Responsibilities: The Family Centered Treatment Therapist- Licensed provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Essential Duties & Responsibilities
Provide Family Centered Treatment (FCT) Clinical Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal training within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT clinical services to children, adolescents, and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Develop and coordinate discharge plan based on treatment, indicating all relevant aftercare needs and plans. Access community resources that would be beneficial to client aftercare needs.
Coordinate and oversee the initial and ongoing assessment activities.
Convene the Treatment Team for Service Plan.
Consult with identified medical (e.g., primary care and psychiatric) and non-medical (e.g., Dept. of Child Services, school, Dept. of Juvenile Justice) providers; engage community and natural supports and include their input in the person-centered planning process.
Provide and coordinate behavioral health services and other interventions for the youth or other family members with other behavioral health professionals and Treatment Team members.
As a licensed clinician working for Health Connect America your NPI number will be used to bill for services performed by those being supervised by you.
As a licensed clinician you will be responsible for ensuring that your credentials remain current, you comply with all training requirements, and you are adequately supervising your staff.
Qualifications:
Master's degree in Social Work, Counseling, or related field.
Must hold a current professional license in state where services are provided including LPC, LMFT, LCSW, or one of the state-specific licenses listed below.

START serves the many - by bringing cancer trials to physicians and their patients in community hospitals and practices when hope is needed most.
The Director, Clinical Research / Genitourinary (GU) Clinical Investigator is a physician-scientist with demonstrated clinical research team leadership skills responsible for the overall preparation, conduct, and management of GU Oncology clinical trials including Phase 1 studies. This role will manage the development and execution of clinical trial protocols, contribute to data analysis, and maintain strong communication and alignment with industry sponsors while maintaining compliance with all regulatory requirements and institutional policies. **Position Summary**
Technical Account Management (TAM) is responsible for playing a key/critical role in realizing business value through the application of project management knowledge, skills, tools, and techniques to meet project objectives. The TAM will also use their rich healthcare domain expertise, along with project management and proactive consulting skills, to solve complex technical challenges for some of the largest pharmaceutical manufacturers in the country. Essential Duties and Responsibilities:
- Enter any missed build information into the software for the doctor to be able to utilize during and after the appointment.
- Ensure providers have the necessary documentation and medical records to properly evaluate Veterans.
- Research medical conditions and new information when necessary in order to assist builders with any case questions during the build process.
- Identify and confirm that all relevant worksheets and diagnostics were added during the build process as requested by the VA.
- Track Case Builder (CB) errors and monitor progress of assigned builders through weekly audit reports and master error log.
- Communicate with CB supervisors when patterns of concern regarding quality and production are identified.
- Communicate with other departments to share relevant information when necessary in order to best complete the case.
- Thoroughly checks over and approves Case Builder's work when in audit, to make sure the build is sufficient.
- Complete audits as assigned by Supervisor or Case Building Management.
- Assists with clarification response (CR) updates when a CB on the build team is out of office.
- Complete one-on-one conferencing with assigned Case Builders to review error trends and provide build feedback with the goal of improving assigned Case Builder quality.
- Responds promptly and appropriately to messages from supervisors, co- workers, and other departments.

**Your Role**
As a Medical Dosimetrist, you will:
+ Design and calculate accurate radiation treatment plans for a variety of techniques including IMRT, VMAT, SBRT, and SRS.
+ Collaborate with physicians and physicists to optimize treatment plans for safety and efficacy.
+ Ensure compliance with departmental protocols and regulatory standards.
+ Support implementation of new technologies and treatment techniques.
**Qualifications**
**Minimum:**
+ Graduate of a JRCERT-accredited Medical Dosimetry program or equivalent.
+ Certified Medical Dosimetrist (CMD) or eligible for certification.
**Preferred:**
+ Experience with Eclipse TPS and Aria R&V.
+ Familiarity with advanced techniques such as SRS and SBRT.
**Physical Requirements**
+ Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.
+ Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.
+ Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.
+ Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.
+ Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc.)

Primary Function/General Purpose of Position
Provides in-person guidance and support to the providers, care teams, and affiliates in clinical integration initiatives that supports the quadruple aim and allows the organization to thrive in a value-based health care model. Key responsibilities include proactively maintaining a deep understanding of the key drivers for each practice, translating the data to insights for the practices, collaboratively identifying actionable steps for improvement through a Plan-Do-Study-Act (PDSA) process improvement model, coaching care teams in process improvements, clinical integration, and outcomes.
Essential Job Functions
Completes In person and virtual rounds in the practices on a regular basis to communicate status reports, key drivers of change, leading or lagging indicators, and providing insights to providers and practices
Manages the review of clinical outcomes, patient panel management, and performance metrics for affiliates and employed physicians in the assigned market or Region's network
Partners with providers and care team members to implement evidence-based protocols and other improvement initiatives based on the insights, monitors ongoing progress, document supports, identify resources, and engages care delivery team
Maintains being a professional role model in practice transformation and under leadership guidance, effectively engages and educates regional stakeholders on the elements, measures, tasks, and tools to be used to support various practice improvement activities
