The **Community Outreach RN Coordinator** is responsible for coordinating and implementing Hackensack Meridian Health's Community Outreach Wellness Plan to address identified community health needs and in support of Hackensack Meridian Health's strategic imperatives and centers of excellence. Short Description The Community Outreach Coordinator will develop and maintain a community outreach strategy to target populations.
