The Central Intake Licensed Clinician will provide direct intake and admissions services for individuals seeking treatment who have substance use disorders for the residential and outpatient treatment programs of the Center for Recovery and Wellness. Provide practical guidance and support to the Intake Team and interns. Assist the Director and Clinical Directors in day-to-day admissions operations. RESPONSIBILITIES Clinical Complete intake and admissions assessments for residential outpatient programs using a trauma informed lens onsite, offsite, or via telehealth) as needed Phone screen and assess potential clients Provide outpatient program coverage as required to meet staffing needs and fiduciary targets: Outpatient counseling duties to include but not limited to: Facilitation of individual and group level interventions during FT clinician vacation or leave Maintenance of primary counseling caseload commensurate with outpatient program needs and intake scheduling demands OASAS compliance reporting as requested Preparation of court reports and federal probation/ pretrial documentation as requested Collect urine, saliva and breathalyzer specimens for toxicology analysis. Administrative Complete quality admissions assessments in accordance with regulatory bodies (OASAS, CARF, and Medicaid) Support the training of new admissions staff Track enrollments using a shared excel system Maintain waiting list as required by regulation Maintain accurate and organized records in the EMR Participate in quality assurance reviews Submit timely documentation for billing purposes Coordinate initial referrals and scheduling of medical, mental health with onsite service providers. Ensure that clients' medical examinations and psychiatric referrals and evaluations are scheduled. Contact referral source upon admission of new clients in a timely manner and provide reports and oral clinical updates. Provide translators when necessary Register clients in drug testing data base Ensure potential admissions are screened and referred to the appropriate level of care. Coordinate financial screens and communicate with billing department during admission procedures. Ensure phones are answered professionally, and responses regarding inquiries are timely. Outreach to referral sources to expand network. We are seeking a qualified healthcare professional to be our Clinical Counselor Outpatient Services who will counsel and aid individuals who have substance use disorders and/or their families/loved ones in a New York State OASAS-certified outpatient treatment program. Utilizing a person-centered and trauma-informed lens, individualized assessment and counseling (telehealth or virtual) services are delivered both in-person and virtually at Project Contact Outpatient at the Center for Recovery and Wellness. Schedule: (3) Weekdays - 10am to 6pm, (2) Weekdays - 12pm to 8pm, and a rotating weekend once a month. Responsibilities Provide caseloads with weekly individual case management and counseling sessions consistent with the individualized treatment plan for individuals in the Substance Use Treatment and/or Significant Other Services treatment tracks. Continuously assess the appropriateness of the current level of treatment, corresponding with LOCADTR and utilization review instruments (Substance Use treatment track). Coordinate integrated services with medical, mental health, vocational, recovery services, and other social service providers. Complete detailed progress notes, comprehensive treatment plans, intake/admission assessments, psychosocial assessments, discharge plans, and other required documentation in accordance with OASAS and CARF standards. Maintain accurate and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS, managed care, and CARF standards. Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System. Specimen collection for clients and monitors for treatment-related issues in order to update the treatment plan (Substance Use treatment track) Check the toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with the client (Substance Use treatment track) Coordinate integrated services with medical, mental health, housing, vocational, recovery services, and other social service providers. Facilitation of individual and group-level interventions during other clinician vacations or leave Ensure that clients' annual medical examinations are completed, with associated referrals as needed. Contact referent and/or family contacts upon admission of new clients in a timely manner, and provide written or oral clinical updates as needed. Effective advocacy and collateral contacts to empower clients Outreach to referral sources to expand the network of clients. Attend staff meetings and trainings as required, as well as individual and group supervision. Perform special projects and other duties as assigned. Qualifications Master's Degree in a mental health discipline (MSW, MHC, MFT, etc), and either licensed in the state of New York, or working under a limited permit and eligible for licensure. SCOPE OF INFLUENCE Provide individual case management and group counseling to adults with substance use diagnoses (and their families/significant others). Partner with referral sources, providing regular updates and progress on clients. JOB PURPOSE Counsel and aid individuals who have substance use disorders in Pride Site, a New York State OASAS-certified residential treatment program-one of four programs in the newly constructed Center for Recovery and Wellness (CRW). SCOPE OF INFLUENCE Provide individual case management and group counseling to adults with substance use diagnoses (and their families/significant others). Partner with referral sources, providing regular updates and progress on clients. Partner with a multi-disciplinary team in assessing diagnoses, planning treatment, providing coverage of clients' individual and group sessions, and offering clinical insight and expertise. KEY RESPONSIBILITIES AND ACCOUNTABILITIES Clinical and Documentation (80%) Provide up to 20 clients with biweekly individual case management sessions to continuously assess appropriateness of current level of treatment. Coordinate services with medical, mental health, and other social service providers; communicate with the on-site outpatient program to ensure clients on caseload are receiving and responding to clinical care in the outpatient program while residing in the treatment facility. Arrange and participate in multidisciplinary interventions for clients on caseload who may be struggling at the current level of care and follow up by assessing progress incrementally. Provide evidence-based group counseling to clients in the residential program. Complete detailed progress notes, comprehensive treatment plans, psychosocial assessments, discharge plans, and other required documentation in accordance with standards. Maintain accurately and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS and managed care standards. Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System. Check the toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with the resident. Ensure that clients' annual medical examinations and psychiatric evaluations are completed, completing referrals as needed. Contact the mandating source upon admission of new clients in a timely manner and provide written/oral clinical updates as needed. Make collateral contacts for clients. Other (20%) Outreach to referral sources to expand the network of clients. Attend staff meetings and training as required, as well as individual and group supervision. Perform special projects and other duties as assigned. JOB REQUIREMENTS Required Knowledge LP-MHC or LMSW. Minimum of 1 year of experience working with the SUD population. Ability to multitask and handle challenging situations in a professional and courteous manner. Maintain confidentiality of records and information. Willingness to work as part of a team. Must be organized, self-motivated, a team player, and computer literate. Ability to work with a diverse set of clients with differing presentations. Ability to work professionally and empathetically with a diverse group of clients and staff.
