Works with leadership to develop risk mitigation plans associated with litigated claims. Ensures that patient care-related incidents are reported to Trinity System Office, CMS, and/or the Department of Health as required by law. *Specific Activities* * Conducts case finding by daily review of reported events and other information retrieved from other sources (i.e., verbal report, phone report, electronic submissions from MIDAS, and other referrals) and initiates appropriate follow-up. * Communicates with regulatory agencies, as needed, including, but not limited to: the NYS Department of Health (DOH), FDA, and others. * Coordinates overall functions including but not limited to: NYPORTS, NIMRS, Justice Center, STARS/ClearSight, Centers for Medicare and Medicaid Services (CMS) death reporting in restraints; ensuring that reporting criteria are met. * Demonstrates a strong ability to identify, analyze and solve problems. * Uses appropriate tools when conducting root cause analysis, failure mode and effect analysis, gap analysis, other risk assessments. * Promotes an environment of learning and safety. * Is readily available to all staff as a resource. * Competent with data display and analysis * Additional duties as assigned.
