Provide oversight and monitoring of individuals served behavior and activities. 2. Prompt and coach individuals with daily living needs such as, bathing, cleaning, social interaction, etc. as needed by each individual. 3. Must successfully complete medication training and deliver medication to individuals as prescribed with appropriate documentation. (as required by site needs) 4. Drives a 15-passenger van and maintains a safe driving record. 5. Record/chart pertinent information and observations, resident discussions, plans, progress, skills and abilities. 6. Implement individualized plans, collect data and submit to supervisor in a timely manner. 7. Coordinate residential and community socialization activities as selected by individuals. 8. Maintain the environment in a neat and clean manner. 9. Maintain inventory lists and alert supervisor of any low inventories and special needs. 10. Attend and participate in regularly scheduled staff meetings and in-services and individual program planning staffings as needed. 11. All other duties as assigned.
