Position SummaryAs part of the bold vision to deliver the "Next Generation" of managed care in Ohio Medicaid, Ohio RISE will help struggling children and their families by focusing on the individual with strong coordination and partnership among MCOs, vendors, and ODM to support specialization in addressing critical needs.
The OhioRISE Program is designed to provide comprehensive and highly coordinated behavioral health services for children with serious/complex behavioral health needs involved in, or at risk for involvement in, multiple child-serving systems.
The Population Health Specialist (PHS) leads initiatives to advance health outcomes and reduce disparities for the OhioRISE population.
This role applies quality improvement science to lead efforts improving health outcomes for OhioRISE members by identifying disparities and implementing targeted interventions.
The PHS analyzes population health data to uncover trends, gaps in care, and opportunities for system-level improvement.
Designs and executes quality improvement projects to address identified needs by collaborating with diverse stakeholders, including internal carecoordination teams, hospital systems, the Ohio Department of Medicaid, Case Western Center of Excellence, and Care Management Entities.
Monitors, evaluates, and reports on the effectiveness of population health interventions to ensure measurable impact.
Serves as a quality improvement methodology subject matter expert within the OhioRISE program.
This is a full time telework position with occasional travel required (within Ohio) as determined by business need - residing in Ohio is strongly preferred.
Primary Responsibilities:Using the Institute for Healthcare Improvement model, this individual will lead population health projects to improve health outcomes for the OhioRISE populations.
Ensure timely management and continual updating of documentation to reflect ongoing changes and outcomes.
Oversee and drive consistent progress in intervention testing.
Analyze complex data sets to identify emerging trends and design, test, and evaluate targeted interventions for improvement.
Serves as a subject matter expert on quality improvement methodologies and tools.
Deliver presentations to internal and external stakeholders.
Collaborate with the Ohio Department of Medicaid, Case Western Center of Excellence, Care Management Entities, internal teams and other key stakeholders, including Medicaid managed care plans, community organizations, members/families, and healthcare providers, to develop and implement population health strategies and interventions.
Support quality improvement initiatives as needed.
Provides necessary updates as needed and supporting documents for state RFP, NCQA and EQRO reviews and proposal submissions.
Assisting in ensuring the Plan meets all regulatory requirements as outlined in the Provider Agreement and other accrediting agencies policies.
Under the direction of the Director of Population Health, develops the annual Population Health Strategy, including assessments on enrollee participation and program impact.
Ability to travel up to 10% of the time in Ohio to office and state meetings.
Must have reliable transportation.
Valid/active driver's license, and proof of vehicle insurance.

Participates in educational programs, inservices, meetings and workshops and utilizes current literature to enhance professional skills and knowledge. Attends all annual mandatory programs.
Participates in quality improvement activities as directed.
Follows standards of the Diligent Minimal Lift Program.

• Meet regulations for timely certifications/re-certifications, and signing of all orders
• Meet all requirements in working with Managed Care organizations
• Daily meetings with therapy and line staff on progress of skilled residents
• Monitors Nursing skilled documentation and provides continued education as needed
• Participates in the development and implementation of the resident assessment process, following the Minimum Data Set requirements of Federal, State and local regulations in accordance with Senior Living Communities/Wellmore policies and procedures
• Responsible for proper ADL Documentation and continued education/training

Maintains a customer information management (CRM) database using the chosen Liberty software system.

Provides required information to the SNF Admissions Office to process referrals and communicates to appropriate facility staff.
Explains cost of care involved in appropriate levels-of-care as determined or required by the admitting physician.
Responsible for the planning, coordination, and implementation of a quarterly facility marketing plan.
Participates in census development and marketing meetings, conference calls, and planning meetings as scheduled.
Conducts a monthly review of competitor census, occupancy, private rate, and service offerings, along with a SWOT analysis.
Understands and effectively utilizes the RUGs methodology and appropriately categorizes Medicare residents in RUGs system while maximizing level of placement.
Assists the facility Admission Office with notifications to all appropriate Facility departments of pending admission.

