Comunilife is looking for experienced Case Managers to fill open positions for their OMH Scatter Program in Brooklyn. The Case Manager's main responsibility is to provide case management services for a caseload of approximately 22 clients. The Case Manager prepares comprehensive assessments and service plans. What You Will Do
Manages and provides full scope of advanced nursing practice for targeted patient populations. Evaluates patient responses to therapy / interventions. Ensures revision of the inter-professional plan of care as necessary to achieve quality outcomes.

Qualifications
Licenses and Certifications:
License and current registration to practice as a Registered Professional Nurse in New York State required
Certificate (license) and current registration to practice as a Nurse Practitioner in the State of New York, with a specialty in adult health, family health or gerontology required
Valid driver's license, as determined by operational/regional needs may be required

Job Description:

The Department is comprised of 70+ healthcare professionals who are responsible for providing re-entry services to individuals detained in the NYC jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city.
The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. Depending on the treatment area, s/he manages patient admissions and discharges, monitors and engages in treatment, oversees quality assurance, manages on-going training and education for treatment-area staff, and performs other managerial administrative tasks including liaising with DOC. In addition to direct supervisory and management responsibilities, the Clinical Supervisor also provides direct care to a small caseload of complex patients and is responsible for the accompanying medical record documentation. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. The service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. The service provides some of the most comprehensive and unique treatment modalities in the country, including specialty housing units for individuals with serious mental illness, a 24-hour on-call psychiatric service, robust creative arts programming, a strong DBT training program, clinics in each of the facilities, substance use treatment, integrated re-entry services, and relationships with multiple training institutions throughout the city.
The Clinical Supervisor is responsible for maintaining the clinical integrity of the specific program or housing area to which they are assigned, including ensuring that all clinicians are providing appropriate and timely clinical interventions and documentation. Depending on the treatment area, s/he manages patient admissions and discharges, monitors and engages in treatment, oversees quality assurance, manages on-going training and education for treatment-area staff, and performs other managerial administrative tasks including liaising with DOC. In addition to direct supervisory and management responsibilities, the Clinical Supervisor also provides direct care to a small caseload of complex patients and is responsible for the accompanying medical record documentation. Clinical Supervisors who are qualified for and interested in providing care on the PACE or CAPS units (specialty MO units) for individuals with serious mental illness will report to a Program Director for direct supervision.
Responsibilities Include:
Formulate and implement procedures for operating, monitoring, and evaluating mental health programs and services in assigned area of responsibility
Develop and lead staff training programs and exercises to maintain a culture of learning and clinical excellence
Serve as the clinical leader for the assigned area of responsibility and provide weekly individual and in-person supervision for all assigned mental health clinical staff; review diagnostic, treatment, and level of care decisions and documentation; ensure adherence to policies and procedures; and conduct clinical case conferences
Lead daily multi-disciplinary meetings with all mental health staff in the assigned area of responsibility, including DOC when appropriate.
Provide quality assurance and quality improvement as directed by the Clinical Director of Psychiatry
Coordinate re-entry services with the social work staff in the assigned area of responsibility
Meet regularly with the Clinical Director of Psychiatry or assigned supervisor to provide meaningful input into program design and policy formation
Personnel management of the clinical staff, with assistance from the Clinical and/or Medical Directors as needed, including recruiting, mentorship, supervision, and disciplinary procedures
20% of time must be devoted to direct clinical care, some of which may include supervision of trainees. Timely and clear documentation of supervision and clinical care is expected.
Minimum Qualifications
1. Possession of a valid license and current registration to practice in a mental health discipline issued by the New York State Education Department (NYSED); and
2. Three (3) years of progressively responsible experience in a health care setting or regulatory agency administration, with an emphasis on development and evaluation of mental health delivery services; one (1) year of which must have been in an administrative, managerial or supervisory capacity.

RESPONSIBILITIES
The Case Manager will conduct home visits, community outreach, and engagement using a culturally sensitive, trauma-informed approach to build trust with adults living with chronic medical, mental health, and/or substance use conditions.
Complete comprehensive, consumer-driven assessments to identify strengths, challenges, and service needs; develop and regularly update individualized care plans reflecting client goals, preferences, and wellness objectives.
Coordinate and monitor behavioral health, physical health, and social service supports in alignment with each clients needs and service plan.

Manage a caseload of approximately 4050 clients, providing consistent contact through home visits, phone calls, and field-based engagement in accordance with client acuity and program standards.
Advocate for and coordinate care across multiple systems: medical, behavioral health, and social service, to ensure access to comprehensive, continuous, and integrated care.
Monitor client progress toward goals, supporting self-management and helping to reduce preventable hospitalizations and emergency room utilization.
The Case Manager will collaborate closely with multidisciplinary providers and participate in joint case reviews, ensuring coordinated, goal-directed care.
Maintain detailed, accurate, and timely documentation in the electronic health record (EHR), ensuring all required clinical and eligibility documents meet standards set by the Department of Health, Office of Mental Health, Health Home program, Suffolk County Division of Mental Hygiene, and Family Service League.
Participate actively in supervision, team meetings, and case conferences, including ACM staff meetings and agency-wide meetings, to share resources, address client needs, and contribute to program improvement.
The Case Manager will maintain a positive, professional rapport with the Suffolk County Adult SPOA and other referral sources to facilitate client enrollment and smooth transitions of care.
Respond promptly to crises or urgent client needs, implementing safety plans and coordinating follow-up support.
Serve as a role model of professionalism and FSLs mission, promoting empathy, empowerment, and recovery-focused practice.
Complete all required reports and documentation accurately and within established timelines to support billing, compliance, and program outcomes.
Follow all agency and program policies and mandates, including confidentiality, safety, and quality assurance standards.

