The pharmacist clinician must have completed a PGY1 or PGY2 ambulatory care residency or have at least 2 years experience in ambulatory clinical pharmacy. Consideration for applicants who have completed an ACPE accredited Medication Therapy Management training program. Applicants must be board certified or become board certified within one year of eligibility to qualify for certification. Evidence of ongoing clinical practice by proof of 25 patient interactions, reflective of the scope and complexity of privileges requested in the previous 12 months or obtain within 3 months of hire.
