The Food and Nutrition Services department at Missouri Baptist Medical Center's mission is to provide nutritionally balanced meals and medical nutrition therapy for patients, staff and visitors of Missouri Baptist Medical Center by working collaboratively with the facility's nursing, ancillary and medical staff professionals. Our licensed clinical dieticians provide medical nutrition therapy to both inpatients and outpatients as it relates to their personal medical needs. The department's patient services, production and retail teams work to deliver a variety of food offerings on a daily basis that are nutritionally balanced, healthy, flavorful and fresh for the hospital's patients, employees and visitors.

Performs a variety of food service inventory functions, including receipt and distribution of raw products/materials, storage of food, inventory maintenance, ordering and security, unloading trailers/pallets of incoming deliveries. This position must adhere to quality control standards, HACCP (Hazard Analysis Critical Control Points) procedures, food code regulations, and FSMA (Food Safety Modernization Act) guidelines as determined by the Manager. This position uses motorized equipment such as a pallet jack and pallet stacker and requires certification for these pieces of equipment.
Responsibilities
Receives, inspects, moves raw food products and non-food supplies from warehouse/storeroom to production locations as well as moving product to delivery/distribution points while utilizing motorized equipment and following safe food handling guidelines.Provides food and non-food supplies by accurately filling requisitions from various departments throughout the hospital and satellite areas.Orders raw food products, paper products, chemicals, and various other supplies through computerized ordering system(s). Performs inventory counts, reconciles, and makes visual checks of on-hand materials.Maintains inventory for storerooms, kitchen coolers and freezers.Maintains clean, sanitary, and orderly work areas such as the storeroom, coolers, freezers and all storage areas and transporting devices. Demonstrates knowledge of all safety processes, food code regulations, HACCP guidelines, and FSMA guidelines.

Essential Function/Core Competencies:
Assist with Activities of Daily Living (ADLs) and Personal Care: Provide support with personal hygiene, bathing, grooming, dressing, oral care, and elimination needs. Encourage and support residents to maintain the highest level of independence and use supportive devices when needed. Ensure comfort and cleanliness, including proper hydration and nutritional intake. All personal care provided should be documented at the time of service in the medical record.
Provide Companionship and Emotional Support: Engage with residents to provide companionship, encourage participation in activities and social functions, and support emotional well-being. Assist by accompanying to events or assist with scheduling transportation services as needed.
Medication Assistance and Documentation (medication technician only): Assist residents with the self-administration of medications in accordance with the five medication rights (right resident, medication, dose, time, and route). Document medication administration, care plans, and other services provided in electronic medical records (EMR).
Ensure Resident Safety and Comfort: Make regular rounds to ensure residents are comfortable, safe, and in no distress. Report changes in condition to the nurse supervisor immediately and assist with any required interventions.
Maintain a Safe and Clean Environment: Keep the resident's living area, common areas, and equipment clean, safe, and free from hazards. Follow infection control guidelines, ensuring proper sanitation and hygiene, including the use of gloves and other safety measures.
Provide Nutritional Support and Assistance: Assist with meal preparation, serving meals, and ensuring that residents receive adequate nourishment. Adhere to any physician prescribed diets. Monitor intake and output as directed, and maintain a constant supply of fresh water and extra nourishment when allowed.
Assist with Mobility and Transportation: Encourage and assist residents with mobility, whether it be walking, using assistive devices, or during transportation for appointments. Provide support for transportation when needed, including scheduling transportation and accompanying residents to outings.
Support in Admissions, Transfers, and Discharges: Assist with the safe admission, transfer, or discharge of residents. Ensure that personal items, clothing, and equipment are properly stored and labeled according to facility/agency policies.
Participate in Care Planning and Communication: Attend care conferences, in-service training, and team meetings. Contribute input on case progress, care plans, and any changes to the resident's condition. Report any discrepancies in care plans or concerns to the licensed nurse.
Provide Pet and Housekeeping Support: In certain cases, assist with pet care (walking, feeding, cleaning), as well as other light housekeeping tasks, such as cleaning and maintaining the living environment to ensure a comfortable and safe setting for the resident.

SUMMARY:
Under the supervision of the Enrollment Program Coordinator, the Enrollment Worker (EW) assists APHCV's patients and clients to enroll into health insurance and subsidy programs defined by the Enrollment Program Coordinator. The main goal of EW is to assist patients to enroll and maintain enrollment into health insurance and subsidy programs and to anchor patients to APHCV if they are interested. APHCV recognizes outreach as an optional strategy to achieve this goal. All duties and responsibilities shall be carried out according to APHCV's policies and procedures.
Although Enrollment Services are housed in APHCV three Health Centers, if directed by their supervisors, EWs are responsible for carrying out their duties at other APHCV service locations. Regardless of where they are assigned to work, EWs are expected to adhere to APHCV's quality, standards, and performance and implementation of APHCV's policies and procedures.
APHCV expects all employees to participate in APHCV's emergency response operations per our emergency policies and procedures.
APHCV requires all staff to comply with Standards of Conduct and Compliance Program related policies and procedures. Such compliance is part of this position's performance evaluation.

