Devereux Pennsylvania Children's Services (DPACS) serves children, adolescents and young adults with autism spectrum disorders, intellectual and developmental disabilities, behavioral, emotional and psychiatric challenges and disorders. Qualifications
EDUCATION:Graduate of an approved school of professional nursing with a B.S.N., Diploma or A.D. Currently licensed to practice professional nursing in the Commonwealth of Pennsylvania.

The CAP Case Manager provides critical case management services to beneficiaries who are at risk of institutionalization, ensuring their health, safety, and well-being are maintained through person-centered care planning and coordination of essential services.
The Case Manager works closely with families, RNs, and other interdisciplinary team members to assess needs, coordinate care, and provide ongoing support to help beneficiaries achieve the best possible quality of life.
DUTIES AND RESPONSIBILITIES:
Assessment and Care Planning:
Conduct initial pre-screening and assessments of beneficiaries and their families to evaluate medical, psychological, behavioral, financial, social, cultural, environmental, legal, vocational, educational, and other areas.
Consultation with the CAP beneficiary and primary caregiver to educate about waiver services, other Medicaid, and community resources to meet the beneficiaries' needs.
Identify needs to prevent health and safety factors to assist in maintaining community placement.
Develop and maintain individualized, person-centered care plans (emergency and disaster planning) to ensure the health, safety, and well-being of beneficiaries.
Review and update care plans at least every 12 months or when the status of the beneficiary changes.
Assist beneficiaries and families in understanding the plan of care and making informed choices.
Coordination of Services:
Link beneficiaries and their families to necessary services, equipment, and supplies to support care in the home.
Collaborate with community resources, healthcare providers, and other agencies to ensure comprehensive care delivery.
Initiate appropriate referrals and utilize community resources for planning and service coordination.
Monitoring and Follow-Up:
Provide ongoing monitoring of services through monthly phone calls and home visits, documenting observations, and beneficiary progress.
Evaluate the effectiveness of care plans and services, recommending or implementing changes as needed to achieve desired outcomes.
Maintain accurate, up-to-date case management documentation within the system, ensuring compliance with state and agency guidelines.
Counseling and Support:
Provide emotional support and basic counseling to beneficiaries and their families to strengthen their support systems.
Assist families in navigating challenges, including long-term palliative care, behavioral issues, and medical needs.
Documentation and Compliance:
Assist in obtaining documentation from medical staff to confirm the need for specific CAP services.

Document all nursing care provided in accordance with regulatory and organizational requirements.
Provide clinical oversight and supervision to unlicensed personnel.
Assist in educational and training programs as needed for unlicensed personnel.
Promote a safe and healthy work environment by adhering to safety and environmental best practices.
Assist physicians during on-site medical clinics when applicable.
Conduct medication pass observations and complete skill competency checklists on healthcare tasks delegated to unlicensed personnel.

Job Responsibilities
Reports client's social, psychological and physical behavior as indicated.
Implements and ensures compliance with standards of nursing practice that promote optimal health care delivery and meet regulatory requirements
Implements policies and procedures to ensure organizational compliance with Confidentiality regulations, HIPAA (Health Insurance Portability and Accountability Act), national accreditation agency and Continuous Quality Improvement. Attend applicable meetings as necessary.

Performs research that uses our model healthcare system to advance clinical investigation and improve patient outcomes and healthcare quality through innovation, discovery and implementation science. This will support Intermountain's Care Delivery Science concept that highlights that there is a science behind how the health care delivery system can improve patient outcomes. This will require a multi-disciplinary approach of applied health services researchers, bioinformatics, systems engineering and implementation science capabilities supporting existing clinical teams to increase the update and adoption of best-evidence based interventions. This position of a nursing research leader will be a member of this multi-disciplinary team and leverage the nursing capabilities throughout the organization on implementing best evidence-based interventions and help evaluate the impact on patient outcomes and costs.

**Scope**
The Nurse Scientist reports directly to the Assistant Vice President of Professional Practice and collaborates closely with the Office of Research to conduct independent scientific studies and research in the field of nursing. This position will sit on the Intermountain Research Guidance Council and work alongside other accomplished clinical investigators from across the system.
**Job Essentials**
Develop a portfolio of independent nursing research that provides a vehicle for achieving defined outcomes. These include increasing the grant awards, growing research publications and increasing the number and types of studies that use nurses to improve deployment of evidence-based interventions across the system.
Provide expertise in quality improvement principles, study design, development, analysis and dissemination of new knowledge and the use of evidence to guide and support clinical nursing practice.
Design nursing specific study protocols, and determine methods and procedures for data collection, data management and quality control, and determine statistical techniques for data analysis.
Build and support a team of nursing researchers
Demonstrate solid understanding of project management including development, support, installation, resource allocation, life cycle planning, cost tracking, and project risk assessment.
Collaborate with healthcare facilities and educational institutions for multi-site nursing research opportunities.
Submit grants to support the integration of nursing research and evidence-based practice.
Mentor others in the research process.
Create and participate in writing reports, research papers, national presentations, articles, and other documentation of nursing study results.
Evaluate innovative nursing research techniques.
**Minimum Qualifications**
+ Doctorate in nursing or a related field with a strong emphasis in research. If the doctorate is in a related field, a Masters or Bachelors degree in nursing is required.
+ Licensure as a registered nurse in the state of practice.
+ Experience as a principal Investigator in grant-funded nursing projects.
+ Experience in obtaining federal and/or foundation grants.
+ Experience in publishing in peer reviewed journals as first or senior author.
+ Five years of research experience in designing and deploying evidence-based interventions in routine care and analyzing the impact patient outcomes using rigorous study design methods.
+ Expertise in nursing practice, education, and research with an ability to coach others.
+ Experience using statistical research methods and their use in outcomes data reporting.
+ Excellent communication skills with experience in study coordination and collaboration.
+ Ability to meet time demands necessary to execute research.
**Preferred Qualifications**
+ Experience in a role requiring the ability to demonstrate proficiency with MS project and database applications.
+ Familiarity with statistical and informatics/bioinformatics analysis tools.
**Physical Requirements:**

+ Operate computers and other IT equipment requiring the ability to move fingers and hands.
+ See and read computer monitors and documents.
