The Center for Recovery and Wellness takes a holistic, strengths-based approach to recovery and wellness that responds to the distinct needs of each individual. Programs include residential and outpatient addiction treatment, recovery services, prevention education, and community programming. CRW offers culturally competent, evidence-based programs and services that strengthen individuals and communities, enhance resiliency, and foster lasting recovery.
We are seeking a full-time Reintegration Coordinator to counsel and aid individuals who have substance use disorders through case management in Pride Site, a New York State OASAS-certified residential treatment program. Utilizing a person centered and trauma informed lens; individualized assessment and counseling services are delivered in Pride Site. Case management strategies are applied throughout the individual's transitional stage of re-entry to the community. During this stage individuals live in the residential environment and receive outpatient services at the Center for Recovery and Wellness.
RESPONSIBILITIES
Provide up to 20 clients with biweekly individual case management sessions in collaboration with the vocational services team to ensure adherence to vocational and treatment schedules, and a smooth transition to the community.
Coordinate services with vocational, psychiatric, medical, mental health, and other social service providers.
Communicate with on-site outpatient program to ensure clients on caseload are receiving and responding to clinical care in the outpatient program while residing in the treatment facility and adhering to residential program reintegration expectations.
Organize and participate in multidisciplinary interventions for clients on caseload.
Provide follow-up interventions, assessing progress, and communicating results to interdisciplinary team.
Provide evidence-based group counseling to clients in the residential program and coverage in the outpatient program.
Participate and provide detailed progress reports in case conference for both Pride and PCO programs
Complete detailed progress notes, comprehensive treatment plans, psychosocial assessments, discharge plans, and other required documentation in accordance with standards.
Engage client and treatment team in warm hand-offs for the step down of care.
Maintain accurate and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS and managed care standards.
Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System.
Check toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with resident.
Ensure that clients' annual medical examinations and psychiatric evaluations are completed, completing referrals as needed.
Contact mandating source upon admission of new clients in a timely manner and provide written/oral clinical updates as needed.
Make collateral contacts for and with clients.
Outreach to referral sources to expand network of clients.
Attend staff meetings and trainings as required, as well as individual and group supervision.
Perform special projects and other duties as assigned.
QUALIFICATIONS
Master' Degree (MSW; MHC; etc.), and either licensed in the state of New York or working under a limited permit and eligible for licensure, or CASAC with 3-years of residential experience required
Experience working directly with clients diagnosed with substance use disorder
Ability to multitask and handle challenging situations in a professional and courteous manner
Maintain confidentiality of records and information
Willingness to work as part of a team and excellent collaborative skills.
Must be organized, self-motivated, detail oriented, and technologically savvy.
Available for shifts to accommodate a caseload of clientele who may be out of the house responding to work/training activities during the standard 9-5 M-F schedule.
Ability to work with a diverse set of clients with differing presentations.
Ability to work professionally and empathetically with a diverse group of clients and staff.

We are seeking a full-time Motivational Enhancement Coordinator who will assist in the overall administrative and clinical operations of the 100-bed, OASAS-licensed residential treatment facility. S/he will supervise the motivational enhancement milieu staff. This position will assume responsibility for the site in the absence of the Clinical Director of Residential Services.
RESPONSIBILITIES
Plan and implement Program policies, procedures, services and activities in collaboration with the Director of Vocational and MEC Services.
Assure site compliance with all OASAS and HRA regulatory standards, perform quality assurance functions, and maintain the site in an audit-ready condition with the assistance of Clinical Director of Residential Services and the administrative staff.
Provide supervision of residential motivational enhancement counselors; facilitate seminars, groups and other direct service interventions and services; supervise behavior modification interventions based on evidence-based practices and clinical expertise
Oversee facilities and inventory, ensuring the safety, security, maintenance and repairs of the site by liaising with the Facilities Department and by routine inspections.
Monitor HRA finger imaging systems
Carry out disbursement and accounting of petty cash, food and client money.
Coordinate staff coverage; supervise staff who oversees Program Departments (Induction, Food Preparation, Medical).
Perform other duties as assigned by supervisor.
Attend regularly scheduled meetings and trainings

QUALIFICATIONS
High School Diploma and CASAC required
BA/BS in Human Services Field preferred

