Job Summary:
Responsible for the total direct nursing care of all patients within established standards of care, hospital and unit policies, procedures, and guidelines.
Telemetry and intermediate step down, PCU nursing experience is required.
Primary Duties:
1. Provides direct patient care and functions as a patient advocate.

License, Certification, Education or Experience:
REQUIRED for the position:
* Graduate of a nursing program (Diploma or Associate Degree in Nursing (ADN) required.
* Licensed as a Registered Nurse in the State of Washington
* Minimum of one-year Progressive Care nursing experience
Current Washington State Registered Nurse License
* Current Healthcare Provider BLS- American Heart Association by start date.
* Current Healthcare Provider ACLS- American Heart Association by start date.

Key Responsibilities
Serve as the primary on-call admissions nurse during weekends, performing initial hospice assessments and completing required documentation.
Conduct in-home or facility-based admissions, evaluations, and symptom management visits as needed.
Provide expert clinical judgment, emotional support, and education to patients and families during the admission process.
Communicate and coordinate with the hospice interdisciplinary team (physician, social worker, chaplain, aides) to ensure timely and compassionate care.
Maintain accurate and prompt documentation in the EMR system.
Respond to urgent calls and coordinate follow-up care as appropriate.
Qualifications
Active Washington State RN license (or compact equivalent).
Hospice admissions and on-call experience required.
Proficiency with HCHB (HomeCare HomeBase) EMR required.
Strong clinical assessment, organization, and communication skills.
Compassionate and composed under pressure; able to manage emotional patient and family interactions.
Reliable transportation, valid driver's license, and current auto insurance.
Must be available to work Friday 5:00 PM - Monday 8:00 AM PST.

Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group.
Provides and maintains a safe environment for the patient.
Assists the patient and family/caregiver and other team members in providing continuity of care.
Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver.
Attends interdisciplinary group meetings.

Job Requirements?
1. Graduate of an accredited school of nursing with a current license for the state of Arizona as a Registered Nurse.

Completes the hospice face to face visit requirements prior to the third and subsequent hospice benefit periods to assess hospice eligibility based on the hospice conditions of participation and the local coverage determinations. Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.

Job Summary:
EvergreenHealth is seeking an experienced Employee Health RN to join the Employee Health Team. Employee Health provides essential services, including pre-employment screening, surveillance services, vaccinations, boosters, fit-testing, exposure management, confidential records tracking, data reporting, and compliance with federal, state and local regulations and organizational policies, to the employees, providers and volunteers at EvergreenHealth.
This position will cover both Kirkland and Monroe Employee Health locations.
Primary Duties:
* Responsible for all activities in Employee Health Services.
* Provides clinical care to employees, physicians and volunteers that is based on policies, protocols and guidelines.
* Works closely with the Manager-Employee Health, other Employee Health nurses and the department Administrative Assistant, the Medical Director-Infectious Disease, authorized health care practitioners, Infection Prevention and Human Resources.
* Performs other duties as assigned.
License, Certification, Education or Experience:
REQUIRED for the position:
* High School diploma or equivalent.
* Bachelor's in Nursing (BSN) required within 5 years of employment if applicable
* Currently licensed as an RN in the State of Washington
* Three (3) years of experience as a RN
* Current America Heart Association BLS Provider certification by date of hire
DESIRED for the position:
* Two (2) years of experience in Occupational or Employee Health
* Vaccination and/or other Employee Health experience
* Experience in Public or Community Health Nursing
* Experience in process improvement and systems optimization
