Included in this nursing area is a 10-bed Rehabilitation Unit which provides services between acute and long-term care. The patient care team researches patient lifestyle, function and mobility, and customizes care plans to return patients to their home setting. Preferred Qualifications Role Purpose Provides direct patient care activities including assessment, diagnosis, planning implementation, and evaluation within the guidelines of the standards of nursing care. Responsibilities Promotes patient and family centered care in a healing environment. Educates patients and their families on how to manage their illness or injury, including post treatment home care needs and medication administration.
