Create timelines and task lists, verify applicable program compliance requirements, identify priorities, and facilitate communications between programs and clients. Determine client needs, identify resources to fill needs, facilitate client connection to resource provider, monitor client interactions with providers, and provide supportive services as needed to improve outcomes or remove barriers. * Guide clients in recovery and goal setting, and assist them with identifying personal strengths. Provide coaching to clients on resolving problems that may arise during recovery and self-sufficiency. Provide clients with short-term crisis intervention and stabilization, and support clients with establishing long-term self-sufficiency plans. * Initiate and maintain positive working relationships and regular communications with clients and other agencies.
