About the Role
We're looking for a builder - someone who wants to take a messy zero-to-one commercial motion and turn it into something that works, fast. As our first Head of Provider Growth, you'll own provider adoption end-to-end: testing channels, refining targeting, knocking on clinic doors with our first reps, and scaling the system once we know it works.
This role is perfect for someone who has built growth engines before (in startups, agencies, or their own companies), loves rapid experimentation, and wants to have massive impact in a short amount of time. What You'll Do - Impact in Your First 3-6 Months
Own the growth target. Build and execute a plan to reach meaningful PA volume across provider settings.
Run fast experiments. Test messages, channels, outreach methods, and install flows to identify what moves the needle fastest.
Get your hands dirty. Shadow field reps, visit clinics, talk to staff, learn the real workflows, and refine our pitch based on firsthand insight.
Target smarter. Partner with our Head of Strat/FinOps to map specialties, geos, and practice types with the highest PA density and frictionless install paths.
Create momentum. Build a simple but effective engine: outbound sequences, enrichment, list building, mini‑landing pages, CRM flows, funnel dashboards.
Produce early proof points. Deliver data that shows what works, what doesn't, and where we should double down.

Position Overview
The Relief Residential Counselor provides support and counseling services to an adult population with psychiatric, substance abuse and/or HIV-AIDS related disabilities. Relief Residential Counselors advocate for and respond to clients' treatment and rehabilitation needs. All agency services are provided from the philosophy of social rehabilitation, which integrates teaching daily living skills in a safe and healthy environment.
Primary Duties and Responsibilities
Perform intake interviews and assist with client treatment plan development, implementation and discharge planning.
Assess the clinical needs of resident clients.
Evaluate clients for potential crisis situations and determine appropriate intervention.
Conduct individual and group counseling sessions as assigned.
Monitor and document clients' progress toward achievement of individual goals.
Record clinical notes and hourly rounds to meet Medi-Cal standards and licensure requirements.

Facilitate and supervise clients with residential house chores, meal preparation, and group activities.

Maintain professional boundaries with clients and staff.
Maintain agency 30 hour per year staff development training requirement.
Ability to work at a San Francisco worksite, effective post Covid.

Minimum Qualifications

Job DescriptionMission Statement: The Birch House Foundation provides wrap-around services to individuals with disabilities so that they may have choice and control over their own lives. Our Supported Living Services will provide Regional Center Consumers with the tools to support developing the agency needed to make meaningful choices in their lives including securing natural supports, community involvement, housing of their choice, and establishing meaningful relationships. Position SUMMARY :
The Direct Support Professional (DSP) is responsible for providing individualized support and training to adults with developmental disabilities in their own homes and communities. DSPs promote independence, inclusion, safety, and quality of life in accordance with Title 17 regulations, agency policies, and person-centered principles outlined in each participants Individual Program Plan (IPP) and Individual Service Plan (ISP).
Essential Duties and Responsibilities:
Assist participants with activities of daily living (ADLs) such as cooking, cleaning, hygiene, and shopping.
Document daily services and progress notes accurately and timely in accordance with agency and Regional Center requirements.
Encourage skill-building and self-determination through consistent coaching and positive reinforcement.
Follow agency policies for medication assistance (if applicable), emergency preparedness, and safety procedures.
Implement ISP goals designed to promote independence in areas such as communication, self-advocacy, budgeting, recreation, and transportation, etc.
Participate in ongoing trainings, supervision, and staff meetings as required by Title 17 and agency policy.
Report special incidents, behavioral concerns, and changes in participant condition immediately to the supervisor.
Support participants in maintaining a safe, clean, and healthy living environment.
Support participants with scheduling and attending medical appointments, classes, and community activities.
Uphold participant rights, dignity, privacy, and confidentiality as outlined in Title 17 and HIPAA.

Required
Qualifications:
Must be at least 18 years old.
High school diploma or GED required; some college or relevant coursework preferred.
At least 6 months experience working with individuals with developmental disabilities is preferred.
Valid California drivers license, proof of insurance, and reliable transportation.
Ability to clear DOJ/FBI background check, TB test, and any other licensing requirements.
Current CPR and First Aid certification (or willingness to obtain within 30 days of hire).
Must have a working cell phone and/or tablet for use of Quick Solve Plus Mobile APP
Skills and Abilities:
Ability to engage in de-escalation techniques during a crisis experienced by the participant.
Ability to maintain professional boundaries and manage challenging behaviors calmly.
Ability to work independently and as part of a team.
Flexibility to work varied shifts, including weekends, evenings, and holidays.

Physical Requirements:
Must be able to lift up to 25 lbs.

Under the direction of the Care Coordinator, Assistant Program Manager and Program Manager , the Direct Support Worker works with adult learners with intellectual and developmental disabilities to ensure that they have access to the support, encouragement, tools, resources, and knowledge to lead happy fulfilling lives. The Direct Support Worker works with individuals and their families to develop individualized personal goals, and then establish and monitor action plans to help them reach these goals. Our skilled staff will provide you with the training you need to feel comfortable and confident in all aspects of the role.
ESSENTIAL FUNCTIONS
* Supervise and train participants on the skills needed to perform specific tasks, routines, and personal life activities by applying a systematic process to improve participant's ability to set goals, take action, and maximize strengths.
* Assist participants in reinforcing essential life skills and/or teaching new skills.
* Coach participants to stay on task, and cultivate a support network between participants, other coaching team members, family and community members.
* Supervise, monitor, and assist participants as necessary to maintain healthy, safe environment, and maximum participation in program activities, on site and in the community.
* Provide support to clients with basic living skills which includes toileting, hygiene care, food prep, feeding assistance, dressing, chores, and other activities that are fundamental to daily living.
* Assist with planning, organizing, developing and implementing program activities for both large and small participant groups.
* Implement IPP goals and behavioral intervention plans as directed by Program Managers and Care Coordinators.
* Documents any unusual or special incidences with participants via Special Incident Report ("SIR"), and coordinates with Program Manager for reporting of suspected abuse.
* Provide updates to parents regarding participants' progress and needs.
* Work with Easterseals Northern California team members and community resources to enhance quality of program and services provided.
* Procure and prepare daily snacks for participants and ensure clean-up of work/activity areas.
* Provide transportation to participants (must be 21 years of age to operate company vehicles).
* Conduct weekly inventory of program supplies and inform Program Manager of inventory needs.
* Must adhere to all federal, state, and local laws/legislations as applicable as well as HIPAA laws and regulatory agencies
QUALIFICATIONS
Minimum Education, Experience & Training Equivalent to:
* High school Diploma or GED required.
* Must be at least 18 years of age (21 years to operate company vehicles).
* One year experience working in a team environment coaching and/or teaching adults with intellectual and/or developmental disabilities a plus but not required.
Knowledge, Skills & Abilities:
