Job Description
1. Medication Administration
a. Demonstrates knowledge of and follows SSH policies and procedures for administering, transcribing, and recording medications.
b. Completes medication reconciliation process following SSH policy and procedure.
c. Demonstrates proper procedure for the documentation of narcotic withdrawal, administration, verifies count, wastes per policy and resolves narcotic discrepancy.
2. Plan of care/Documentation/Patient Family Centered Care/Patient Experience Develops, discusses, and communicates a prioritized problem list/plan of care for each patient.
a. Develop, evaluate and update individualized plan of care for patient and documents outcomes.
b. Initiating admission assessment within 8 hours of admissions, identifies and documents patient/family/significant other teaching needs upon admission and throughout hospitalization.
c. Continues assessment/reassessment and identifies care needs within established nursing practice.
d. Documents all patient care following the department of nursing policy, unit based standards, disease processes (CHF, PNA vaccines) and nurse sensitive indicators (falls, skin, CAUTI)
e. Assess/reassesses and documents patient's level of comfort utilizing the appropriate pain scale and the patient's response to each intervention both pharmacological and non-pharmacological per South Shore Hospital policy.
f. Works on behalf of patient /family. Seeks help to represent patient/family when they are unable to represent themselves.
g. Raises ethical questions and concerns with clinical team. Seeks available resources to help formulate and understand ethical decisions.
h. Actively communicates plan of day via white board updating every shift.
i. Facilitates discharge/transfer by goal of 12:00pm, communicates barriers utilizing chain of command.
3. Safety/Quality - Foster's a "Culture of Safety" through personal ownership and commitment to a safe environment.
a. Verifies patient identification with two identifiers prior to the start of any invasive procedure, including "time out", administration of care, medications, labeled specimen's an documents confirming the correct patient, procedure, site, equipment and consent.
b. Complies with the current CDC hand hygiene guidelines through proper handwashing. Adheres to universal precautions, makes appropriate use of personal protective equipment at all times and appropriately disposes of hazardous materials. Maintains awareness of MSDS sheets and how to access.
c. Ensures environment meets regulatory requirements at all times.
d. Understands and is able to demonstrate individual roles and responsibilities in the event of hospital codes/emergency preparedness.
4. Professional Development: Assumes overall responsibility for own professional development by incorporating evidenced-based practice, research, and performance improvement initiatives as a part of ongoing nursing practice.
a. Obtains at least 5 contact hours per year in area of practice.
b. Practices within the legal boundaries of MA Nurse Practice Act. Directs other licenses and non-licensed personnel as assigned.

Shift:
Day (United States of America) Reporting to the Chief of Cardiovascular Medicine for clinical issues and the Director of Cardiovascular Medicine for administrative issues, the Cardiovascular Nurse Practitioner will work with the Cardiovascular physicians and particularly with the Interventional Team to facilitate the assessment, evaluation, preparation, procedures and follow up of interventional cardiology patients.
Compensation Pay Range:
$107,000.00 - $224,123.00
1 - Facilitate the assessment and work up of cardiology patients in any environment including, but not limited to: ED, Noninvasive lab, CCL, Inpatient floors.
a - The Staff Physician Assistant will be responsible for obtaining and coordinating the following general items: history and physical, laboratory results, charting of clinical findings, admission orders, nursing instructions, pre-Cath orders, clinical documentation required by various registries and other required elements.
b - In collaboration with the Cardiovascular Access Coordinator and Hospitalist Service, will coordinate admissions and transfers to the hospital, and ensure that informed consent for procedures is obtained at the appropriate time.
c - Perform pre-procedure assessments in the Recovery room/holding area, Emergency Department, Inpatient Floors, or outpatient setting as needed.
2 - Interface between Cardiac Catheterization Laboratory and Recovery Area:
a - Maintains knowledge about the Cardiac Catheterization Laboratory scheduling, procedure room turnover and availability of resources including bed availability.
b - He/she will be called upon to manage patients after their procedures and while they await transfer to patient floor, and will assure availability of equipment for special procedures.
c - Ensure blood bank preparation and formal or standing surgical backup, when required for patient care.
d - The Cardiovascular Physician Assistant will assist in communicating with the patients families at all intervals of the care process and treatment.
3 - Medical Treatment and Post-procedure Care
a - The Staff Physician Assistant will be responsible for all aspects of care under the supervision of the attending physician, including but not limited to: fluid and electrolyte management, nutrition, pain management, transfusion therapy, anticoagulation management and patient education.
b - Post procedure, the Physician Assistant will manage vascular access, monitor and manage arrhythmias, pacing, drug therapy, O2 therapy, and other non-cardiac medications, under the supervision of the attending physician.
c - Assist in the evaluation and management of clinical events such as fever, altered hemodynamics, CHF, pulmonary complications, abrupt vessel closure, arrhythmias, access site complications and others.
d - In collaboration with the Cardiovascular Access Coordinator and the Hospitalist Service, assist in transfers to and from the intensive care units.
4 - Procedure responsibilities:
a - Demonstrate knowledge and proper use of all required equipment and supplies used during diagnostic and interventional procedures performed in the Cardiac Catheterization Laboratory and other clinical areas.
b - Demonstrates sterile equipment and instrument set up according to standards including: flushing and storage of guidewires, sheaths & coronary catheters; preparation of stent and balloon catheters; proper set up and use of medications and contrast on sterile field; Proper set up and continuous flushing of manifold to assure there is no air in system.
c - Provide support as 2nd operator to assist the attending physician during routine/elective diagnostic and interventional procedures performed in the Cardiac Catheterization Laboratory and other clinical areas.
d - Provide support as 2nd operator to assist the attending physician during emergency on call diagnostic and interventional procedures performed in the Cardiac Catheterization Laboratory and other clinical areas.
e - Maintains knowledge and competence regarding radiation safety and use of fluoroscopy during interventional procedures.
f - Demonstrates ability to gain and secure vascular access including: ability to properly access femoral artery & vein; proper sheath placement.
g - Demonstrates ability to perform catheter, sheath and guidewire exchanges including: short exchange; long exchange; sheath; catheter;
guidewire (standard & angioplasty)
h - Demonstrates ability to insert Angio seal/Vaso seal device
i - Demonstrates ability to properly prepare and use ancillary equipment and supplies including: Intra-Aortic Balloon pump; Temporary Pacemaker and catheter; Thrombectomy devices.
5 - Post Procedure Care:
a - Performs vascular assessment and confirm assessment findings: Skin color and temperature, Capillary refill, Pulses, Edema, Sensation/strength

