Provides direct spiritual care to patients and families/caregivers. Documents direct services and ongoing communication. Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group. Provides consultation, education, and support to the interdisciplinary group on spiritual care. Provides for funeral or memorial services for patients as requested. Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team. Assists in supervision of spiritual care volunteers when assigned to patients/ families/caregivers. Actively participates in quality assessment performance improvement teams and activities. POSITION QUALIFICATIONS: Bachelor's or Masters degree in Divinity, Theology, Pastoral Counseling, or a related field Ordination or endorsement from a recognized faith group Hospice or palliative care experience (preferred) Empathetic, respectful, and comfortable working with individuals from diverse backgrounds Valid drivers license and reliable transportation

As part of the interdisciplinary team, chaplains use evidence-based practices to guide meaning-making, grief, and decision-making in alignment with CMS hospice guidelines. **Essential** **Functions** + Provides spiritual and emotional support to patients, families, and caregivers facing terminal illness, including grief counseling, legacy work, and end-of-life decision support. + Develops, implements, and documents spiritual assessments, interventions, outcomes, and individualized plans of care, and participates as a member of the interdisciplinary hospice team to ensure whole-person care. + Participates in weekly interdisciplinary team meetings as required by CMS, collaborating with medical and psychosocial staff to integrate spiritual care into the overall hospice plan. + Responds to caseload assignments and referrals, initiating pastoral visits regardless of religious affiliation, and offering appropriate spiritual care based on patient and family needs. + Assists patients and families with resources for transition, bereavement, and loss, while identifying and referring ethical concerns appropriately. + Offers support for grief, moral distress, vicarious trauma, and compassion fatigue, and facilitates family and group dynamics during emotionally intense situations. + Appropriately and professionally documents all spiritual care activities in the electronic medical record in accordance with CMS and organizational standards and may participate in quarterly memorial services and veteran honoring processes. + Shares responsibility for on-call coverage, ensuring availability for crisis response and urgent spiritual care needs. The letter must be mailed, emailed, or faxed directly from the endorsing/recommending organization.
