Ebenezer is looking for an Admissions Coordinator II to join our corporate admissions team at the Ebenezer Corporate Office in Edina, MN! This position will develop relationships with referral sources and work with internal teams to promote and maintain census. Assist Social Service Department in meeting the needs of the residents and families of the care center.
This admissions schedule includes;
* On Call/Casual
* Day shifts available
* To maintain Casual status, employees must be willing to be available to work a minimum of three (3) shifts per month (two shifts must be weekend shifts) and one (1) Holiday shift per year. The weekend starts with the beginning of the night shift on Friday and ends with the beginning of the night shift on Sunday
Responsibilities:
* Ability to process and complete admissions
* Provides pre-admission counseling, information and tours
* Clinically evaluates referrals to ensure all needs can be met from a medical perspective
* Facilitates the intake process and secures primary care provider
* Reviews EPIC for proper documentation and orders
* Contacts pharmacy as needed to verify cost of meds
* Determines need for equipment and supplies and coordinates with internal team
* Facilitates PAS Screening and Level II as needed
* Coordinates admission process with resident and resident representative, community providers and staff as appropriate
* Sets up interpreter services as needed
* Assures completion of admission paperwork
* Informs residents and resident's representatives of financial responsibilities, payment options and agency contacts according to the state and federal regulations
* Builds and maintains relationships with referral sources and assists with marketing and outreach activities as appropriate
* Maintains internal and external waiting lists as appropriate
* Verifies necessary payer source(s) as appropriate
* Coordinates room change process as appropriate
Required Qualifications:
* Associate of Arts Vocational/Technical Training or Associate Degree
* One to three years of previous experience
* Licensed Practical Nurse Licensed Practical Nurse (LPN) or Registered Nurse Registered Nurse (RN)
Preferred Qualifications:
* Bachelor of Science In Medical or Human Service Field
* Three to five years of previous experience
* 2 years experience in LTC
* Computer Skills

As an integral member of the primary care team, the team coordinator contributes to overall quality of care and patient experience through performing high-level functions to support the needs of the healthcare team and patients/support network. We have TC's partner with all members of the team to ensure patients needs are optimally addressed. They will deliver direct support to patients on the coordination of medical information requests and updates, as well as referrals and appointment scheduling. The TC will use a variety of communication tools and systems in their work to support patients and their care team. Additionally, we ask the TC's to maintain complete, timely and accurate documentation for each patient. They will also continually look for opportunities we can improve processes and workflow.
* Performs telephone & MyChart message support & follow-up communication in a timely and professional fashion; uses communication systems for both routine & emergency transactions
* Schedules, reschedules & maintains patient appointments & supports clinic leadership in schedule management
* Partners collaboratively with the patients care team to address patient needs
* Find opportunities for improvement in workflow & participate in clinic and system meetings
Required Qualifications
* Successful completion of formal medical training program such as MA, HUC, NA or Medical Secretary, or min. of 1 year comparable experience in a healthcare setting

This role reports to the Director of Student Services Support, serving our students in Mayo Clinic College of Medicine and Science. These verifications are crucial to our trainees' transition into employment and advanced training. Education is very cyclical with different tasks occurring at different times of year, and our spring and summer timeframe (April-August) is an especially busy season. We are looking for someone that is self-motivated, organized, customer focused with a strong attention to detail.
This position is a combination of remote and on-site work; individual must live within driving distance to any Mayo Clinic campus.