b - Properly and safely performs sheath removal
c - Demonstrates knowledge of actions and data required for patient transfer
d - Performs patient education regarding discharge including preventive care and risk factor reduction; assists in coordinating health related services (Cardiac Rehab/Home Care, etc.).
e - Writes discharge orders, prescriptions and dictate discharge summaries in collaboration with the attending physician.
6 - Education and Professional Development
a - Attends internal and external education meetings and read appropriate professional journals in order to maintain and enhance skills and clinical competence.
b - Completes 100 hours of continuing medical education necessary during each 2-year period of employment.
c - Attends all Cath Lab Staff Meetings and Interventional Physician meetings (scheduled and ad-hoc)
d - Analyzes causes of problems, identifies and evaluates alternative solutions, and selects appropriate solution, and communicates appropriately and effectively utilizing chain of command.
e - Provides teaching to students, house staff, nurses, technologists and cardiology fellows as instructed by the attending physician, particularly with regard to the evaluation, interventional treatment and longitudinal care of patients with cardiovascular disease.
f - Demonstrates conflict resolution amongst colleagues
g - Assumes an active role in keeping informed about changes in hospital policy, procedure, and equipment.

Day (United States of America) The Oncology RegisteredNurse is focused on patient assessment, patient education, the coordination of care, administration of therapy, symptom management and supportive care for the Oncology and Hematology patients at Dana-Farber Cancer Institute. The RegisteredNurse cares for individuals receiving therapy for oncologic and hematologic problems. The nurse's responsibilities include assessment, planning, implementation and evaluation of nursing care for a complex patient population. The nurse's role includes administration of chemotherapy and other medications and infusions, telephone nursing care, direct patient care and charge capture/billing for all services rendered. The nurse provides patient and family education and psychosocial and emotional support. He/she is a member of a multidisciplinary care team that requires effective communication and collaboration with co-workers and staff as well as collaborating institutions and agencies. The role requires clinical expertise, critical thinking, collaborative practice, excellent communication skills and an understanding of the principles and practices of clinical research.

