Delaware Guidance Services for Children and Youth, Inc. (DGS) is the largest provider of outpatient behavioral health services to children and families in Delaware. In 2021, DGS clinicians provided over 54,000 direct care hours to children, youth, and families in Delaware. Over 93% of clients report improvement in their level of functioning following treatment, and therapeutic benefits derived from trauma-informed care extend to clients' siblings, caregivers, classmates, and peers.
Our client population is racially and ethnically diverse; 80% of DGS clients are Medicaid recipients and 90% have household incomes under $30,000. Many of our clients present with significant treatment challenges including learning impacts due to missed school and negative economic impact due to parental missed work. Summary/objective
Under the supervision of the Crisis Coordinator, the CARES Clinician will provide screening and assessment and intensive therapeutic intervention services to youth and families experiencing high intensity psychological and emotional problems (ie. aggressive and defiant behavior due to problems with emotion regulation, suicidal/homicidal ideation, plans, or attempts, family conflicts, etc.) while maintaining safety and implementing therapeutic treatment plans in a community-based setting. The incumbent is responsible for providing intensive, community-based therapeutic intervention as a bridge to other intensive services for approximately 90 days. In addition, the incumbent is responsible for therapeutic documentation of cases from intake through discharge.
Essential functions:

Use critical thinking and risk management skills when assessing and developing treatment goals that allows for stabilization and effective transition to alternative services at discharge
Understand and utilize knowledge about warning signs and risk factors associated with suicide risk and risk of harm to others
Develop comprehensive skills in planning for safety and providing evaluations, diagnosis and therapeutic intervention services to children, adolescents, and their families.
Plan and administer solution-focused and trauma-informed interventions to maintain safety, build skills, and address problem solving, communication, and relationship conflicts.
Case Management
Communicate with and provide appropriate documentation to the DPBHS team
Communicate with appropriate stakeholders to maintain a systems-based approach to stabilization/transition of clients in services
Maintain complete, accurate, and timely documentation

• Provide specialized case management and aftercare services to clients and conduct follow-ups as scheduled

An Advanced Practice Clinician (APC) (Nurse Practitioner or PhysicianAssistant) to join the Gastroenterology team in both inpatient and outpatient settings. The APC will work collaboratively with gastroenterologists to provide high-quality care for patients with digestive disorders, including inflammatory bowel disease (IBD), liver disease, gastroesophageal reflux disease (GERD), and gastrointestinal cancers. Responsibilities will include conducting patient assessments, diagnosing conditions, ordering and interpreting tests, assisting with procedures, and managing treatment plans.
Responsibilities:
Outpatient Responsibilities:
• Conduct comprehensive patient evaluations, including medical history, physical examinations, and diagnostic assessments.
• Diagnose and manage acute and chronic gastrointestinal conditions such as IBD, GERD, celiac disease, and functional bowel disorders.
• Order and interpret laboratory tests, imaging studies, and diagnostic procedures (e.g., stool tests, liver function tests, endoscopy results).
• Develop and implement treatment plans, including medication management, lifestyle modifications, and dietary recommendations.
• Educate patients and families on gastrointestinal health, disease management, and preventive care.
• Assist in outpatient procedures such as capsule endoscopy placement, percutaneous endoscopic gastrostomy (PEG) tube changes, and hemorrhoid banding.
• Monitor and manage patients on biologic therapies or other advanced treatments for GI conditions.
Inpatient Responsibilities:
• Provide consultations and follow-up care for hospitalized patients with gastrointestinal conditions, including GI bleeding, pancreatitis, and liver disease.
• Assist with emergent GI cases, coordinating care with the hospital team and specialists.
• Perform bedside procedures as appropriate under physician supervision.
• Collaborate with gastroenterologists, hospitalists, and nursing staff to ensure continuity of care.
• Participate in multidisciplinary rounds and contribute to patient care planning.
Qualifications:
• Clinical experience in gastroenterology, internal medicine, or hospital medicine preferred.
• 1-3 years of experience in gastroenterology or a related field (preferred but not required).