DUTIES AND RESPONSIBILITIES:
Enrollment Services:
Screen APHCV's patients and community members for eligibility in various health programs and CalFresh benefit and provide appropriate enrollment or referral or troubleshooting assistance.
Function as on-site enrollment specialist for APHCV patients including assisting patients in completing applications and preparing for the interview with the DPSS eligibility worker and assisting them stay enrolled.
Assist community members how to choose their Primary Care Provider (PCP), educate how to navigate health care system and to make medical appointments.
Assist APHCV patients with PCP and Health Plan assignment if they are interested in becoming APHCV patients or continuing to receive services at APHCV.
Assist seniors and disabled persons with Medicare and Medicare Part D enrollment, coverage, and supplemental programs as well as promote APHCV's clinical services to older adults and to Medicare eligible individuals.
Provide follow-up, case management and troubleshooting of client's and APHCV's patient's cases (including enrollment cases as well as managed care patients and others as assigned by Enrollment Program Coordinator and/or the Unit Manager).
Ensure proper documentation of all charts and follow-ups being made on monthly basis.
Enter enrollment and its related information into designated programs such as Covered CA, One- E-APP and/or Your Benefit Now program.
Maintain program participate files including enrollment documents, records of services accessed, and verification. Input participant enrollment information into CHOI or other database program on weekly basis.
Provide monthly report and telephone log to Enrollment Program Coordinator by 10

Outreach:
Outreach is conducted when it is evident that it helps achieve the EW's main function to enroll patients and community members. Outreach plan shall be approved by the Enrollment Program Coordinator.
Assume a liaison role between APHCV and appropriate community including businesses, temples, cultural centers, service organizations, schools, and other community institutions.
Plan and implement culturally appropriate outreach activities to enhance their success in enrolling patients and community members or help them stay enrolled.
Provide outreach productivity report and sign in sheet to Enrollment Program Coordinator and input outreach number into CHOI or other database program on weekly basis.
Translation/Interpretation for the Enrollment Services activities.
Conducts interpretation service for the Unit. If interpretation support is requested by other department, it shall be first approved by Enrollment Program Coordinator, and only when the protected scheduled time is set up to function as interpreter.
Translate health education for the Unit. This may include flyers, information about the program activities, articles for newspaper, and other program related activities in appropriate language as assigned.
Participate in the process to evaluate the accuracy of translation per APHCV procedures.
Type set or coordinates with other bilingual staff to type set translated materials.
Other:
Submit any other reports and forms to the Enrollment Program Coordinator when requested.
Provide the Enrollment Services office reception support, if needed.
Answers incoming calls, retrieves answering machine messages and returns calls.
Provide other support and assume other duties as assigned by Enrollment Program Coordinator or Unit Manager.

About the Position:
In our residential programs, the Substitute Worker has the responsibility of working closely with residents who are progressing toward independent living. The Substitute Worker provides the daily support needed for each resident to function at the highest possible level. These supports will include emotional support, demonstrating and teaching living skills, coordinating program activities, and facilitating groups, and client activities.
At our Crisis Respite Center (CRC), the Substitute Worker will assist individuals in managing their crisis and will offer information, advocacy, and referrals for continued assistance and support after they leave the CRC. The (CRC) is a program designed to provide any Sacramento / Sacramento County resident (18 years and older) who is experiencing a mental health hardship/crisis a place to go to receive support and gain hope for a resolution. The CRC is a person-centered program, providing a welcoming and non-judgmental environment for individuals who need relief from the stresses of life, and who need assistance in stabilizing their mental health crisis. The individuals who come to the CRC may be experiencing a wide variety of mental health issues.
Please note, this position is on an as-needed basis, there is no guaranteed hours and/or shifts.
Essential Duties & Responsibilities:
Support individuals with active listening, demonstration of empathy and carry a message of hope and recovery
Observe, assess, intervene and refer (within one's scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues
Develop service/goal plans with clients and meet with the clients as needed to encourage, support and advocate for progress and assist with skill development
Provide positive public relations, community referrals, information and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified
Accurately complete all necessary forms, reports and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, Medi-Cal billing, Avatar forms, HMIS)
Shop for food and household supplies to meet program or client needs
Ensure the office, house, rooms, units and yard are clean and fit for occupancy, where applicable. Provide and assist with cleaning as needed
Act as a designated Mandated Reporter for the observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults and elder/senior citizens. Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm
Ensure the protection and confidentiality of all Protected Health Information
Qualifications:
Ability to work with individuals who have a mental health diagnosis. Experience is highly desirable
Lived experience as a mental health consumer or a family member/loved one of a mental health consumer is desirable
Knowledge of Sacramento community organizations and community resources is highly desirable
Mental health crisis intervention experience is highly desirable
Experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse) is highly desirable
Knowledge of 12-Step Programs and/or personal experience with substance abuse recovery is highly desirable
Other Skills, Abilities, and Job Requirements:
Ability to pass clearance of the Federal Bureau of Investigation (FBI) and/or Department of Justice (DOJ) background screenings
