Provides assistance to clients of Nuvance Caregivers at Home(NHCH) , with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as directed on clientsï ½ individual care plans, within the guidelines of the certified nursing assistant (CNA) scope of practice and according to the NHCH policies and procedures.
Responsibilities:
1. Assists the client with personal care activities including bathing, oral hygiene, feeding and dressing.
2. Assists the client with exercises, ambulation, and transfer activities. This includes knowledge in use of devices such as canes, walkers, crutches, and lifts.
3. Independently assists the client with meal preparation/cooking and/or feeding. Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
4. Assists the client with medication prompts to remind the client to self-administer his/her medications.
5. Identifies and reports changes in client condition.
6. Independently provides basic homemaking and laundry assistance for client in the home.
7. Follows policy and procedure for assigned client care activities.
8. Maintains a safe and orderly work environment in the clientsï ½ homes. This includes maintaining professional boundaries and integrity of client financial and personal information in clientsï ½ homes, as well as following network/agency infection and prevention policies and procedures in clientsï ½ homes.
9. Provides companionship and engages in activities of interest to the client.
10. Assists the client with light pet care as needed, such as walking a dog, feeding a dog/cat, changing a cat litter pan.
11. Transporting the client to appointments and errands.
12. Fulfills all compliance responsibilities related to the position.
13. Performs other duties as assigned.

Summary:
To assist in providing a safe environment to assigned patients whose physical, neurological or psychiatric condition puts them at risk for harm to self/other, by maintaining visual contact at all times often remaining within arm's length of patient.
Responsibilities:
1. Keeps within visual contact of patient at all times. Remains within arm's length of patients as needed and does not leave the patient unattended.
2. Provides for the patients' safety and comfort needs according to the RN's direction, such as feeding, toileting, ambulating and cognitive stimulation alone or in combination with unit staff according to patient abilities.
3. Maintains a calm and pleasant atmosphere for the patient. Reassures the patient they are safe. Re-orients patient to time, place and situation as needed.
4. Utilizes the Safety APRN, Education staff and the Unit staff when in doubt about as to how to best meet the needs of the patient.
5. Monitors closely, tracks on paper and reports to the RN the patient's nutritional intake, behaviors, sleep and requests. Communicates to the attending RN immediately any changes in patient's cognitive, emotional or physical status.
6. Accompanies patient to all diagnostic tests.
7. Maintains patient confidentiality.
8. Avoids over stimulation for the patients, providing for patient's need for quiet rest and sleep.
9. Fulfills all compliance responsibilities related to the position.
10. Performs other duties as assigned.
Education: HS GRAD/EQUIVALENT

* 1. Clinical Care * The Physician Assistant (PA) or the Nurse Practitioner (NP) provides assessment, triage, and therapeutic management of patient/family needs throughout the disease process as an integral part of an interdisciplinary team. The NP/PA may perform procedures, prescribe treatment and medications in accordance with institutional and state guidelines, as well as, when necessary for scope within specialty or professional competency standards. In some clinical specialties, the NP/PA may have autonomous practice managing certain patient populations. * The NP/PA may also function as first or second assistant on operative cases (as long as within position scope and job educational prerequisites are in place), using fine motor skills and manual dexterity. * Other clinical job duties may be applied in accordance with departmental and operational goals, training related to profession and practice necessitated guidelines and responsibilities, inclusive but not limited to responding to emergency situations within the YNHHS parameters

In collaboration with the supervising/collaborating MD, this may be inclusive of admitting instructions, treatment plan, actual and potential side effects of treatments/medications, and discharge plan. * The NP/PA, as part of a multidisciplinary team, provides psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner.

Information Management * The NP/PA ensures appropriate documentation within the accordance of hospital and regulatory requirements and protects information of all forms, whether computer-based, paper, film, voice, or other media from unauthorized access modification, destruction, or intentional or accidental disclosure.
* 4. Quality Management * The NP/PA is expected to demonstrate a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care. * Ensures compliance with patient related healthcare regulatory requirements, standards and recommendations.