Delaware Guidance Services for Children and Youth, Inc. (DGS) is the largest provider of outpatient behavioral health services to children and families in Delaware. We operate offices in Wilmington, Newark, Dover, Lewes, and Seaford, and we are the only statewide not-for-profit outpatient behavioral health organization for children and adolescents accredited by JCAHO. In 2020, we were honored as a Compassionate Champion for our dedication to serving youth and families using a trauma-informed approach.
Through DGS, clients and families have access to outpatient counseling, group therapy, parent-child interaction therapy, and school-based services. Our community-based service lines serve youth and families who require a more intense therapeutic protocol. DGS' 24-hour statewide Mobile Response and Stabilization Services (MRSS) dispatches skilled crisis intervention clinicians to provide immediate care to children experiencing behavioral health crises. Therapeutic options are available for a wide range of diagnoses including depression, anxiety, mood disorders, eating disorders, traumatic stress-related disorders, and suicidal ideation. In 2021, DGS clinicians provided over 54,000 direct care hours to children, youth, and families in Delaware. Over 93% of clients report improvement in their level of functioning following treatment, and therapeutic benefits derived from trauma-informed care extend to clients' siblings, caregivers, classmates, and peers.
Our client population is racially and ethnically diverse; 80% of DGS clients are Medicaid recipients and 90% have household incomes under $30,000. Many of our clients present with significant treatment challenges including learning impacts due to missed school and negative economic impact due to parental missed work. We provide care to anyone in need, regardless of insurance status or ability to pay for care. Summary/objective
Under the supervision of the Crisis Clinical Coordinator, the Crisis Clinician will provide rapid response screening and assessment and counseling intervention services to youth and families experiencing urgent psychological and emotional problems (ie. aggressive and defiant behavior due to problems with emotion regulation, suicidal/homicidal ideation, plans, or attempts, family conflicts, etc.) while maintaining safety and implementing therapeutic treatment plans in a community-based setting. The incumbent is responsible for conducting risk assessments, consulting with appropriate supervisors or DPBHS workers to make decisions about level of care needed to maintain safety, and providing short-term, therapeutic crisis intervention services for up to 30 days. In addition, the incumbent is responsible for therapeutic documentation of cases from intake through discharge.
Essential functions:
Risk Assessment
Maintain professional, trauma-informed, and empathetic interactions during crisis interactions with families and all stakeholders
Use critical thinking and risk management skills when assessing and making decisions about the least restrictive level of care
Understand and utilize knowledge about warning signs and risk factors associated with suicide risk and risk of harm to others
Develop comprehensive skills in planning for safety and providing evaluations, diagnosis and therapeutic crisis intervention services to children, adolescents and their families.
Case Management
Provide therapeutic recommendation and referrals for children, adolescents, and their families referred for assessments and follow-up services
Communicate with and provide appropriate documentation to the DPBHS team
Communicate with appropriate stakeholders to maintain a systems-based approach to stabilization of crisis situations
Maintain complete, accurate, and timely documentation
Therapeutic Crisis Intervention
Plan and administer solution-focused and trauma-informed interventions to maintain safety, build skills, and address problem solving, communication, and relationship conflicts.

HJMC is committed to increasing access to quality, culturally sensitive and cost effective comprehensive health care services for the under-served, uninsured, and under-insured individuals throughout the community.
Mental Health Therapists at HJMC offer brief counseling and psychotherapy to adults, children, adolescents and their families in order to improve psychosocial functioning. The therapist part of a multidisciplinary team that work together with medical providers to offer services through an integrated care model. This is an exempt position and the incumbent will be expected to work a minimal of 40 hours a week with evening hours.
Essential Duties and Responsibilities:
Provide individual, family and group mental health counseling for patients.
Deliver individual and small group counseling services using evidence-based or best practice models of care.
Complete required documentation promptly and efficiently, according to organizational policy.
Participate with the HJMC interdisciplinary team to provide mental health care recommendations for patients and/or multi-disciplinary team meetings with outside agencies.
Consult with primary care provider, psychiatrist, substance abuse counselor, care coordinator, and other mental health professionals as needed.
Make appropriate referrals as needed based on clinical judgment.
Maintain professional behavior and relationships with HJMC staff and referral resources.
Participate in activities that promote professional development and use clinical/ educational research to enhance therapy services.
Identify psychological, emotional or behavioral issues and diagnose disorders, using information obtained from interviews, tests, records, consultations with primary care providers and reference materials.
Support medication therapy prescribed by PCP.
Attend staff meetings and other meeting as required.
Perform general administrative/office duties as required.
Fill out and maintain client-related paperwork.
Provide crisis intervention, emotional support, resources and referrals as needed.
Screen and assess patients for common mental health and substance abuse disorders.
Facilitate patient engagement and follow-up care.

Facilitate treatment plan changes for patients who may need more intensive or more specialized mental health care.
Develop and complete relapse prevention self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the caseload.
Support the mental and physical health care of patients on an assigned patient caseload.
Closely coordinate care with the patient's medical provider and, when appropriate, other mental health providers.
Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g. problem-solving treatment or behavioral activation) as clinically indicated.
Position Requirements
Qualification: To perform this job successfully, an individual must be able to perform each essential duty satisfactory.
Licensed Clinical Social Worker or Licensed Professional Counselor and with up-to-date Delaware Licensure Required
