Comunilife Inc. is looking for an experienced Substance Abuse Counselor for its DHS Transitional Housing program in the Bronx. The Substance Abuse Counselor completes initial and annual substance abuse assessments for all clients, participates in the development of the initial service plan and its revisions, and provides individual, family and group substance abuse counseling and advocacy and referrals to community programs as needed. The Substance Abuse Counselor monitors clients' attendance at programs and provides documentation of all contacts. Counselors will be responsible for coordinating Mental Health services for mentally ill homeless adults residing in a ten unit 24-month transitional OMH licensed Community Residence on West 173rd Street in Manhattan. The salary range for this position is between $16.50 and $17.52 per hour.