Job Summary
The Clinical Services Nurse (CSN) is responsible and accountable for clinical expertise for patients with heart failure treated with mechanical circulatory support devices, including Left Ventricular Assist Devices (VAD). The incumbent functions as clinician, educator, consultant and leader. Clinical judgements are based on syntheses of current research, concepts, principles, theories, and outcomes to solve complex problems. The CSN functions as an integral part of an interdisciplinary team, contributing to excellence in patient care, research, teaching, and in providing leadership in the organization.
Job Duties and Responsibilities
Direct Care
• Assessment of outpatient and inpatient VAD patients • Serve as a VAD resource for patients and families by providing pre-operative evaluations and counseling
• Identify patient/family education needs and priorities
• Coach patients and families to help them prepare for discharge from hospital after index VAD implant
• Provide ongoing VAD education for patients and families
• Participate in developing, implementing and evaluating patient education resources and programs
• Assess learning needs of nursing staff and multidisciplinary team; plan and implement appropriate educational forums and opportunities to address critical learning needs and skills
• Establish and maintain collaborative relationships within and across departments that promote patient safety, culturally competent care, and clinical excellence in the care of patients with VADs
• Practices collegially with multidisciplinary team including but not limited to medical staff, advanced practice providers and other members of the healthcare team
• Role model professional nursing through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, continuous learning and problem-solving skills
• Provides clinical experience in a preceptor roles for nursing students attending institutions that have contractual agreements with DUHS.
• Participates in the orientation of new nursing staff and provides expert coaching and guidance through both formal and informal one-on-one teaching with nursing staff
• Employs evidence-based clinical practice guidelines tailored to the individual
• Integrates evidence in the management of patients, families and groups
• Participate in phone triage/on call pager coverage for Duke VAD emergency pager
• Provide patient follow up via faxes, phone calls, emails and electronic medical record communication
• Complete paperwork for prescription prior authorizations in collaboration with physician and/or advanced practice provider
• Remain up to date with clinical resources and most recent guidelines pertaining to the care of the VAD population

Job Eligibility Requirements
Education
• Graduate of an accredited BSN program
License
• Current unrestricted license issued by the NCBON. AHA ACLS and BLS required.
Certification
• No specific certification required for this role
Experience
• Minimum 2 years of applicable RN experience required with 1 year experience with patients with durable VAD

As part of the bold vision to deliver the “Next Generation” of managed care in Ohio Medicaid, Ohio RISE will help struggling children and their families by focusing on the individual with strong coordination and partnership among MCOs, vendors, and ODM to support specialization in addressing critical needs. The OhioRISE Program is designed to provide comprehensive and highly coordinated behavioral health services for children with serious/complex behavioral health needs involved in, or at risk for involvement in, multiple child-serving systems.
The Population Health Specialist (PHS) leads initiatives to advance health outcomes and reduce disparities for the OhioRISE population. This role applies quality improvement science to lead efforts improving health outcomes for OhioRISE members by identifying disparities and implementing targeted interventions. The PHS analyzes population health data to uncover trends, gaps in care, and opportunities for system-level improvement. Designs and executes quality improvement projects to address identified needs by collaborating with diverse stakeholders, including internal carecoordination teams, hospital systems, the Ohio Department of Medicaid, Case Western Center of Excellence, and Care Management Entities. Monitors, evaluates, and reports on the effectiveness of population health interventions to ensure measurable impact. Serves as a quality improvement methodology subject matter expert within the OhioRISE program.
This is a full time telework position with occasional travel required (within Ohio) as determined by business need - residing in Ohio is strongly preferred.
Primary Responsibilities:
Using the Institute for Healthcare Improvement model, this individual will lead population health projects to improve health outcomes for the OhioRISE populations.
Ensure timely management and continual updating of documentation to reflect ongoing changes and outcomes. Oversee and drive consistent progress in intervention testing.
Analyze complex data sets to identify emerging trends and design, test, and evaluate targeted interventions for improvement.
Serves as a subject matter expert on quality improvement methodologies and tools.
Deliver presentations to internal and external stakeholders.
Collaborate with the Ohio Department of Medicaid, Case Western Center of Excellence, Care Management Entities, internal teams and other key stakeholders, including Medicaid managed care plans, community organizations, members/families, and healthcare providers, to develop and implement population health strategies and interventions.

The typical pay range for this role is:

This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
