Participate in Quality Assurance programs, the Pharmaceutical Services Committee, and any other facility committee or program that affects the medication pass and seeks to improve the performance and accuracy of the medication process. Accurately and safely prepare, administer, and document the oral and/or topical medications that are commonly used in this facility and that may be ordered for resident use by the attending physician or the Medical Director. Verify that any medications brought into the facility by a newly admitted resident are examined and identified by the attending physician or the facility pharmacy or pharmacist Follow facility policies and procedures regarding the destroying of medications; assist in carrying out these rules under the direction of the facility pharmacist or Director of Nursing Services, including the documentation requirements. Follow the labeling policies and practices of the facility. Ensure that all medications administered are properly labeled. Follow facility procedures in regard to charting medications. Assist in documenting and removing medications that are discontinued by the attending physician. Follow facility procedures governing drug receipt errors, including notifying the delivery source and the Director of Nursing Services. Ensure that documentation of the drug receipt error includes a written error report, signed by an observer/witness and you. Follow facility procedures in holding medications for residents who are transferred from the facility orwhen the attending physician discontinues medications Observe and verify that medication is ingested or applied as directed. Document any instance where prescribed medication is not administered, including reason(s) for refusal. Promptly notify the Charge Nurse, Nurse Supervisor, or Director of Nursing Services of any errors or reactions to medications by residents. Verify the identity of the resident before administering the medication treatment. Accurately measure, record, and report the vital signs of residents. Position Summary
This position requires a mission-driven and detail-oriented Advanced Practice Provider (APP) to serve as our lead clinician for Medicare Annual Wellness Visits (AWVs), preventive care services, and related quality initiatives. This is a high-impact position designed to help our organization improve preventive care rates and quality outcomes across all 28 Cahaba Medical Care clinics.
The ideal candidate will have a passion for health promotion, quality improvement, and rural health equity. This provider will perform in-person wellness visits across our locations, follow up on necessary documentation (including insurer-specific requirements), and support training for other staff and providers in performing high-quality, compliant AWVs. This position reports directly to the Chief Executive Officer (CEO), Chief Medical Officer (CMO), and works in close coordination with the Quality Improvement Officer.
Responsibilities & Duties
Perform in-person Medicare Annual Wellness Visits and other preventive care visits across Cahaba Medical Care sites.
Ensure all required elements of AWVs are accurately completed, documented, and submitted per payer and CMS guidelines.
Submit insurer-specific forms and documentation in payer portals (e.g., closing care gaps, diagnosis attestation, HCC coding updates, etc.) to ensure loop closure.
Collaborate with the Quality Team to ensure visits meet internal quality standards without additional remediation.
Serve as a clinical resource and subject matter expert for AWVs, helping to train Medical Assistants, front-desk staff, and fellow providers on workflow best practices.
Identify system barriers to completion of AWVs and work collaboratively to improve clinic workflows and efficiency.
Support Cahaba Medical Care's system-wide goal of achieving a defined percentage of Medicare AWV completion prior to the calendar year's end (goal to be determined annually).
Provide occasional care for acute, walk-in visits if clinically appropriate and needed on-site.
Participate in organizational quality and training initiatives, special projects, and data-driven improvement work related to population health.

Only if permitted by state specific regulations, may assist residents with medication as defined in medication procedure. Follows physician orders and state laws to administer medications to residents who cannot self-administer their medications. Exhibits understanding of and follows medication policies and procedures. Maintains a clean, safe, and orderly environment for the residents. Performs general housekeeping, following cleaning schedules for resident laundry, bedrooms, dining area, living space, bathrooms, kitchen, etc.

The Childcare Attendant's primary duty is to ensure the safety and security of all children that attend our childcare area. The parents, our gym members, will remain onsite to excerise knowing their most valuable assets are in great hands.
Responsibilities:
- Provide quality care for children in a defined area of the gym for the attending members and guests
- Greet and properly check-in all members and guests
- Maintain a sanitary environment by thoroughly cleaning toys, counters, tables, and floors
- Interact with the children in a positive and age appropriate manner
- Maintain order and a controlled environment
- Keep security records on individual children ensuring that all children have a registration form on file.
- Ability to kneel, get up and down off the floor, and pick up babies and toddlers up to 30 lbs

Job Title: Caregiver / Home Care Aide
Reports To: Care Coordinator / Agency Administrator
We are seeking compassionate, reliable, and dedicated caregivers to provide non-medical home care services to seniors and individuals with disabilities. As a caregiver with God's Sitting Partners Home Care, you will assist clients with daily living activities, ensuring their safety, dignity, and quality of life.
Responsibilities:
Assist clients with personal care tasks such as bathing, grooming, dressing, toileting, and hygiene
Provide companionship and emotional support
Assist with mobility and transfers, including use of assistive devices
Prepare and serve meals according to dietary needs
Perform light housekeeping tasks (laundry, dishes, vacuuming, etc.)
Remind clients to take medications (no medication administration)
Provide transportation or run errands as needed (if applicable)
Monitor and report changes in client condition or behavior
Maintain accurate documentation of services provided
Qualifications:
Prior caregiving experience preferred (formal or informal)
Ability to pass a background check and drug screening
Valid driver's license and reliable transportation (if required for the role)
CPR/First Aid certification (or willingness to obtain)
Strong communication and interpersonal skills
Ability to work independently and follow care plans
