Overview
Under the direction of the Director of Assisted Living and Care Management, the RegisteredNurse is responsible for providing for the health care needs of Duncaster residents in their apartments. This position functions as a professional caregiver who is accountable for assessing, planning, implementing, and evaluating all aspects of resident care, including physical, psychosocial and environmental needs.
Essential Functions
Admits residents to receive Assisted Living services and complete essential documentation to include MD orders, chronic & stable letter, medication competency, etc.
Develops the plan of care for the resident and instructs the assisted living aide of services to be provided.
Completes data collection or assessments as necessary based on the resident's condition but not less frequently than every 120 days.
Responds promptly to changes in resident condition to include communication with physician or practitioner and family or responsible party as indicated.
Refers to appropriate professionals or agencies whenever the condition necessitates ensuring that if the resident's condition is no longer chronic and stable, services of a licensed home care agency are engaged, or other appropriate arrangements are made.
Manages medications and other treatments to include verifying orders, pre-pouring or working with pharmacy managing pre-pour, administering, and documenting; communicate with MD or practitioner when indicated.
Responds to calls for aid to include assessment for injury, completion of documentation and communication with appropriate parties, and adjustment to plan of care if indicated.
Provides accurate and timely and regular documentation on 24-hour reports and resident chart of interventions, changes in condition, referrals to other services, notifications of MD and/or family, etc.
Utilizes standard precautions at all times and reports potential exposure or hazardous condition to supervisor.
Supervises provision of care by Assisted Living Aides.
Demonstrates skills specific to providing care to residents affected by dementia.
Assumes administrative responsibilities within the department to include electronic medical record charting, staff scheduling, and ordering medical supplies
Performs other duties as assigned.
Education and Experience
Associate or bachelor's degree in nursing is required.
Current RN License in the State of Connecticut is required.
ANA Certificate in Geriatric Nursing is preferred.
A minimum of 1 year of full-time clinical experience in nursing is required.
Experience with MatrixCare or other electronic medical records is preferred.
Comprehensive knowledge of principles of nursing, physical assessment skills, and geriatric nursing care, preferable in outpatient services.
Strong organizational and administrative skills.
Basic knowledge of human relations skills necessary to deal effectively with the geriatric residents, their families, and Assisted Living staff.
Environment and Working Conditions
Ability to focus on task needs and perform intellectual executive functions as related to documentation, charting, care device operations, etc.
Ability to execute intellectual tasks and duties under conditions of high demand and distraction.
Ability to complete the essential functions of the position with or with adaptive personal devices (e.g., hearing aid, corrective lenses).
Ability to operate equipment and technology required for the position.
Able to wear personal protective equipment.
Must be able to communicate effectively.
Able to assist with the evacuation of residents under emergency status to the best of their physical ability.
Able to function independently with integrity and work effectively with residents, personnel, and support agencies.
Able to constantly stand or walk to provide resident care and assistance.
Able to frequently push and/or pull a force up to 30 pounds including residents weighing up to 250 pounds in wheelchairs and geri-chairs.
Able to reach overhead, bend, and carry or lift 50 pounds occasionally and 35 pounds frequently.
Able to frequently transfer and support residents weighing up to 250 pounds while positioning, and transferring residents to beds, toilets wheelchairs, etc. Additional employees or mechanical assistance may be used.
Able to frequently reposition residents weighing up to 250 pounds in bed for assistance with treatments, hygiene, and comfort needs. Additional employees or mechanical assistance may be used.
Hours
Full-time, 2nd shift: 3:00pm - 11:00pm. 34 hours per week, Every other Weekend
Pay
$41.00 per hour + shift differential (additional $2.00 p/h weekday evenings & $3.00 weekend evenings)

Essential Duties and Responsibilities:
Serves as a Staff Development Educator to ensure that educational needs are met throughout Masonicare Health Center.
Demonstrates advanced clinical knowledge in geriatric nursing through direct and indirect clinical practice.

Utilizes Relias Learning Management System to track employee education.

Provide and maintain employee education record for CPR/BLS training for direct care staff as required.

Qualifications

Manages assignment process on a daily basis including: Delegate assignments to appropriate staff on all shifts; Completes patient assignment sheet taking into account patient acuity, staff level, ratio, ability, and medical schedule assignments; Makes appropriate change to assignments as patient/unit needs/acuity changes; Communicates changes in level of care/acuity to staff; Ensures that tasks are carried out as assigned; ensures that all staff arrive to work on time and depart on time; reports variances in a timely fashion, to the Director/and or Supervisor on the off shifts.

