Horizon House is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strengths and choices.
Our Delaware County Program is seeking a Blended CaseManager to ensure that all individuals receive the services they need to live, learn, work and socialize in the local community. The Blended CaseManager will provide a continuous flow of services, in compliance with contract and Agency regulations
This position also has the responsibility to identify, provide and maintain regular supports to the clients and to assure continuity of care;
Participate in the development and refinement of individual rehabilitation plans incorporating social, educational, vocational, self-care and residential skills and objectives; complete Functional Assessment with clients; provide guidance and review accessibility of services; update and amend plan as client negotiates through the system.
Participate in the resolution of crisis situations; assist with identifying key individuals and resources that can provide direct support and/or immediate intervention during a crisis situation.
Serve as an advocate and encourage clients to assume a proactive role in their rehabilitation process. Provide direct support through effective communications with program staff, clients and other providers.
Maintain a close liaison with the local community and area service providers/agencies to facilitate program cooperation and provide for constructive input and/or alternative resources to current plans.
May assist Benefits Manager in monitoring the management of client's Representative Payee Accounts.
B.A/B.S. degree in Human Services/Social Science, Healthcare or a related field and 3 years of MH direct care experience; or any combination of experience, education or training that would provide the level of knowledge. skill and ability required.

Specific Job Responsibilities
Under the direction of the ELECT Supervisor, the CaseManager will:
* Maintain a caseload of up to 25 students and work with the student's directly at their school, home or community sites on a weekly basis.

Provide Family Finding Services Including:
* Communicate with the assigned DHS or CUA worker in writing and via phone after the case was assigned. If communication does not occur after three consecutive phone calls and follow-up e-mails, the FF supervisor will be notified to assist. Initial questions for CUA and intake should be used (for children under 12) during this initial conversation to obtain accurate and complete case background, as well as request documentation.
* Visit with the youth or make contact via phone, depending on location by the 7th day of service and then every month as appropriate. In-person intake MUST occur barring unusual circumstances.
* Send letters to required parties by the 15th day of service; follow-up with a phone call and/or follow-up letter as needed. May be required to visit in person if no response. Follow protocol to document other family information and provide proper notifications based on success or lack thereof of this effort.
* Follow through with identified supports ONGOING during casework. If supports are identified early in the case, the casemanager is expected to begin moving forward with their level of support. This will be specific to each family, but should be an ongoing process during each case.
* Complete reports including a detailed background, including names and DOBs for all kids being served, the reason for placement or system involvement, situation of bio parents, brief case history, current placement details, court history, and identification of program court-ordered status (if applicable).
* Complete timely supervision notes that include: case background, updated family list, notes about communication with the team, progress on the case AND identified next steps. Steps from the previous note should be completed and identified as completed.

SERVICE AREA: Housing & Justice Related Services (HJRS) PROGRAM: Forensic Intensive Recovery (FIR) The FIR CaseManager works directly with participants who are court-stipulated for treatment services with oversight from the Adult Probation and Parole departments. The primary responsibilities are to ensure that services are provided to participants and documented according to DAP, CBH, Forensic Services, and PHMC requirements. This individual will provide support to the participants while working with all levels of management within the treatment program, Forensic Services, and the recovery community.

RESPONSIBILITIES:
Responsibilities include, but are not limited to, the following:
* Provide casemanagement services to a chemically dependent forensic population, with a caseload of up to 45 active participants.
* Establish and maintain service productivity goals at minimum designated service units per week.
* Document service provision within 24 hours.
* Participate in monthly team meetings and scheduled case conferencing with Probation, meeting reporting requirements.
* Participate in biweekly supervision meetings.
* Collaborate and attend meetings with Probation/Parole, treatment providers, and other collaborating agencies.
* Complete discharge summaries within timeframes specified by the program.
* Assist individuals to link to recovery support services, including PRO-ACT recovery center, Access to Recovery, faith-based institutions, mental health services, and other identified community supports.
* Create weekly schedules detailing activities to be completed for each week.
* Maintain participant records in accordance with agency fee-for-service standards, managed care, and licensing requirements.
* Prioritize assisting participants to enroll in treatment, physical health services, DPA, vocational services, and educational services.
* Identify, coordinate, and link community resources to each participant to promote accessibility to recovery-oriented community services.
* Coordinate and communicate with other programs within and outside the agency to foster a collaborative team approach.
* Spend an average of 80% of each workweek in the field for off-site participant appointments and meetings.