JOB SUMMARY: Barclays Programs Rebound Elevate and Goal-E are job readiness programs that incorporate various sports to engage participants including Basketball, Soccer and Tennis. The programs serve young adults, who are disconnected, justice involved, overage and/or under-credited. In close collaboration with other program staff, the Internship Career Coordinator provides job readiness and career exploration workshops, as well as work one on one with youth. The Rebound Elevate program uses basketball as a vehicle for engagement, but also as avenues for learning job readiness skills such as teamwork, conflict resolution and taking direction from a coach/supervisor. Rebound Elevate is centered around exploring careers in sports. Goal-E (Goal Employment) is a career readiness program utilizing Soccer as a focal point to explore careers, employment, education and vocational options for participants. As the Soccer Coach for Goal-E, this role is responsible for coordinating and facilitating Soccer and sports related skills and drills to engage participants.
MAJOR DUTIES:
Facilitate career exploration, work readiness and financial literacy workshops for program participants.
Lead work readiness workshops utilizing virtual platforms as needed, specifically Zoom Video Conferencing.
Provide individual 1 on 1 job and career counseling to young adults centered on career mapping and goal setting.
Support participants growth by providing continuous feedback and resources to meet individual needs.
Assist the Basketball Coach in utilizing basketball to teach life and job skills (i.e., teamwork and conflict resolution).
Responsible for recruitment and enrollment of youth to the program with Sr. Program Manager and Basketball Coach.
Build Partnerships via community canvasing, tabling events, and facilitation of presentations.
Assist youth in preparing for and securing employment opportunities.
Monitor and provide support to participants once they have gained employment.
Provide alumni outreach and support including career counseling, referrals, and skill development.

Apply for adoption subsidy and medical eligibility for qualifying children. Maintain tracking and be up to date on all adoption subsidy requests.
Work with GSS medical department in an effort to obtain required documentation to finalize adoption subsidy board rates and adoption subsidies are approved on a timely basis.
Maintain updated Kingap and Adoption tracking to ensure that progress and barriers for each case is fully documented.
+ Coordinate the photo listing process to ensure compliance with registration requirements ensuring that all children required to be photo listed in the OCFS Adoption Album are kept up to date.

Write Affidavits to the courts when requested.
Maintain constant communication with foster parents and case planning team on the status of both adoptions and KinGaps.

Qualifications:
Education and/or credential requirements are determined by childrens acuity level and requires one or more of the following:
* Bachelor of Arts or Science degree with two years experience required

We are seeking a full-time Reintegration Coordinator to counsel and aid individuals who have substance use disorders through case management in Pride Site, a New York State OASAS-certified residential treatment program. Utilizing a person centered and trauma informed lens; individualized assessment and counseling services are delivered in Pride Site. Case management strategies are applied throughout the individual's transitional stage of re-entry to the community. During this stage individuals live in the residential environment and receive outpatient services at the Center for Recovery and Wellness.
RESPONSIBILITIES
Provide up to 20 clients with biweekly individual case management sessions in collaboration with the vocational services team to ensure adherence to vocational and treatment schedules, and a smooth transition to the community.
Coordinate services with vocational, psychiatric, medical, mental health, and other social service providers.
Communicate with on-site outpatient program to ensure clients on caseload are receiving and responding to clinical care in the outpatient program while residing in the treatment facility and adhering to residential program reintegration expectations.
Organize and participate in multidisciplinary interventions for clients on caseload.
Provide follow-up interventions, assessing progress, and communicating results to interdisciplinary team.
Provide evidence-based group counseling to clients in the residential program and coverage in the outpatient program.
Participate and provide detailed progress reports in case conference for both Pride and PCO programs
Complete detailed progress notes, comprehensive treatment plans, psychosocial assessments, discharge plans, and other required documentation in accordance with standards.
Engage client and treatment team in warm hand-offs for the step down of care.
Maintain accurate and organized clinical charts, engaging in pre-audits of charts to ensure compliance with OASAS and managed care standards.
Send weekly, monthly, and quarterly reports to HRA through the Substance Abuse Tracking and Reporting System.
Check toxicology reporting portal daily to ensure any positive toxicology reports are communicated to supervisory staff, documented, and addressed with resident.
Ensure that clients' annual medical examinations and psychiatric evaluations are completed, completing referrals as needed.
Contact mandating source upon admission of new clients in a timely manner and provide written/oral clinical updates as needed.
Make collateral contacts for and with clients.
Outreach to referral sources to expand network of clients.
Attend staff meetings and trainings as required, as well as individual and group supervision.
Perform special projects and other duties as assigned.
QUALIFICATIONS
Master' Degree (MSW; MHC; etc.), and either licensed in the state of New York or working under a limited permit and eligible for licensure, or CASAC with 3-years of residential experience required
