with computers.
* Work independently with minimal supervision.
* Communicate fluently in spoken and written English and other alternate language commonly spoken by GVHC patients. Pass alternate language assessment on an annual basis.
* Written communication skills in English adequate to draft reports, memos, business correspondence, etc.
* Effectively organize and present information in group settings, both formal and informal.
* Pleasant and effective communication skills; strong interpersonal/communication skills.
* Work effectively both independently and collaboratively; self-starter.
* Travel to other locations as requested.
* Flexibility in working hours, which may include weekends and evenings.
* Valid California driver's license, vehicle insurance, acceptable driving record and reliable transportation.
Education/Experience
* High school diploma or equivalent.
* Minimum two (2) years' experience working in community education, or health care setting.
* Bachelor's degree in Public Health, Health Science, or related field, preferred.
* Successful completion of a CommunityHealthWorker formal accredited training program, such as from a college or other education institution.
* As determined by site need, completion of Diabetes Prevention Program (DPP), Diabetes Self-Management Education and Support (DSMES), or Chronic Disease Management training.
* Medical terminology and/or background preferred.

OPTIMUM:

Job Summary:
The Health Services Coordinator serves on the health services team supporting the overall operations of the department through a variety of administrative and clinical functions as directed by the Director of Health Services (DHS). The Health Services Coordinator preserves dignity and promotes independence for each resident while providing care and services according to each individual service plan and in accordance with MBK policy and procedure.
Duties & Responsibilities:
Conduct and coordinate assessments / evaluations of potential residents and make recommendations for admission in accordance with current rules, regulations, and community policies and procedures that govern resident assessment.
Draft initial individualized Service Plans and update as needed.
Review service plans with responsible parties and Executive Director as requested by DHS.
Ensure continuity of the assisted living residents' total care regimen.
Under the direction of the DHS, provide training and education as needed on a range of essential topics including competent medication delivery, acceptable treatments, safety protocols, emergency procedures, accurate record, and state requirements to provide the best possible resident care.
Perform all assigned duties accurately and timely including required documentation.
Serve as a medication technician, if needed.
Provide coverage of job duties within the department during absences, either through assistance in finding coverage, or personal completion of duties.
Check vital signs as directed and look for signs that health is deteriorating or improving.

The Health Services Coordinator serves on the health services team supporting the overall operations of the department through a variety of administrative and clinical functions as directed by the Director of Health Services (DHS). Educational Reimbursement Program: Eligible employees may be reimbursed for career-related course work up to a maximum of $850 per fiscal year. Selection Plan
Applicants who meet the minimum qualifications will go through the following examination process:
* Written Exam: The civil service written exam is a multiple choice format. If the written exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
* Oral Exam: The oral exam is a structured interview process that will assess the candidate's education, training, and experience and may include a practical exercise. The oral exam selection process is not a hiring interview. A panel of up to four people will determine the candidate's score and rank for placement on the eligible list. Top candidates from the eligible list are referred for hiring interviews. If the oral exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
* Written & Oral Exam: If both a written exam and an oral exam is administered, the written exam is weighted at 60% and the oral exam is weighted at 40% unless otherwise indicated on the announcement. Candidates must achieve a minimum rating of 70% on each examination in order to be placed on the eligible list.
* Rate-out: A rate-out is an examination that involves a paper rating of the candidate's application using the following criteria: education, training, and experience. Candidates will not be scheduled for the rate-out process.
Note: The rating of 70 referred to may be the same or other than an arithmetic 70% of the total possible points.

