    Job Description Mobile Clinic Clinician Under the general supervision of the Associate VP of Grants and Community Engagement, the Mobile Clinic Clinician will respond on-site to crisis calls that cannot be resolved by the Access & Crisis Helpline telephonically in Wayne County. The Mobile Clinic Clinician will utilize the best crisis intervention practices in behavioral health, prevention and post-intervention crisis services, education, crisis stabilization services, short-term treatment, and follow up services until an individual is connected to an ongoing service provider. PRINCIPLE DUTIES AND RESPONSIBILITIES: Performs mobile crisis services in the community, including crisis intervention and de-escalation for individuals of all ages. Provides direct clinical, short-term and solution-focused counseling and services. Problem solves and resolves conflict utilizing motivational interviewing and other communication tools and techniques. Coordinates with the Crisis Traffic Coordinator via text or cell phone before reaching and entering the destination and upon leaving for safety and to be redeployed. Utilizes Crisis Theory to provide crisis counseling to people to facilitate immediate stabilization of emotions and behavior. Provides direct and indirect services involving intensive case-management services including plan development, placement services, assistance in daily living, individual and/or group counseling, and crisis intervention. Documents all crisis intervention activities ensuring timely and complete case documentation following DWIHN policies and procedures for accuracy of records and regulatory compliance. Maintains clinical documentation that is accurate and up to date, including assessments, progress notes, crisis/safety plans, and other required documentation. Refers individuals to the appropriate service provider to support clinical psychosocial evaluations in a timely manner through in-office, telehealth, or telephone visits. Clinical services may also be provided at community-based locations including schools, homeless shelters, and home-based services when appropriate. Develops and implements person-centered treatment plans for clients based upon clients' areas of need, diagnoses, and symptoms. Maintains multi-disciplinary working relationships with clinicians and other related professionals in the community. Makes appropriate referrals and provides clinically appropriate multi-disciplinary interventions and collaborations. Provides coordination and follow-up for clients to additional behavioral health and community resources. Engages and educates families about developing natural supports to increase individual and community success. Participates in community meetings, agency and system staff meetings. Collaborates with schools, law enforcement and other organizations. Performs related duties as assigned. KNOWLEDGE, SKILLS AND ABILITIES (KSA'S) Knowledge of DWIHN policies, procedures and practices. The primary goals of these programs are to promote normal development, promote healthy family functioning, support and preserve families, reunite families who have been separated, and reduce the usage of, or shorten the length of stay in, psychiatric hospitals and other substitute care settings. The family unit is the focus of treatment. The service style must support and empower families, parent and professional teamwork and community collaboration with other service providers. The Home Based Services Clinician will be responsible to perform psychosocial assessments and develop Family/Person Centered Plans. Services relating to plan goals will be provided primarily in the family home or a setting of the family's choosing. Mental Health home based services programs combine individual therapy, family therapy, crisis intervention, case management and family collateral contacts. Services provided in a home based program vary from assisting consumers in meeting basic needs such as food, housing and medical care to more therapeutic interventions such as individual or family therapy. Services begin with an assessment of the family's strengths. Referrals are received either from the Authority's Access Department or other Agency programs, via Access. Typically, the length of treatment will be less than a year. Two or three contacts per week per family is the service level more likely to occur because of family need. The Home Based Services Clinician may be required to provide crisis support services, possibly to include pre-screening for inpatient or partial hospitalization, if needed for assigned consumers and families. Qualifications Masters degree in social work, psychology or related field with corresponding Michigan licensure. Must be trained and have one year of experience in the examination, evaluation and treatment of minors and their families. Prefer one year of experience with emotionally disturbed youth in home based service delivery, crisis intervention and substance abuse treatment. Must be able to work well with staff from other agencies. Demonstrated knowledge and skills necessary to provide the appropriate care for the specific clinical needs and ages of the individuals served. Demonstrated knowledge of community and family systems, symptoms, and behaviors of individuals served, as well as understand the principles of growth and development over the life span; have the ability to assess data reflecting each individual's status; interpret the appropriate information needed to identify each individual's requirements in relation to his or her age-specific/population specific needs; and to provide the care needed by the appropriate age group - (child, adolescent and adult).
