The Community Health Representative will be responsible for assisting in the implementation of medical and educational services in a caring, professional manner to eligible Tribal Members in the local community. The Community Health Representative will be working within the boundaries of the Klamath Tribes service area (Klamath County). Routine duties shall include providing health care related services to individuals eligible for services provided by Klamath Tribal Health & Family Services in homes, schools, clinics, alternate job sites and other community locations within the Klamath Tribes' Service Unit. Serve as a liaison and advocate between medical staff, other health facilities, social services and/or other resources on behalf of the patients needs. 2.