TYPICAL DUTIES
Assists various behavioral health professional and paraprofessional staff by providing a wide variety of services in the areas of community behavioral health programs.
Assists in teaching basic behavioral health coping skills to clients and families, particularly in areas of utilizing the resources of the children's, adult and older adult systems of care; may assist in teaching basic behavioral health concepts of support, relationships, crisis management, medication management, vocational involvement and related areas.
Provides outreach to locate persons who may need additional support and services to maintain and develop optimal functioning in the community; may assist in providing follow-up counseling as to treatment options and resources available; learns to make referrals as appropriate.
Assists in interviewing clients, gathering, and assembling related information and preparing reports; maintains appropriate records and files. May obtain information from clients on non-clinical intake forms.
Participates in development of programs and delivery of services for behavioral health consumers; learns to provide information and counseling regarding rights, complaint and grievance procedures, and advocacy.
Provides basic community behavioral health education to consumers, family groups, community groups, and other interested persons.
May be required to perform basic housekeeping and maintenance tasks while caring for and assisting clients; May transport clients and assist with other community referrals or appointments. May be required to pick up and deliver supplies, food, or other program related materials.
MINIMUM QUALIFICATIONS EITHER PATTERN I Experience: One year as a Behavioral Health Outreach Worker Trainee.
OR PATTERN II Experience: One-year responsible work experience at a community-based organization providing behavioral health services to culturally diverse adults, older adults, or children.
AND FOR BOTH PATTERNS License: Possession of a valid California Driver's License.

Bereavement Leave: 3 days of paid leave for the death of qualifying family member, 2 additional days of accrued leave for death of employee's spouse, domestic partner, parent or child.
Merit Salary Increase: New employees will receive the starting salary, which is the first step of the salary range. After employees serve 52 weeks (2080 hours) on each step of the range, they are eligible for a merit increase to the next step.
Job Sharing: Employees may agree to job-share a position, subject to approval by a Department Head and the Director of Human Resources.
Educational Reimbursement Program: Eligible employees may be reimbursed for career-related course work up to a maximum of $850 per fiscal year. Eligible employees enrolled in an approved four (4) year College or University academic program may be reimbursed up to $800 per semester for a maximum of $1600 per fiscal year.
Parking Supplemental Downtown Stockton: The County contributes up to $20 per pay period for employees who pay for parking and are assigned to work in the Downtown Core Area.
School Activities: Employees may take up to 40 hours per year, but not more than eight (8) hours per month, to participate in their children's school activities.
Selection Plan
Applicants who meet the minimum qualifications will go through the following examination process:
Written Exam: The civil service written exam is a multiple choice format. If the written exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
Oral Exam: The oral exam is a structured interview process that will assess the candidate's education, training, and experience and may include a practical exercise. The oral exam selection process is not a hiring interview. A panel of up to four people will determine the candidate's score and rank for placement on the eligible list. Top candidates from the eligible list are referred for hiring interviews. If the oral exam is administered alone, it will be 100% of the overall score. Candidates must achieve a minimum rating of 70% in order to be placed on the eligible list.
Written & Oral Exam: If both a written exam and an oral exam is administered, the written exam is weighted at 60% and the oral exam is weighted at 40% unless otherwise indicated on the announcement. Candidates must achieve a minimum rating of 70% on each examination in order to be placed on the eligible list.
Rate-out: A rate-out is an examination that involves a paper rating of the candidate's application using the following criteria: education, training, and experience. Candidates will not be scheduled for the rate-out process.
Note: The rating of 70 referred to may be the same or other than an arithmetic 70% of the total possible points.
Testing Accommodation: Candidates who require testing accommodation under the Americans with Disabilities Act (ADA) must call Human Resources Division at ************** prior to the examination date.
Veteran's Points: Eligible veterans, unmarried widows and widowers of veterans of the United States Armed Forces who have been honorably discharged and who have served during wartime shall be given veteran's points in initial appointment to County service. What You Will Do to Change Lives
The Peer Recovery Coach II engages, inspires and facilitates meaningful conversations with members which assist the members to explore, create and meet their own recovery goals. Additionally, Peer Recovery Coaches respond to critical situations with high-level engagement and de-escalation skills which support a least restrictive environment for members served experiencing an emergency related to a mental health/addiction challenge.
