As the Personal Care Assistant, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home.
Primary Responsibilities:
Provides or assists clients with activities of daily living including:
bathing in bed, tub or shower
care of hair including shampoo, combing and brushing
care of teeth and mouth including denture care
nail care, filing only
skin care including pericare and applying lotion
transfer of patient from bed to chair and to wheelchair
transfer of patients on and off bedpan, commode and toilet
assist client in preparing for bed
position patient in bed as directed
assists with ambulation including with cane, walker and crutches
personal care dressing of client
medication reminder
Live-in and Sleep Over's:
secures clients home for the night and provides reassurance by being present at the client's home during the nighttime hours
(Washington and Oregon does not provide this service)
Utilize the E.V.V. (Electronic Visit Verification) program as applicable
Assists with household tasks directly essential to client's personal care
Accompanies client to medical appointments or shopping if necessary and performing other essential errands
Measures and records oral, rectal, and auxiliary temperatures, pulse and respiration, and blood pressure when ordered within the training limitations of the aide
Prepares meals as required based upon client's preferred diet
may need to feed client if required
Provides clean and safe environment within the home including safe use of equipment such as side rails, oxygen, wheelchairs, Hoyer lifts, etc.

Required Qualifications:
Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation

WORK SCHEDULE: Schedule to be determined by provider, minimum of 16 hours weekly. Occasional weekends and holidays
Ensures that the psychiatric needs of each client are met in appropriate time frames and serves as an additional clinical support to the team.
ESSENTIAL DUTIES:
Typically sees the beneficiaries for the assessment and treatment of the beneficiary's symptoms and response to medication including side effects. Frequency will vary for each beneficiary, with the majority seen within 4 to 6 weeks of last appointment, with many of the contacts being in the community. Less frequent visits should only occur when there are unusual circumstances present (for example, when the beneficiary has been difficult to find);
ACT Team psychiatrist provides clinical supervision and oversight of the psychiatric services delivered by the NP or PA;
Collaborates with the team leader in sharing overall clinical responsibility for monitoring beneficiary treatment and clinical supervision to the team;
Actively collaborates with nurses to develop and implement medication administration policies and procedures as well as oversee the medical care of beneficiaries that include regular screenings for medical conditions and assessment of wellness and health management;
Educates non-medical team members on psychiatric & non-psychiatric medications, their side effects, & health-related conditions; provides diagnostic and medication education to beneficiary, with medication decisions based on shared-decision making;
Regularly participates in daily team meetings and treatment planning meetings; attends daily team meetings in proportion to time allocated on team;
Provides brief therapy (formal or informal); and
Provides psychiatric back-up to the program after-hours and weekends. (Note: may be on a rotating basis as long as other psychiatric care providers who share on-call have access to beneficiary's current status and medical records/current medications).

LEVEL OF EDUCATION/TRAINING/QUALIFICATIONS:
ACT Team Psychiatrists must be board eligible or certified by the American Board of Psychiatry
and Neurology and Licensed to practice in NC and meet the requirements as specified in NCAC 27G .0104.
PHYSICAL DEMANDS: Regularly walk, stand or stoop; occasionally lift, carry, push, pull or otherwise move objects weighing up to 25 pounds; and regularly drives a motor vehicle.

Position Summary
The Direct Support Professional (DSP) plays a vital role in advancing our mission by providing compassionate, hands-on support to individuals with intellectual, developmental, physical, and mental disabilities. DSPs empower participants to build independence, develop skills, and engage meaningfully in their community through daily activities, work skills training, community coaching, and personal support.
We are currently seeking two (4) full-time Direct Support Professionals to work Monday through Friday from 8:30 AM to 3:00 PM, as well as two (2) PRN Direct Support Professionals to provide coverage on an as-needed basis.
Compensation
Pay Rate: $12.00 per hour
Key Responsibilities
Deliver purposeful, age-appropriate activities that promote independence, engagement, and personal growth
Implement training techniques and interventions to support social skills, personal hygiene, motor coordination, and daily living skills
Provide personal care and hygiene assistance in a respectful, person-centered manner
Supervise and support participants during all program activities, including providing coverage when additional assistance is needed
Encourage individuals to actively or partially participate in daily routines and program activities
Accompany participants on group outings, community activities, and work-based experiences
Transport participants using agency vehicles as required and serve as a backup van driver
Maintain program equipment and supply inventory and notify supervisors when reordering is needed
Complete accurate and timely documentation for assigned individuals by the end of each shift
Identify program challenges and recommend solutions that support quality services and positive outcomes
Attend required trainings and staff meetings to support professional growth and program excellence
Perform additional duties as assigned to support program objectives, participant safety, and team success
Qualifications
High School Diploma or GED required
Valid Drivers License (required if operating agency vehicles)
CPR and First Aid certification, or willingness to obtain and maintain certification
NCI certification, or willingness to obtain and maintain certification
Ability to pass criminal background and social services background checks
