Plan, oversee, facilitate and document patient's recovery.

Required Education, Skills, and Qualifications:
HS graduate or equivalent required;

Qualifications
Master's Degree in Social Work, Psychology, or related discipline from an accredited graduate school

Job Summary:
The Substance Use Disorder Professional role is part of the Behavioral Health and Counseling team that supports ETS' mission through serving as the primary care team coordinator for their assigned patients.
This role makes a critical difference at ETS by providing assigned patients with, assessment, individual and group counseling, case management and coordinate overall patient care while enrolled at Evergreen Treatment Services.
The position is full time onsite, and the days and hours are Monday through Friday, 5 AM - 1:30 PM. (Hours may flex to a later time depending on program needs. Any changes to work schedule is negotiated with employee's supervisor after hire.)
What you contribute to this role - Responsibilities:

Experience of substance abuse treatment planning and counseling.
Knowledge and Skills:

Ability to establish and maintain effective working relationships with clients, patients, and staff from a wide variety of ethnic, socioeconomic, and cultural backgrounds.

Job Summary:
The TIM (Treatment in Motion) SUDP (Substance Use Disorder Professional) position is part of the Counseling team in Behavioral Health and supports ETS's mission by providing assessments, individual and group counseling, case management, community outreach, and comprehensive care for participants enrolled in the Treatment in Motion program.
This role is essential to ETS's mission, serving as the primary care team coordinator for assigned participants and connecting them to the Evergreen Treatment Services Mobile Dispensary (TIM) as well as other community and clinic-based resources through outreach and engagement.
What you contribute to this role:
Essential Duties & Responsibilities
Complete an initial ASAM assessment.
Provide the initial intake when needed.
Facilitate individual and group counseling sessions.
Maintain accurate clinical documentation of all engagement with participants and external stakeholders.
Create and update individualized treatment plans.
Provide community-based SUD services for individuals seeking and participating in MOUD services.
Regular travel in support of community-based MOUD services as required.
Provide timely responses to positive urinalysis/breathalyzer results.

Substance Use Disorder Professional Job Responsibilities:
Provide a quality treatment experience for clients by engaging in and modeling healthy relationship skills while assessing treatment planning, and counseling with clients in group and individual settings.
Communicate client's needs, progress, problems, and recommendations, to treatment teams and various agencies involved in the client's life. When required, represent the client in court.
