The Clinic Quality Coordinator is responsible for supporting projects working towards specific objectives defined by the Quality Performance and Clinical Informatics Department. Working under close supervision, this position is accountable for assisting HEDIS and or Quality Measure chart reviews, physician notification of individual member gap results (EMR alerts or other means), and the pulling of accurate supplemental data for assigned member lives to achieve high quality care and exceptional health outcomes. Additionally, this position will provide support patient and provider engagement and care coordination efforts for Medicare Advantage and Commercial ACO patients attributed to Triad Healthcare Network.
Responsibilities
Provides patient and provider outreach related to Health Effectiveness Data and Information Set (HEDIS) measures and coordinates patient care, including the review of patient charts and abstract compliant documentation.
Identifies trends within offices and/or groups and communicates findings effectively to Manager, Physician Educators and/or THN Quality Performance and Clinical Informatics Department.
Assists with performing monthly analysis of physician intervention reports and other quality reports to prioritize chart review goals or meet team initiatives.
Promotes a positive working and team environment that is subject to stress due to evolving and quickly expanding business needs, tight deadlines, and heavy workloads.
Helps utilizes various Electronic Medical Record (EMR) Systems in a "read only" access to identify accurate supplemental data.
Performs other duties as assigned.

Responsibilities:
- Completes the pre-admission assessment of candidates referred by acute care case managers, therapists, physicians, or external referral sources

- Determines patient/family overall goal for rehabilitation, establishes a preliminary discharge plan and determines available insurance benefits for inpatient rehabilitation

- Facilitates daily communication with unit leadership and the rehabilitation physician to discuss potential admissions to the unit and any barriers that might exist including medical, clinical, financial, or social barriers
- Ensures accurate collection of referral, denial, and admission's data
- Seeks pre-authorization from insurance companies and communicates availability of rehab benefits to prospective admissions
- Works with financial counselors to assist our uninsured with potential access to the rehab level of care
- Ensures accurate collection of data related to the insurance authorization process
- Works with referral sources, physicians, and the interdisciplinary team to identify opportunities to improve the efficiency/effectiveness of the admission's process, improve overall communication, and improve patient outcomes
- Provides ongoing education to referral sources and staff related to admission criteria and regulatory changes that affect the rehab level of care
- Performs other duties as assigned