PRIMARY DUTIES AND RESPONSIBILITIES:
A. Delivers expert, evidence-based nursing care to patients and families.
1. Implements nursing process: Assessment, Nursing Diagnosis, Planning, Implementation, Evaluation.
2. Assesses patient and family needs.
3. Promotes health care outcomes in concert with currently accepted clinical practice guidelines.
4. Analyzes assessment data to develop and support clinically accurate diagnoses.
5. Collaborates proactively with all interdisciplinary team members with a patient centered focus to facilitate and maximize patient healthcare outcomes.
6. Plans care by establishing goals with measurable outcomes.
7. Implements the plan of care.
8. Evaluates the patient's response to interventions.
9. Documents care according to Institute and regulatory agencies' requirements.
10. Administers medications and other treatments, which may include chemotherapy, and performs procedures safely and efficiently in accordance with Institute policy. May administer blood transfusions.
11. Evaluates educational and psychological needs of patient and family and incorporates identified needs into the plan of care.
12. Educates patient and family regarding treatment modalities of disease, which may include chemotherapy and radiation therapy and symptom management.
13. Reviews patient's clinical status and laboratory values pre-treatment and consults with provider when appropriate.
14. Evaluates and determines need for patient referrals to other members of the interdisciplinary needs as appropriate.
15. Identifies appropriate community resources to promote continuity of care and makes appropriate referrals.
16. Manages patient care across the continuum, making appropriate referrals to home care and hospice, as necessary.
17. Prepare and administers chemotherapy/biotherapy and supportive therapy in accordance with ASCO/ONS Guidelines.
18. Recognizes and responds to emergency situations appropriately. Maintains BLS certification.
19. Provides telephone triage according to policies and documents telephone interactions.
20. Performs the following technical skills:
a. Intravenous therapy including peripheral and central venous vascular access devices, including obtaining IV access
b. Safe handling practices for hazardous, cytotoxic and biological agents
c. Chemotherapy administration skills, including vesicant and non-vesicant medications
21. Complies with accepted standards of professional nursing practice, as well as with all licensure/registration requirements applicable to the nursing role. Supports the DFCI Nursing Department Professional Practice Model and Quality Plan.
22. Accurately records charges for all services rendered on the day service.
23. Collaborates with Clinic Assistant to assure accurate inventory of all supplies.
24. Serves as a resource to medical personnel in other health facilities, providing telephone consultation to engender optimal Oncology practices and knowledge among health care professionals caring for cancer patients.
B. Establishes rapport with patients and families from a variety of cultural, ethnic and religious backgrounds.
1. Interacts with patients and families in a caring and compassionate manner respecting the need for privacy and confidentiality.
2. Develops approaches to problem solving which meet the individual needs of the patients and families.

1. Schedule
Completes weekly schedule and makes scheduled home visits per assignment. Schedules and prioritizes nursing visits according to client acuity and in accordance with the organization's emergency preparedness plan

Manages individualized, outcome oriented nursing care through the use of the nursing process. This Nurse will provide individualized, outcome-oriented nursing care through the use of the nursing process. Provides care, treatment and services through the successful coordination and completion of a series of processes that include assessment, planning care, providing care, coordinating cross continuum care and assessing outcomes of interventions in accordance with system, departmental and ambulatory policies and procedures.
This role provides clinically-based, risk reducing care and coordination that facilitates the delivery of cost-effective quality healthcare along the continuum. It directly interfaces with physicians, care managers, health care teams, patients and their caregivers to achieve desired patient outcomes. ESSENTIAL FUNCTIONS

1 - The NP practices in collaboration with a supervising physician.
a - A physician reviews all historical and physical data according to medical staff by-laws and regulations.

2 - In any emergency situation, a NP may render emergency medical service necessary to avoid disability or death of an injured person until a doctor arrives.

5 - The responsibilities of the wound NP, Podiatry Services will include but not be limited to the following:

b - Assist in the overall preparation of the patient for surgery. Perform daily patient rounds and follow up on all patients and consults including physical exams, diagnostic and therapeutic intervention and order routine labs, diagnostic tests and consults after conferring with the MD
c - Participate as assigned in surgical operating procedures.
d- Provide initial interpretation of EKG and CXR and initiate definitive treatment.
e - For inclusion in patient medical records, write pre and post op orders, brief op notes, procedure notes, progress notes, transfer and discharge orders, write prescriptions. Dictate discharge summaries.

g- Perform BLS and ACLS measures as needed.

b - Verbal Orders: Verbal orders may be communicated from the supervising physician to the NP, who should promptly write such orders in the patient chart (on physician order sheets). The NP shall indicate the order is being written per the verbal order of the supervising physician. In the emergency, the NP may give verbal orders.
c - The nurse is responsible for the administration to the patient.

Shift:
Day (United States of America) Responsible as a member of the interdisciplinary team to the Director and Nurse Manager for assigned functions related to providing information about services to potential Hospice of the South Shore patients and admitting patients to Hospice of the South Shore service.
Compensation Pay Range:
$70,345.60 - $136,136.00Advanced clinical and assessment skills with ability to deal with a variety of clinical situations. Pain management experience , Critical thinking, ability to multi-task, excellent communication skills, ability to work within a team or autonomously, Fexible, excellent attention to details, well-developed documentation skills, IV therapy experience, computer experience, end-of-life care, teaching skills. Support of the Hospice philosophy.
36 hours, 4- 9 hour days Monday through Friday every 4th weekend and 1 holiday a year
