CaseManager I is an in-person role that delivers client services by coordinating care within the organization's continuum of services and with external human services providers. KEY RESPONSIBILITIES
Direct Service
Assist the client with intake by completing casemanagement assessment and entering financial and benefit information.

Assess the client's casemanagement needs and complete all releases of information.
Connect the client to benefits, mental health, physical health, employment, probation, DCFS, housing, community resources, outpatient substance use disorder services, and aftercare.
Coordinate communication and external service linkage including assisting with scheduling appointments, communicating with probation, scheduling child visits, communicating with DCFS, obtaining all court minute orders, providing appointment reminders for therapy and psychiatrist.
Deliver services in outpatient, residential, or field depending on assignments and client need.

Reports To: Community Supports Program Operations Supervisor
Seeking a full-time Housing CaseManager to join our CalAIM - Community Supports team. The Housing CaseManager will offer casemanagement services to Justice Involved (JI) individuals transitioning from homelessness to temporary or permanent housing or assist those formerly homeless in retaining housing. The ideal candidate will have a client-centered approach and provide excellent customer service, addressing the challenges faced by homeless individuals with medical or behavioral health issues as they transition to permanent housing. The ideal candidate will possess extensive knowledge of local resources and community agencies and demonstrate strong communication and problem-solving skills to effectively address members' needs. Experience conducting in-home visits, visiting homeless encampments, and interacting within correctional facilities is essential. Preference will be given to candidates with personal experience of the justice system or a proven record of working with individuals involved with the justice system.
Duties include but are not limited to serving as the main contact and advocate for justice involved individuals. This role includes outreach, housing assessments, creating Housing Support Plans, setting goals, addressing barriers, providing rental assistance, linking to health and support services, benefit establishment, transportation, crisis management, eviction prevention, client and landlord education, and housing location services. The position also requires maintaining active engagement and interaction with program-enrolled members to ensure proper coordination of care.
Candidate must have a minimum of 1 year experience working as a Housing CaseManager, as well as a background in working with individuals who have mental health conditions and substance use disorders. Candidates must have a flexible schedule that includes late evenings and weekends.

Provides outreach and follow up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, make needed referrals.
Facilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical psychiatric, financial, aftercare, etc.
Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues
Provides regular, comprehensive progress note charting in patient's Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning and other casemanagement activities.
Performs other duties as assigned by Director and/or Administrator.
EXPERIENCE
1. For AOD Programs:
Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP
1 year experience in a Behavioral Healthcare setting.
2. For all other Programs:
1 year experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff
3. Bilingual English/Spanish preferred
4. Valid driver's license and insurance

CATEGORIES OF DUTIES
General Duties:
* Conducts self in a professional manner in areas of dress, language, attitude and behavior.

* Assist with employment/vocational referrals, provide vocational counseling re resume preparation, job interviewing etc.
* Provides outreach and follow up services to patients in and out of treatment, to check progress, offer additional services, help reengage in treatment, make needed referrals.
* Facilitates discharge-planning efforts, commencing at time of admission on all assigned patients, including areas of housing, employment, medical psychiatric, financial, aftercare, etc.
* Makes direct contact within agency and with outside entities as appropriate, in order to assist patients in resolution of legal, financial, housing, medical, psychiatric and discharge planning issues
* Provides regular, comprehensive progress note charting in patient's Medical Record which document contacts, progress towards identified goals, program participation, referrals, discharge planning and other casemanagement activities.
* Performs other duties as assigned by Director and/or Administrator.
EXPERIENCE
1. For AOD Programs:
* Enrollment in an academic Addiction Studies program and registration with CCAPP, CAADE, or CADTP
* 1 year experience in a Behavioral Healthcare setting.
2. For all other Programs:
* 1 year experience in a Behavioral Healthcare setting (HIV programs) and DVSS Certification for DV specific staff
3. Bilingual English/Spanish preferred
4. Valid driver's license and insurance

The Martin Luther King Jr. Behavioral Health Center (MLK BHC) is a program of HealthRIGHT 360 and is contracted with the Department of Public Health's Substance Abuse Prevention and Control (SAPC) program. MLK BHC offers residential Drug Medi-Cal (DMC) services for a maximum of 99 adult individuals, comprising 33 beds each for judicially involved men, men, and women. MLK BHC collaborates closely with other Behavioral Health Center programs to ensure a comprehensive range of services for low-income and Medi-Cal-eligible individuals. Situated on the Martin Luther King Jr. Hospital campus in the Willowbrook area of South Los Angeles, the program serves residents of all Los Angeles County.
CaseManager I is an in-person role that delivers client services by coordinating care within the organization's continuum of services and with external human services providers. They collaborate closely with clients to assist in completing intake paperwork, outcome assessments, and applying for benefits. Working alongside a multidisciplinary team, CaseManager I ensures optimal treatment outcomes aligned with HealthRIGHT 360's philosophy, goals, policies, mission, and vision. Services are provided flexibly across outpatient offices, residential treatment settings, or in the community as dictated by client needs and program requirements. Therefore, a reliable transportation is required.
KEY RESPONSIBILITIES
Direct Service
Assist the client with intake by completing casemanagement assessment and entering financial and benefit information.
Support the client in applying for Medi-Cal or transferring Medi-Cal County when appropriate.
Assess the client's casemanagement needs and complete all releases of information.
Connect the client to benefits, mental health, physical health, employment, probation, DCFS, housing, community resources, outpatient substance use disorder services, and aftercare.
Coordinate communication and external service linkage including assisting with scheduling appointments, communicating with probation, scheduling child visits, communicating with DCFS, obtaining all court minute orders, providing appointment reminders for therapy and psychiatrist.
Deliver services in outpatient, residential, or field depending on assignments and client need.
Complete the VI-SPDAT and connect the client to safe housing options.
Prepare the client for job search by assisting with resume writing and job search preparation.
Monitor client's progress toward achieving casemanagement treatment plan goals and provide treatment plan input.
Seek clinical consultation throughout the treatment episode and when encountering new and/or high-risk clinical circumstances.
Work collaboratively with other members of the treatment team including the Clinical Therapist, Counselor, Medical, and Psychiatrist.
Communicate collaboratively with all members of the behavioral health team including medical, mental health, psychiatry, substance use disorder, and other staff.
Transport clients as needed.
Complete client UA tests and document test results.
Documentation
