Provide psychosocial assessments of patients and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and/or life situation.

Visit notes and orders are completed and transmitted in a timely manner according to policy. Corrections to care plans are entered and transmitted according to time line.

Implements social work plan that results in: a) enhanced strength of family systems, b) patient/family/caregiver utilization of community resources, c) maximization of medical benefits, d) enhanced environment for care delivery, e) dignity for the dying patient, f) maximized patient/family coping skills, g) support for patient/family cultural beliefs and values. Evaluates effectiveness of social work plan of care and modifies intervention as indicated.

This position will be a part of the City of Peoria Social Services Unit (SSU). SSU personnel are tasked with assisting those who come into contact with, or are referred to, the SSU. One of the goals is to promote safer encounters between citizens in need of crisis services and police department personnel while maintaining a high level of professional service. The other main goal is to refer to, and encourage participation in, appropriate mental health, substance use, or other needs-based services that may fit the individual. These duties are critical to an overall healthier, safer community and are aimed at facilitating more stable community members. This position provides crisis intervention services/assessments/evaluation and stabilization/case management to people in a primarily community-based environment in collaboration with the Peoria Police Department (PPD). This role will include field response into the community in collaboration with the PPD, likely several times per day, to provide mobile mental and behavioral health services to citizens within the city of Peoria, Illinois. This role will consist of supervision of LicensedSocialWorkers and Licensed Clinical SocialWorkers, clinical oversight for staff, continued follow up and case management to connect citizens to ongoing services and supports.

Responsibilities:
As a Hospice Clinical SocialWorker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical socialworker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Experience or education in grief counseling preferred.

Responsibilities:
As a Hospice Clinical SocialWorker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Required Certifications and Licensures:
Licensed to practice as a clinical socialworker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

The Adult Military and Family Life Counselor (Adult-MFLC) position offers a unique opportunity to serve a vital role, in providing non-medical support, which includes counseling, consultation, and outreach, to service members, military family members and eligible staff. Non-medical counseling is preventive in nature and aims to address and provide solution focused approaches to issues before they become greater challenges.
Service members, spouses and family members face unique emotional and environmental challenges due to the stress of military life. This is an opportunity for you to reach them where and when you are most needed.
Adult-MFLC responsibilities include:
Face-to-face counseling to military service members and their families (individuals, couples, and families).
Observe, participate, and engage in activities with service members, military spouses and families; provide coaching, guidance and support to installation staff; and model behavior management techniques for staff and military family members.
Offer presentations to service members, military families, and military leadership at military-connect events.
Promote creativity and positivity through approved materials.
Build rapport with parents and families by attending installation and community events.
Help with transition adjustments, such as a new school and a new home.

Licensure Portability
Great news! Under the recent National Defense Authorization Act (NDAA), licensed Military and Family Life Counselors may provide non-medical counseling services to military families at any location in a U.S. state, the District of Columbia, or a territory or possession of the United States, regardless of which state the counselor is licensed in. Within the scope of the MFLC program, a counselor may hold any independent and clinical license recognized by the Secretary of Defense as an appropriate license for the provision of non-medical counseling services.
Minimum Requirements:
A Master's degree or higher in the Behavioral Health Professions (MS/MSW/MA/PhD).
Licensure to practice at the independent practice level (LCSW, MFT, LPCC, Psychologist) in any US State.
Must be a U.S. citizen and have an active passport that is good for 6 months after the end of the assignment.
Must be able to pass a basic background check for employment, a Child National Agency Check, and Inquiries (NACI/CNACI) clearance, commonly called a Public Trust clearance.
Pay Range $55,250.00 - $99,875.00
