Under the direct supervision of the system director of clinical pharmacy services, the Clinical Pharmacy Implementation Advisor supports the effective and efficient implementation of medication-related changes across the health system. This position will help the pharmacy department to stay agile and achieve progress through change. With deep expertise in the medication-use process from ordering through administration they evaluate and develop best practices, with a strong focus on understanding and mitigating the impact of clinical practice changes. This role collaborates closely with clinical and operational pharmacists, providers, non-pharmacistclinicians, Information Technology (IT), and other stakeholders to optimize the design and functionality of the electronic medical record (EMR), including powerplans and clinical decision support tools. The advisor serves as a key resource for system-wide updates related to medication ordering and decision support, providing education and guidance to pharmacists and other clinicians to ensure safe, effective, and seamless transitions in practice. They are connected to pharmacy implementation efforts across the network - sharing ideas, communicating clearly, and leading the department to work together as a unified team to care for those who need us.
Responsibilities:
1. Implementation Strategy & Workflow Optimization (40%) - Leads the analysis and design of new clinical workflows in collaboration with local pharmacy leaders and the pharmacy informatics team; Manages implementation strategies for medication-related solutions, including ordering processes, practice changes, and clinical decision support; Oversees development, implementation and ongoing support for pharmacy system enhancements.
2. Develops interdisciplinary education materials to support pharmacy practice changes (20%) - Ensures education materials are relevant to end users by understanding differences in local needs and gathering real-time feedback.
3. Identifies and promotes best practices with a focus on understanding and managing the impact of changes on clinical staff and conducts direct observations of workflows and activities to inform optimization efforts. (15%)
4. Actively participates in the Pharmacy & Therapeutics Committee and other governance teams and partners with the pharmacy informatics team to support system-wide optimization initiatives. (10%)
5. Supports Clinical Workflow (10%) - Assesses unresolved issues related to workflow, functionality, ownership, and technology; Supports workflow analysis and redesign to enhance inpatient clinical operations.
6. Serves as a preceptor for pharmacy students and residents, fostering professional development and experiential learning. (5%)
7. Maintains Regulatory Compliance & Professional Standards - Demonstrates compliance with all applicable pharmacy regulations, including USP 797, USP 800, Joint Commission standards, CMS Conditions of Participation, state laws, and hospital/network policies; Maintains current drug knowledge and clinical competency, staying informed on emerging therapies and trends; Retrieves, analyzes, and communicates drug information professionally and efficiently.

9. Demonstrates regular, reliable and predictable attendance.
10. Performs other duties as required.
Other Information:

* *Doctor of Pharmacy - active license in NY and/or CT*
* *Minimum 5 years inpatient hospital experience and/or PGY2 Residency or 2 years inpatient hospital experience in addition to PGY1 residency; or equivalent experience*
PREFERRED:
* Experience with multi-hospital health system pharmacy workflows, clinical systems, education and technology; Board Certification
* Knowledge in Cerner
* Good communication skills (both verbal and written), Computer skills in Microsoft Office (Word, Excel and PowerPoint), ability to multi-task and work independently.
Regular travel to seven hospital pharmacy sites across CT and NY.

Qualifications

Two years experience in an acute care hospital pharmacy is preferred for this position.

Providing prior authorization services, including insurance benefit investigation, copay assistance, foundation support, and insourcing the dispensing function of Specialty Medications. Responsibilities
* 1. Identification and qualification of patients eligible for medication patient assistance programs and foundations. Interfaces with program administrators for reimbursement assistance programs sponsored by the drug manufacturer. Tracks, documents, and reports out cost savings associated with patient assistance drug recovery programs and provides updates to pharmacy management. Provides manufacturer coupons to patients, if available.
* 2. Assists the pharmacist in all operational activities of the pharmacy in order to optimize customer satisfaction and maximize profitability of the pharmacy. The specific duties would include customer relations, computer and prescription filling, drug orders and inventory, marketing, accounting and any supportive tasks involved with workflow. Responsible for knowledge of the Policies and Proceedures (as they pertain to operations, quality assurance, goals and priorities).
* 3. Customer Service- communicate with patients in a friendly, welcoming manner with greeting provided by management. Completion of updating accurate demographic information such as address, birthdate, insurance information and allergies. Setting up appropriate pick-up or delivery times via telephone or in person. Responsible for conducting a benefits investigation to find a patients insurance if the patient is unable to provide. The "Service for Excellence" mission provided by YNHH is the main focus. Awareness of all of the "non-negotiables" (10/5 rule, no venting, library voices, AIDET and the Pledge to Service of Excellence).
* 4. Computer and prescription filling- demonstrates proficiency in cross-funtional areas of the department, including (but not limited to) QS/1, Mckesson ordering process, Epic, Epic Specialty, Emdeon, Macro Helix, INFOR and the DSS website. Responsibilities also include awareness of specific programs (MAP, Workers Comp, Columbus House, etc) and how to process prescriptions within these programs. Responsibility for regulatory awareness and reporting of medication errors through RL Solutions.
* 5. Drug Orders and Inventory - accumulate information on prescription drugs needed. Place and transmit orders daily into Mckesson. Review reports from Macro Helix and the management of our 340B inventory. Report any discrepencies to management and review weekly for prescriptions not picked up and return to stock every 14 days. Review backordered items to see if another source can be ordered. Review and verify 340B reports. Fulfillment of prescription orders in a timely and accurate manner. Maintain the cleanliness and organization of the dispensary, responsible for cycle counts throughout the pharmacy, as well as quarterly review and removal of outdated products.
* 6. Performs other tasks directed by Manager, Outpatient Pharmacy Services, or other Pharmacy Management team members.
Qualifications
EDUCATION
High School Diploma or equivalent required with additional broad training in fields such as medical office administration, pharmacy technician, social services or similar vocations generally obtained through completion of associate's degree program, or pharmacy technician program.
EXPERIENCE
1-3 years of retail pharmacy technician experience, specialty pharmacy technician experience preferred. Experience with direct patient counseling and excellent customer service skills are required.
LICENSURE
Active Connecticut Pharmacy Technician Registration without restrictions AND Active Pharmacy Technician Certification Board (PTCB) certification or equivalent within 1 year (12 months) of hire.
SPECIAL SKILLS
Strong customer service skills and independant problem solving is a requirement. Must have excellent communication skills and the ability to prioritize and multitask/change tasks frequently and quickly. Knowledge of drug products preferred.
