Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness Care is an independent, nonprofit healthcare provider licensed in 11 counties in northwest Illinois. We provide exceptional serious illness care, hospice care and grief support for adults and children. The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 24-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices. We currently have a part time opening for a Triage RN working 24 hours per week. The Triage RN works two (2) twelve (12) hour shifts weekly from 8 am - 8:30 pm and requires flexibility in days of the week. This position responds to calls from patients/families regarding symptom/pain management and responds in a timely manner. The Admissions Nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education and experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Patient Care * Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing. * Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment of all current and previous health history. * Evaluates patient status by: * Writing and initiating plan of care * Regularly re-evaluating patient and family/caregiver needs * Participating in plan of care revision as necessary * Uses health assessment data to determine nursing diagnosis. * Develops a plan of care that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Includes patient and family in the planning process. * Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed and determined by the physician's plan of care. * Provides health care instructions to the patient as appropriate per assessment and plan. Completes, submits, and maintains accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management and changes/outcomes as appropriate. * Communicates with the physician regarding patient needs and reports changes in patient condition; obtains/receives physician orders as required. * Assists the patient with the activities of daily living and facilitates the patient's efforts toward self- sufficiency and optional comfort care. * Provides and maintains a safe environment for the patient. * Counsels and works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. * Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician. Recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with interdisciplinary care team. * Coordinates plan of care with relevant community health partners. * Ensures arrangements for equipment and other necessary items and services are available. * Acts as RN Case Manager when assigned by Clinical Supervisor and assumes responsibility to coordinate patient care for assigned caseload. * Responsible for maintaining compliance with all HIPAA regulations and requirements. Treats all patient information as confidential and utilizes protected health information on a minimum necessary basis. This role will be based in our growing Lincolnshire IL branch. Essential Functions and Skills * Receives case referrals. Reviews available patient information related to case, including home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with the Medical Director or Hospice Physician regarding any questions about an individual's eligibility for services. * Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality hospice care services. * Available at all times during operating hours to assist clinicians as appropriate. * Reviews patient's medical diagnosis, prognosis, medications, procedures, and clinical course. * Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plans of care. * Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement. * Conducts quarterly record reviews and communicates findings and recommendations to the Manager of Clinical Services and hospice personnel. * Assists in the orientation of new employees. * Assures proper maintenance of clinical records in compliance with local, state and federal laws. * Assists Manager of Clinical Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in implementing organization policies, procedures, goals and objectives, both short and long range. * Complies with accepted professional standards and principles. * Assesses the health needs of the community and assists in the development of plan(s)/ program(s) to meet needs. Participates in public relations and community activities that promote the organization's role as an effective member of the hospice and health care delivery system. * Promotes customer service orientation to all hospice personnel. This position works a flexible schedule with minimum of 4 x 8 hrs shifts per month consisting of Friday 1159pm-8am, and Saturday 1159pm to 8am. Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness is an independent, non-profit healthcare provider licensed in 11 counties in northwest Illinois. We have a state-of-the-art facility and the first free standing in-patient Hospice Home in Illinois. We have a 35 year history for providing compassionate professional care to terminally ill patients and their families. We are state licensed and Medicare/Medicaid certified. The majority of our direct patient care is in the field with Lightways team members providing care in the home, nursing facility or assisted living facility. We also have a 20-bed inpatient unit in Joliet. Non-clinical staff are based out of our Joliet administrative offices. We are currently seeking a full-time RN to care for our patients residing in their private home, nursing home or long term care facility in DuPage County. Our RN Case Managers have a caseload of patients that they visit according to the plan of care and are responsible for providing loving care for patients in their activities of daily living. This position works Mon-Fri and must have the availability to do a minimum of 20 hours of on-call per month which may increase based on staffing. On-call shift coverage is for our entire service area. Illinois Registered Nurse with a minimum of one year experience in either home health or nursing home/long term care. Previous Hospice experience helpful.