EDUCATION

EXPERIENCE
* Clinical working experience as an NP/PA in the specialty/service line is preferred but not required.
* APRNs, PAs, CRNAs, and CNMs must be appointed to the Affiliated Medical Staff of a YNHHS hospital and go through the Medical Staff credentialing process. To be eligible, evidence of current competence to practice as an APRN, PA, CRNA, or CNM must be provided. This includes obtaining appropriate reference letters from physicians and other practitioners during the Medical Staff appointment process.
LICENSURE
* Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified, when indicated for position profile DEA License.
* Pediatric position profile PALS, when indicated for neonatal care NRP Certification.
* Additional licensure, certifications and/or continuing education credits may be required during employment based on position and area of specialty. As a valued member of our team, you will:
* Conduct comprehensive health assessments, including obtaining detailed medical histories, performing physical examinations, and ordering/interpreting diagnostic tests.
* Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions.

As a valued member of our team, you will:
* Conduct comprehensive health assessments, including obtaining detailed medical histories, performing physical examinations, and ordering/interpreting diagnostic tests.
* Develop and implement individualized treatment plans in collaboration with the supervising physician, focusing on health promotion, disease prevention, and management of acute and chronic conditions.
* Prescribe medications, order therapies, and manage medical devices according to state regulations and collaborative practice agreements.
* Provide patient education and counseling regarding health maintenance, disease management, medication adherence, and lifestyle modifications.
* Evaluate and manage common acute illnesses (e.g., upper respiratory infections, influenza) and chronic conditions (e.g., hypertension, diabetes, asthma).
* Perform minor office procedures as trained and within scope of practice.
* Collaborate effectively with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated and comprehensive patient care.
* Document all patient encounters accurately and thoroughly in the electronic health record.
* Participate in quality improvement initiatives and adhere to all organizational policies and procedures.
* Serve as a resource for medical staff and participate in educational activities as needed.
* Provide age-appropriate care to patients across the lifespan, from infants to geriatrics.
* Manage referrals to specialists and coordinate follow-up care.
* Maintain current knowledge of family medicine best practices and engage in ongoing professional development.
Qualifications:
* 1-2 years clinical experience in a primary care/internal medicine setting
* Active PA or NP in family medicine specialty certification.
* Active, unrestricted CT medical license or in process of obtaining
* ACLS/BLS/PALS (as required) Certification
* DEA License
Schedule:
40 scheduled hours, M-F, 7:30am-4:30pm, 4 day work week available
Skill Set:
1-2 years of clinical working experience as a NP/PA within the given specialty/service line recommended. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process.

1.Assists the client with personal care activities including bathing, oral hygiene, feeding and dressing.

3.Independently assists the client with meal preparation/cooking and/or feeding. Must have working knowledge of special diets, diet restrictions, and techniques to maintain sanitary kitchen conditions to prevent food-borne illnesses.
4.Assists the client with medication prompts to remind the client to self-administer his/her medications.
5.Identifies and reports changes in client condition.

This individual works in collaboration with the attending supervising/collaborating physician(s) in accordance with state law and hospital regulations, hospital policy, protocols and clinical pathways and may provide autonomous team based care. * Obtains medical histories, performs physical examinations, reviews/orders diagnostic/therapeutic tests and radiologic results, formulates and manages patient treatment plans. The NP/PA may perform procedures, prescribe treatment and medications in accordance with institutional and state guidelines, as well as, when necessary for scope within specialty or professional competency standards. In some clinical specialties, the NP/PA may have autonomous practice managing certain patient populations. * The NP/PA may also function as first or second assistant on operative cases (as long as within position scope and job educational prerequisites are in place), using fine motor skills and manual dexterity. * Other clinical job duties may be applied in accordance with departmental and operational goals, training related to profession and practice necessitated guidelines and responsibilities, inclusive but not limited to responding to emergency situations within the YNHHS parameters
* 2. Patient Education & Psychosocial Support * The NP/PA will educate patients/families on care and recommended or available resources. In collaboration with the supervising/collaborating MD, this may be inclusive of admitting instructions, treatment plan, actual and potential side effects of treatments/medications, and discharge plan. * The NP/PA, as part of a multidisciplinary team, provides psychosocial support to patients, their family members, and significant others in a caring, ethical, and professional manner.