The Case Manager III (CM III), a key member of the primary care interdisciplinary team, provides services for patients with complex care needs. This position conducts patient outreach, engagement and psychosocial service assessment, assists in developing a patient-centered care plan, is the lead implementer of Enhanced Case Management (ECM) and coordinates service referrals and delivery. The case manager meets clients in home, clinic, or community as appropriate or required by the specific program/site. The CM III provides services to specific populations that have multiple complex health and social services needs and often provides care outside of a traditional health center setting, such as home visits, hospitals, supportive housing sites, encampments and shelters. In addition they provide comprehensive housing navigation support to clients.

This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.

Responsibilities
Outreach, via telephone and in person at LifeLong, community and residential sites, to patients who meet case management program eligibility criteria or are prioritized by LifeLong for this service
Proactively meet and engage with patients to build effective relationships and assess strengths and needs through use of standard intake, screening tools, and health, and social services records review
Actively involve patients and caregivers, as appropriate, in designing and delivering services, including development of care plans, assuring alignment with patients' values and expressed goals of care
Provide and facilitate referrals for internal and external resources, and collaborate with the patient to complete required applications, forms, or releases of information
Maintain a patient caseload in accordance with LifeLong standards for the specific population served or site requirements
Utilize data registries and reports to manage caseload, meet program requirements, maintain grant deliverables, and promote high quality care
Provide health education and training to patients, including but not limited to, harm reduction and disease risk-mitigation strategies that empower patients to manage their own health and wellness (e.g. overdose prevention, mitigating spread of communicable diseases)
Assist patients with accessing and retaining public benefits and insurance (e.g. MediCal, SSI/SSDI, CalFresh, General Assistance), and affordable/subsidized housing
Respectfully and routinely communicate with patients, their care team members, external partners, and identified social supports
Maintain knowledge of patients' medical/behavioral health treatment plans and facilitate utilization of services by providing resources such as accompaniment, transportation, in-home care, reminder calls etc.
Participate in team meetings to coordinate care, support patient goals, and reducing barriers to accessing services
Provide case management services to patients with multiple complex acute or chronic medical or behavioral health conditions (e.g. HIV/AIDS, Hep C, congestive heart failure, severe diabetes, severe hypertension, psychosis, pregnancy, and homelessness)
Provide general housing case management services that includes document readiness, housing problem solving, and assessments for Coordinated Entry System
Assess patients to identify cognitive and/or behavioral health needs and provide brief interventions and short-term support using standardized tools and effective approaches for patient care
Co-facilitate patient groups
Provide intensive case management to a caseload size in accordance with site or program standards focusing on a subset of the highest acuity patients
Provide specialized housing navigation services to patients who are matched to a housing resource through Coordinated Entry System
Lead crisis intervention response, de-escalation procedures, notification of the local mental health department and/or crisis response team, and follow-up care
Provide and document billable services to eligible populations that result in revenue generation for LifeLong
Advocate on behalf of patients to get their needs met and/or support patients to learn advocacy strategies for themselves.
Keep current on community resources and social service supports to effectively serve the target population
Document patient contacts/services in required data systems (EHR, HMIS etc.) according to LifeLong policy
Specific activities may vary depending on the requirements of the program and funder.

Job Requirements
High School diploma or GED
At least three (3) years of progressively responsible work or volunteer experience in a community-based health care or social work setting or at least one (1) year of experience as a Case Manager II or equivalent position or registration or certification as a Certified Alcohol and Drug Counselor by one of the two certifying bodies in California
Proficient skills using Microsoft Office applications like Word, Excel, and Outlook, as well as the ability to work in and/or manage databases
Access to reliable transportation with current license and insurance

SUMMARY
Family Service League is seeking a full-time Case Manager for the Critical Time Intervention (CTI) program in Mastic, NY. The Case Manager will work as part of an interdisciplinary team to provide Care Management services to adolescents and adults who have behavioral health needs and who have not been successfully engaged in services during or after critical times in transition. RESPONSIBILITIES
The Case Manager will assess needs and coordinate services for adolescents and adults within the full range of DSM diagnoses.
Collaborate on referrals, resources, and interventions within a multidisciplinary team that includes Master's level clinicians, Case Managers, Peer Specialists, and nurses.
Develop a person-centered care plan, share knowledge of community resources, engage individuals in skill building, offer crisis management, collaborate with other providers, assist individuals in navigating complex systems, and work with informal support.
The Case Manager will facilitate linkages and coordinate care planning with other providers of services/resources to ensure goal directed collaborative care, including care transitions.

Establish and maintain positive working relationships with others, both internally and externally.
Complete all clinical documentation in accordance with NYS Office of Mental Health in an electronic medical record.
Assess situations to determine importance, urgency and risks, and make clear decisions which are timely and in the best interests of the organization.
The Case Manager will set priorities, develop a work schedule, and monitor progress towards goals, and track details, data, information, and activities.
Provide support to clients in the community with in-person, field-based interaction, as well as in-office settings.
Provide services to clients beginning in hospital settings and spanning approximately 9 months following discharge.
Coordinate and connect to community resources.
Attend routine supervision/team meetings.
Complete training and maintain proficiency with de-escalation skills.

RESPONSIBILITIES
The Social Worker will conduct clinical crisis assessments as part of a Mobile Crisis team serving all of Suffolk County for adults, adolescents and children with the full range of DSM diagnoses, including both mental health and substance use disorders.
