Overview
The Physical Therapy Assistant, under the direction of the Physical Therapist collaborates in planning and implementing programs that help to restore or improve function in mobility, strength, coordination, and range of motion in patients suffering from disease or injury. Services are provided across the full continuum and rendered in a cost conscious, quality focused, and customer-oriented manner. Compliance with all applicable regulatory standards is also required with this role.
Responsibilities

Provision of Care
* Implements the physical therapy treatment plan under the direction of the physical therapist.
* Selects appropriate treatment activities to progress patient towards goals.
* Revises goals/plan of care in collaboration with the physical therapist and patient/family
* Provides ongoing patient/family education and training
* Provides discharge instructions, follow-up and referral to community resources as appropriate
* Acts as a referral source for staff, families, physicians on services and equipment related to rehabilitation services.

* Documents each treatment session according to departmental protocols
* Completes and submits family education, discharge and other required documentation within facility and program guidelines

Follows the established treatment plan and provides treatment according to funding source and regulatory guidelines to meet functional goals; accurately documents, including need for skilled services, patient progression, patient response, and then bills accordingly; provides education as appropriate
Serves as CI/mentor for interns as needed: Provides direct supervision/mentorship, taking direct responsibility for the patient & care provided. Provides daily feedback to interns regarding treatment interventions & directs the learning experience. Critiques/co-signs documentation daily to be sure it meets regulatory requirements & funding source specifications. Completes weekly, midterm and final evaluations for interns & communicates w/ their school regarding performance.
Participates in hospital and discipline specific skills check: Demonstrates competency through completion of didactic training and testing as well as demonstration of skills specific to their position and skill set as well as skills obtained at Wellstar Roosevelt.

Job Summary:
The Occupational Therapist is responsible for age appropriate interventions for the client that meet physical, emotional, developmental, psychosocial and educational needs to patients whom require rehabilitative care. - Direct Patient Care a) Access and manage patient's rehabilitative needs utilizing general principles governing reasonable and necessary therapy. b) Manage patient from admission through discharge and appropriately schedule visits per MDs order. Adjust frequency as needed based on status and MD order c) Perform routine occupational therapy procedures to meet the need of the patient. d) Meet established productivity standards. e) Seek training, supervision or direction from leadership as needed
- Coordination of Services a) Develop the patient's plan of care and revise it based on patient need within MD orders b) Coordinate the care and services with the Clinical Supervisor and other team members and support services. c) Perform other duties as assigned by Home Health Leadership. d) Maintain communication with MD reporting any changes that may affect the patient's recovery.
- Patient/Caregiver Education a) Serve as patient educator to foster care or caregiver support for the home patient. b) Be aware and report concerns to authorities or agency leadership on any issues that affect patient or staff safety c) Utilize both verbal and written tools to facilitate learning and promotion of independence during recovery.
- Staff Education and Professional Development a) Participate in the orientation and education of new employees as appropriate. b) Responsible for attending continuing education and in-services as directed and needed. c) Accept responsibility to remain current in clinical knowledge for the care of the home health patient.
- Communication and Documentation a) Responsible for accurate completion of skilled intervention documentation within established timeframe. b) Communicate patient status changes to the MD c) Demonstrate knowledge and appropriate application of regulatory guidelines d) Participate in care team conferences with coworkers e) Responsible for checking voice mail box and secure messaging a minimum of once daily.

Essential Functions
Promotes mission, vision, and values of Intermountain Health, and abides by service standards.
Competent Services: Provides skilled occupational therapy services, staying updated on standard practices for different patient groups. Conducts evaluations and treatments according to professional standards (AOTA), considering diagnosis, history, and physician referrals, and develops care plans with specific goals based on patient assessments.
Productivity Standards: Meets established productivity standards for the department or service line.
Documentation and Billing: Keeps thorough and timely patient records as required by regulations and facility policies. Completes patient billing accurately and promptly.
Communication: Communicates effectively with patients, families, physicians, and healthcare providers about patient needs and goals. Ensures smooth operations and optimal use of rehabilitation services.
Patient Care and Supervision: Manages all aspects of patient care and education, including supervising assistants, aides, office staff, and students to ensure high-quality care.
Continuing Education: Participates in and promotes continuing education opportunities for self and staff, seeking new learning opportunities according to licensure requirements.
Quality Improvement: Engages in quality improvement processes, including safe equipment use, improving clinical outcomes, patient access, patient experience and all aspects of the Intermountain Operating Model. Participates in utilization review audits.
Meetings: Participates in patient care conferences, interdisciplinary meetings, staff meetings, and necessary professional and work groups in collaboration/consensus with leader.
Program Development and Marketing: Contributes to program development and marketing strategies to grow the physical therapy program and achieve department goals.

Current Occupational Therapist license in states where you work.
Basic Life Support (BLS) for healthcare providers.
Basic Computer skills.
Exceptional interpersonal and communication skills.
Possess skill sets and experience with target population of job setting.
Current driver's license, reliable transportation, and acceptable driving record.
Physical Requirements:
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)
May be expected to stand in a stationary position for an extended period of time.