Essential Duties and Responsibilities:
Maintains established departmental policies, procedures and objectives to ensure that the standards of nursing practice are consistently met throughout the Masonicare AT BISHOP WICKE (MBW) facility.
Upholds the nursing program during the off shift through the allocation of nursing personnel, coordination of nursing activities, supervision of nursing staff and serves as a clinical resource to all staff.
Demonstrate knowledge and skill to provide care based on physical, psycho/social, educational, safety and related criteria appropriate to age, needs and abilities of the resident/patient in his/her assigned unit. Interacts professionally with staff, physicians, patients and families to ensure that care is effectively and efficiently delivered.
Communicates effectively to all staff through verbal reports and written documentation and attends necessary administrative meetings to disseminate important information to off shift staff.
Enhances professional growth and development through participation in educational programs both in and out of the facility.
Attends all education programs, mandatory in servicing and meeting.
Performs other duties as assigned.
Minimum Qualifications:
Education: Graduate of school of professional nursing, BS in nursing or healthcare related field.
Experience: 1-2 years of experience in a supervisory capacity preferred.
Certificates, Licenses, Registrations: Registration and current RN license in the State of Connecticut; CPR certification
Key Competencies: Ability to accurately assess and prioritize resident needs. Management/supervisory skills. Ability to resolve conflicts. Interpersonal skills. Excellent oral/written communication skills. Working knowledge of computers.

Overview
Under the direction of the Director of Nursing, the RegisteredNurse (RN) is responsible for providing quality care to resident of assigned neighborhood. The RN functions as a professional caregiver who is accountable for assessing, planning, implementing and evaluating all aspects of resident care, including physical, psychosocial and environmental needs.
Essential Functions
Administers medications accurately and in a timely fashion.
Documents according to nursing practice standards.
Provides treatments to residents according to physician orders and Duncaster policies and procedures.
Provides physical comfort and a safe environment.
Initiates, implements, and evaluates care plans/MDS on an ongoing basis and accurately carries out physician/provider orders in a timely fashion.
Accurately completes admission and discharge procedures.
Coordinates staff and resident care appropriately to meet resident needs.
Utilizes the electronic medical records system.
Ensures their license is up to date.
Follows all policies and procedures, annual mandatory in-services, required meetings, and all other conditions of employment are consistently met.
Performs other duties as assigned.
Education and Experience
Bachelor's degree in Nursing is preferred.
Requires licensing as a RegisteredNurse licensed to practice in the State of Connecticut.
2 years of full-time clinical experience in nursing is required.
Must be able to speak, read, and write the English language in an understandable manner.
Experience with MatrixCare or other electronic medical records is preferred.
Comprehensive knowledge of geriatric nursing care, principles of nursing, and physical assessment skills.
General knowledge of supervisory principles and practices.
Basic knowledge of human relations skills necessary to deal effectively with the geriatric residents, their families, and CHHC staff.
Environment and Working Conditions
Ability to focus on task needs and perform intellectual executive functions as related to documentation, charting, care device operations, etc.
Ability to execute intellectual tasks and duties under conditions of high demand and distraction.
Ability to complete the essential functions of the position with or with adaptive personal devices (e.g., hearing aid, corrective lenses).
Ability to operate equipment and technology required for the position.
Able to wear personal protective equipment.
Must be able to communicate effectively.
Able to assist with the evacuation of residents under emergency status to the best of their physical ability.
Able to function independently with integrity and work effectively with residents, personnel, and support agencies.
Able to constantly stand or walk to provide resident care and assistance.
Able to frequently push and/or pull a force up to 30 pounds including residents weighing up to 250 pounds in wheelchairs and geri-chairs.
Able to reach overhead, bend, and carry or lift 50 pounds occasionally and 35 pounds frequently.
Able to frequently transfer and support residents weighing up to 250 pounds while positioning, and transferring residents to beds, toilets wheelchairs, etc. Additional employees or mechanical assistance may be used.
Able to frequently reposition residents weighing up to 250 pounds in bed for assistance with treatments, hygiene, and comfort needs. Additional employees or mechanical assistance may be used.
Hours
Full-time, 3rd shift: 32 hours per week, 11:00pm-7:30am, Every other Weekend
Pay
$41.00 per hour + shift differential (additional $2.00 p/h Weekdays and $3.00 p/h Weekends)
