Coordinates services including aftercare planning and makes referral recommendations when necessary. Maintains regular communication with referral sources, legal systems, and outside providers. Assesses healthcare equity for current and future patient needs. Conducts educational seminars and lectures to patient population in a professional and competent manner. Facilitates individual, group and family counseling sessions. Assists with the collection of random urine drug screen samples from patients as needed. Participates in clinical supervision, department meetings and trainings as required. Protects the confidentiality of patient information. Performs other related duties as assigned. Qualifications Education and Experience: Master's degree or above from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in human services) or other related field which includes a practicum in a health or human service agency preferably in a drug and alcohol setting. Bachelor's degree from an accredited college with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in human services) or other related field and one-year of clinical experience in a health or human service agency, preferably in a drug and alcohol setting. Must hold a professional license through the PA Department of State or be credentialed through PCB at the time of hire or be actively working towards licensure or certification. A PCB credential must be obtained within two years of hire or licensure within 4 years of hire. Knowledge of Substance Use Disorder (SUD) treatment and/or ASAM criteria.
