Responsible for performing all duties associated with interpretation, order verification, dispensing and monitoring of medications ordered by prescribers; the evaluation and dissemination of comprehensive information about medications and their use, to patients, families, and medical staff; and providing quality pharmaceutical care to make a healthy difference in people's lives. Education Qualifications:
Graduate of a college of pharmacy accredited by the American Council on Pharmaceutical Education (BS or PharmD degree required)
ASHP-Accredited Hospital Residency, or three or more years of hospital pharmacy experience preferred.
Licensure/Certification Qualifications:
Certification/Licensure Required:
* Licensed as a Registered Pharmacist by the Board of Pharmacy of the state in which they will be working
* Pharmacists in the SMPHM Union, are required to be licensed in both Minnesota and Wisconsin (within one (1) year from the date of hire), regardless of their primary worksite for all regular full-time pharmacists and part-time (.4 FTE or greater)
If not licensed as a Registered Pharmacist upon hire, the candidate must have a Pharmacy Intern License and/or is eligible to work as an intern and must become a Registered Pharmacist within 90 days of hire:
* Minnesota: Must be licensed as a Pharmacy Intern through the Minnesota Board of Pharmacy

In collaboration with the patient and the patients' health care team, the ambulatory pharmacist clinician works to optimize the patient's drug therapy to improve the quality of life and health outcomes of the patient. The standard of care includes ensuring the patient's medications are appropriate for the patient, effective for the medical condition, safe given the comorbidities and other medications being taken, and able to be taken by the patient as intended. The position requires high level of clinical skills, and the ability to build positive relationships with patients and colleagues, including other health care providers. Essentia Clinical Pharmacy is looking for the right candidate to open a new practice in one of our rural communities, either the Baxter, Virginia, Deer River, or Grand Rapids clinics.
**Education Qualifications:**
Graduation from a pharmacy program accredited by the Accreditation Council for Pharmacy Education (ACPE) or a program outside the U.S. that qualifies the individual to practice in the jurisdiction.
The pharmacist clinician must have completed a PGY1 or PGY2 ambulatory care residency or have at least 2 years experience in ambulatory clinical pharmacy. Consideration for applicants who have completed an ACPE accredited Medication Therapy Management training program.
Applicants must be board certified or become board certified within one year of eligibility to qualify for certification.
Evidence of ongoing clinical practice by proof of 25 patient interactions, reflective of the scope and complexity of privileges requested in the previous 12 months or obtain within 3 months of hire.

Iowa: Effective July 1, 2010, all pharmacy techs working in Iowa pharmacies must be nationally certified through the Pharmacy Technician Certification Board (PTCB) or the National Healthcareer Association (NHA). If enrolled in a technician training program, or not previously having worked as a pharmacy technician, must register as a pharmacy technician trainee within 30 days of starting technician training or employment. A technician trainee must become a nationally certified pharmacy tech within 12 months of starting tech employment or training. A pharmacy tech trainee who does not complete national pharmacy tech certification before expiration of the pharmacy tech trainee registration, must discontinue working as a pharmacy tech. Minnesota: As of January 1, 2012 any individual registering for the first time as a technician needs to be 18 years of age or older. Must have active/current pharmacy technician registration with the Minnesota Board of Pharmacy. For new technician registrants, the individual must complete a Sanford provided (or equivalent) pharmacy technician training program within 12 months of registration with the board. North Dakota: Pharmacy Technicians must register as a Pharmacy Technician or Technician in Training with the ND Board of Pharmacy, before beginning employment in the pharmacy. An applicant for registration as a pharmacy technician in training must register for Sanford provided (or equivalent) ASHP accredited education and obtain national certification approved by the board of pharmacy within the board of pharmacy's timelines. The Pharmacy Technician Certification Board (PTCB) or National Healthcareer Association (NHA) are approved certification bodies. The registration as a pharmacy technician in training or pharmacy technician will continue as long as the applicant maintains a pharmacy technician-in-training or pharmacy technician license from the North Dakota board of pharmacy. Failure to obtain ASHP accredited education and National Certification before the board of pharmacy removes the technician in training license will result in termination of position. South Dakota: Registration required within 30 days from the date of hire. Technicians registered with South Dakota Board of Pharmacy after July 1, 2014 will be registered as a “technician-in-training” and will have two years to complete training education and national certification through the Pharmacy Technician Certification Board (PTCB). Failure to obtain National Certification within two years of registering as a “technician-in-training” will result in termination of position. Technicians registered with the South Dakota Board of Pharmacy after July 1, 2011 will be required to obtain National Certification through the Pharmacy Technician Certification Board by July 1, 2014. Failure to obtain National Certification will result in termination of position.