Information Management * The NP/PA ensures appropriate documentation within the accordance of hospital and regulatory requirements and protects information of all forms, whether computer-based, paper, film, voice, or other media from unauthorized access modification, destruction, or intentional or accidental disclosure.
* 4. Quality Management * The NP/PA is expected to demonstrate a high standard of moral and ethical behavior, professionalism, compassion, and commitment to patient care. * Ensures compliance with patient related healthcare regulatory requirements, standards and recommendations.
* 5. Professional Development * The NP/PA continuously engages in projects that maintain and advance professional competency. * The NP/PA serves as a clinical resource for new house staff and may provide service specific orientation pertinent to clinical education * Consistently demonstrates high reliability organizational behaviors through reporting safety events as well as near misses. * The NP/PA continuously engages in projects that maintain and advance professional competency. * Maintains all employee requirements determined by hospital policy. * The NP/PA may serve as a clinical preceptor for NP/PA students and other health care professionals and may serve as a mentor to pre-PA/NP candidates. * The NP/PA may collaborate on research, quality assurance and performance improvement projects, and assist with the formation of protocols that will be applied to clinical practice or further their educational and clinical expertise.
Qualifications
EDUCATION
Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
EXPERIENCE
Prefer clinical working experience as a NP/PA within the given specialty/service line recommended, but none required. APRNs, PAs, CRNAs and CNMs are required to be appointed to the Affiliated Medical Staff of a YNHHS Hospital and credentialed through the Medical Staff process. Evidence of current competence to practice as {an APRN, PA, CRNA, CNM} via appropriate reference letters from physicians and other practitioners must be able to be obtained during the Medical Staff appointment process in order to be eligible.
LICENSURE
Active PA Certification by the National Commission of Certification of Physician Assistants (NCCPA), Connecticut State Physician Assistant License, ACLS/BLS Certified, when indicated for position profile DEA License, when indicated for pediatric position profile PALS, when indicated for neonatal care NRP Certification. Additional licensure, certifications and/or continuing education credits may be required during employment based on position and area of specialty. ORConnecticut State Registered Nurse License, Connecticut State Nurse Practitioner License, Board specialty certification, as appropriate for position profile ACLS/BLS Certified, when indicated for position profile DEA License, when indicated for pediatric position profile PALS, when indicated for neonatal care NRP Certification. To assist in providing a safe environment to assigned patients whose physical, neurological or psychiatric condition puts them at risk for harm to self/other, by maintaining visual contact at all times often remaining within arm's length of patient.
Responsibilities:
1. Keeps within visual contact of patient at all times. Remains within arm's length of patients as needed and doesnot leave the patient unattended.
2. Provides for the patients' safety and comfort needs according to the RN's direction, such as feeding, toileting, ambulatingand cognitive stimulation alone or in combination with unit staff according to patient abilities.
3. Maintains a calm and pleasant atmosphere for the patient. Reassures the patient they are safe. Re-orients patient to time, place and situation as needed.
4. Utilizes the Safety APRN, Education staff and the Unit staff when in doubt about as to how to best meet theneeds of the patient.
5. Monitors closely, tracks on paper and reports to the RN the patient's nutritional intake, behaviors, sleep andrequests. Communicates to the attending RN immediately any changes in patient's cognitive, emotional orphysical status.
6.Accompanies patient to all diagnostic tests.
