Purpose and Impact: The SNAP Case Manager is responsible for the day-to-day case management and system navigation of SNAP . Essential Functions: Completes staff training as required by SNAP and agency; Completes intake assessments for SNAP including all required pre and post assessments. Responsible for the day-to-day case management and system navigation of SNAP Follows up on incoming referrals and schedules clients for intakes appointments. Monitors youth and family's progress through weekly compliance calls; Facilitates SNAP curriculum within the fidelity of the model; Liaises with relevant community outreach responsibilities including, but not limited to community relations with: agencies including schools, community-based agencies, local DCF and DJJ community partners, state attorneys' offices, and Police Departments to enhance outreach; Completes data reporting requirements for intake assessments and ongoing weekly data reporting requirements to all relevant systems in a timely manner (examples include: NETMIS, JJIS, Dropbox, and ASEBA, etc.) Ensures weekly materials are prepared and ready for SNAP groups including handouts, manuals, and all other required items; Supports the SNAP Coordinator with organization of food arrangements as needed; Participates in pre-briefing and de-briefing meetings weekly with other SNAP Team members; Participates in weekly consultation calls with the Florida Network; Willingness to work nontraditional business hours including evenings when Groups are scheduled. Physical Requirements: Ability to sit or stand for extended periods of time, typically in an office or remote work environment. Frequent use of computer, telephone, and other standard office equipment. Must be able to lift and carry files, documents, or supplies up to 20 pounds. Ability to travel occasionally for home visits, community outreach, or training (as required). Visual and auditory acuity sufficient to perform detailed work, including reading fine print and hearing conversations in person or over the phone. Working at NYAP: * Generous Time off: 22 Days of Paid Time Off + 11 Paid Holidays, Half Day Friday's during the summer! * Health and Wellness: Comprehensive healthcare packages for you and your family; Paid Parental leave * Professional Growth: CEU's, ongoing training/education, student loan repayment program, and supervision hours * And So Much More: 401K and 401K Matching flexible hours, mileage reimbursement, phone allowance Responsibilities The Intensive Home-Based Treatment (IHBT) Clinician will perform duties including but not limited to: * Perform all work in a manner consistent with the National Youth Advocate Program's mission, values, and philosophies. * Actively supports, represents, and extends the organization's mission, vision, and values. * Provides appropriate individual, group, and family mental health services, case management, transportation, and other professional services, meeting or exceeding established outcomes thresholds for persons served. * Travels daily to provide community-based services to, and on behalf of, youth and families in compliance with organizational, contract, and regulatory requirements . • Completes all required documentation promptly. * Completes all required training promptly. * Consistently achieves established productivity thresholds. * Coordinate and monitor services for youth and families in the community, including, but not limited to, mental health, medical, educational, psychological, vocational, and social services. * Regularly reviews and modifies youth-based plans to ensure consistent progress and success on written plan goals. * Establishes and maintains strong relationships with assigned foster parents and families of origin. * Assists in family reunification and/or stabilization efforts with the youth's primary family or other permanent living arrangement consistent with the youth's case plan. * Provides safety assessment, crisis response, and behavioral stabilization services as required to assure the safety and stability of the youth served. * Participates in the Continuous Quality Improvement activities on a monthly/quarterly basis. * Actively participates in the weekly supervision process. * Performs other duties as requested. Minimum Qualifications * Bachelor's degree in Social Work. * Active and valid LSW licensure. * Two years of work experience working in direct service with youth and families strongly preferred. * A valid advanced professional license as required and acceptable by state or contract regulations. * A willingness to work flexible and non-traditional hours in the service of foster caregivers, families of origin, and persons served. * Capacity to be flexible and responsive to youth served, foster caregivers, system partners, and internal customers. * Capacity to remain objective and professional in all areas of job function. * Demonstrates tolerance and respect for the ideas and actions of others. * Capacity to effectively work with and be respectful and sensitive to persons from various cultures, socioeconomic, ethnic, religious, and racial backgrounds. * Proficient use of desktop and laptop computers, smartphones and tablets, printers, fax machines, photocopiers, including word processing, spreadsheet, and database programs.
