As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required

The home base for this position will be the Buzz Westfall Justice Center in Clayton working with individuals in the County jail. Examples of Duties
Assess patients for substance use disorders and evaluate to determine the most appropriate level of care and treatment support needed.
Make referrals, when determined appropriate and support patient access to treatment and needed services.
Provide substance use counseling and interventions either individually or in group sessions to promote recovery and good behavioral and mental health.
Develop and implement patient treatment plans based on clinical experience and knowledge.

Minimum Qualifications
Graduation from an accredited college or university with aMasters Degree in Social Work or Counseling and a minimum of one year of related counseling experience or any equivalent combination of experience and post graduate training. Must possess a valid driver's license. Summary

POSITION SUMMARY:
The Community Mental Health Worker (CMHW) provides mental health service to residents of Prairie Mountain Health, and may be based in both community and facility settings. Bond & Specialty Insurance (BSI) provides management and professional liability, cyber, crime insurance, and surety bonds to or on behalf of businesses of all sizes. The BSI Claim Counsel team conducts thorough investigation, analysis, evaluation, and disposition of claims and claim litigation to achieve superior customer service and to optimize claim results. Travelers' Cyber Claim Team is responsible for handling claims focusing on cyber privacy and security breach incident response, computer crime, infrastructure attacks and restoration, notification to affected parties and regulators, business interruption, consumer privacy liability, and identity fraud/theft. The First-Party Cyber Claim Team partners with and supports business units including Private-Non-Profit, Public Companies, Financial Institutions, Insurance Companies, Investment Advisors, and Professional Liability. As a valued member of our team, in addition to claim, you also will provide legal advice to our underwriting colleagues and non-attorney claim colleagues, and participate in BSI claim strategic initiatives.

**What Will You Do?**
+ Follow operational policies to analyze, investigate, and resolve BSI claims of varying levels of severity with moderate supervision from claims management.
+ Prepare and present reports for management that accurately reflect loss development, potential/actual financial exposure, reserve adjustments, coverage issues, and claim and recovery strategies.
+ Retain, monitor, and manage outside counsel actively utilizing litigation management plans and budgets.
+ Communicate with underwriting on significant claim exposures.
+ Continue to develop the ability to identify all recovery opportunities and coordinate recovery efforts with the Recovery Management Unit and Claim management.
+ Travel to and attend mediations, settlement conferences, claim conferences, field office visits, trials and depositions.
+ Refine policy interpretation/coverage analysis skills including drafting coverage opinions with case law included.
+ Strengthen the ability to negotiate settlements/resolutions, review releases and settlement agreements, including confidentiality and non-disclosure provisions.
+ Support underwriting marketing efforts, including participation in broker and account visits and risk mitigation seminars and authoring customer-focused white papers and articles.
+ Monitor marketplace conditions, conduct research, and draft reference materials to inform claim and underwriting colleagues on law impacting Travelers' underwriting and claim decisions.
+ Obtain and maintain required adjusters' licenses.

Job Description
This short-term, intensive intervention role focuses on expanding efforts to locate, notify, and engage safe, appropriate relatives and kin for youth entering foster care. Using the evidence-based 30 Days to Family model and family search and engagement practices, the Specialist works to ensure timely compliance with the Fostering Connections to Success and Increasing Adoptions Act of 2008, Senate Bill 683, and other relevant Missouri statutes.
The Specialist immediately begins identifying and engaging non-offending parents and grandparents, followed by an extensive search to locate, contact, and explore up to 80 additional maternal and paternal relatives for potential placement and ongoing support. Because foster care placements can be fragile, the goal is to identify multiple relatives or kin who can provide placement options or be part of a robust support network. Every effort is made to keep siblings together, maintain the youth's school of origin, and preserve meaningful relationships with friends and supportive adults.
Once a placement provider is identified-whether prior to the Protective Custody Hearing or later-the Specialist offers targeted support to promote placement stability. This includes assessing youth and family needs, identifying family and community resources, removing barriers to relative placement, and developing a comprehensive plan that outlines formal and informal supports available to the family.
