About the Role
As a Young Adult CaseManager, you'll help youth aged 17-21 transition into independent adulthood. You'll support them in finding safe housing, pursuing education and employment, building life skills, and creating lasting support systems. In this field-based role, you'll collaborate with families, community resources, and internal teams to help youth thrive while making a meaningful impact every day. Spanish language skills are a bonus.In this role, you will:
* Provide individualized casemanagement and support to a caseload of 6-8 young adults.
* Meet regularly with youth, conduct home and community visits, and accompany them to schools, courts, and appointments.
* Develop, implement, and update service plans using a strength-based, solution-focused approach.
* Collaborate with DCFS, probation, therapists, schools, and other providers to coordinate care.
* Maintain accurate and timely documentation in compliance with agency and state requirements.
* Participate in team meetings, supervision, and ongoing professional development, including trauma-informed trainings.
Who we're looking for
Required:
* Bachelor's degree in Social Services or a related field with at least 1 year of relevant experience.
* Valid Illinois driver's license, reliable transportation, and access to a personal vehicle.
* Strong communication, documentation, and time-management skills.
* Empathy, adaptability, and a commitment to trauma-informed care.
Preferred:
* Child Welfare Employee Licensure (CWEL) at hire
* Bilingual in English and Spanish is a plus
* Experience working with transition-age youth or those exposed to trauma.
* Knowledge of youth development, positive youth development, and DCFS standards.
Additional Information
Full-time, flexible schedule including evenings, weekends, and on-call rotations. Based in community settings across the Chicago area with local travel required. Field-based role with walking, driving, and occasional lifting (up to 25 lbs).

SALARY: The average starting salary for this position will fall in the range of $47,000 and $52,000 annually. Where candidates fall in this range will be based on skill and experience level.
ESSENTIAL JOB FUNCTIONS:
Provides community care coordination utilizing High-Fidelity Wraparound or Intensive Care Coordination service delivery model.
Facilitates Child and Family Team Meetings for designated caseload.
Conducts necessary assessments and develops collaborative, strengths-based child and family service plans.
Provides outreach and education to children and families about care coordination and eligible services.
Conducts community engagement and provides services to the child and family in home and community-based settings.
Gathers information for the IM+CANS and related care plans.
Monitors daily alerts for participants entering an emergency room or inpatient hospital processes and other coordinated care interfaces, following up with clients
and facilitating post-discharge appointments.
Documents the care/service plans and other vital information in electronic health record after each contact and service.
Actively leads care planning process that specifies direct care resources to meet physical and psychosocial needs; by prioritizing problems and establishing mutually
agreed upon goals specific to the client.
Participates as assigned in participant education, including development of materials, conducting presentations or supporting other team members in such
efforts.
Works with multidisciplinary professionals and community agencies to provide a continuum of coordinated care to meet the complex needs of clients.
Maintains timely and accurate client records and any necessary agency and funder required program reports.
Participates in agency and site based required meetings and trainings.
Other duties as assigned.
QUALIFICATIONS:
Bachelor's degree in a human services field or 2+ years' supervised experience in the mental health field.
Bilingual in Spanish
preferred.
Experience working with diverse child and family populations with significant behavioral needs or social service needs,
preferred.
KNOWLEDGE, SKILLS, and ABILITIES:
Experience working with diverse child and family populations with significant behavioral health needs; social service experience is preferred.
Requires mobility to work across the region and attend meetings as necessary, with vehicle, valid driver's license, and insurance

About the Program and CaseManager Position:
Our Home Study and Post Release Services (HS/PRS) program is dedicated to making a significant impact on the lives of unaccompanied children (UC) aged 0- 17 ½. Referred through shelters or trusted foster care providers, these children rely on our continuous support throughout their journey from the Office of Refugee Resettlement (ORR) to their parents or sponsors. We can guide and empower these children and sponsors through an unfamiliar system, which makes a great difference.
One of the responsibilities of the CaseManager is to conduct home studies. These studies involve assessing the safety of the home and the suitability of the sponsor before the child is unified with them. This assessment ensures that the child will be placed in a secure and appropriate environment. The casemanager will employ child welfare principles and best practices during this process to ensure the child's well-being.
Furthermore, the CaseManager's role extends beyond the initial placement. They also provide post-release services to assess the child and the sponsor's areas of need. This ensures that the child and the sponsor are doing well and are connected within the community. They act as advocates for the child, helping them navigate the unfamiliar system and connecting them with resources that meet their specific needs. This could include medical care, education, mental health services, and any other support required for their well-being. The CaseManager will continue to monitor the placement on an ongoing basis, ensuring that the child's safety and well-being are maintained.
How you get to use your skills as a CaseManager…
Ensure safety for all youth on caseloadManage a high turnover caseload
Comply with agency and regulatory policies and procedures
Coordinate initial intake for each youth
Conduct home studies
Coordinator and handle an estimated caseload of 40 youth at any given time (HS and PRS cases)
Contact each child and sponsor within 24 hours of case assignment
Conduct safety screenings and mental health assessments
Coordinator, schedule, and confirm necessary services for each youth in their location
Communicate regularly with the youth and sponsor to address any needs and/or concerns
Ensure all assessments, reports, and team case notes are current in the hardcopy file and electronic file
Ensure each youth is enrolled and attending school
Respond to client emergencies within or outside work hours
Travel throughout the region, and potentially outside of the assigned region, to complete home studies and in-person case contacts. CaseManagers may be required to travel up to 3 hours for home visits in their personal vehicle
Required Experience as a CaseManager:
Bachelor's degree in a relevant behavioral/social science field required
Experience conducing home studies according to state requirements - required
Strong professional writing, particularly for assessment reports - required
Knowledge of local community social services - required
Experience working with immigrant populations - required
One to three years of progressive experience in casemanagement - preferred
Child welfare experience and professional licensure - strongly preferred
Ability to work in a fast-paced environment
Strong skills in Microsoft Office
Ability to work well with people from many cultures, non-English speakers, and LGBTQ children
Bilingual - English/Spanish required. All agency documents and records are in English, therefore English writing proficiency is required. The position itself requires Spanish speaking, writing, and reading proficiency

MINIMUM QUALIFICATIONS

Each CaseManager will be assigned a “home program” for primary placement; however, the expectation is that all CaseManagers operate inter-programmatically to provide unified, unsiloed, and comprehensive care. This approach ensures that every client can receive consistent support across programs and that the organization maximizes team capacity, continuity, and equity in service delivery.
Key Responsibilities
Client Assessment & Engagement
Conduct comprehensive intake and biopsychosocial assessments for new clients in assigned program(s).
Develop individualized service plans that integrate housing, recovery, employment, health, and justice-related needs.
Build therapeutic, professional relationships with clients using trauma-informed, strengths-based practices.
CaseManagement & Coordination
Provide regular one-on-one casemanagement sessions, adjusting service plans as needs evolve.
Collaborate across programs to ensure continuity of care and equitable access to services, regardless of a client's assigned housing program.
Facilitate connections to in-house resources (employment, education, clinical services, recovery supports) and community partners.
For NPTS clients, coordinate with the Office of the Chief Judge and electronic monitoring officials to ensure compliance and support successful re-entry.
Documentation & Compliance
Maintain timely, accurate case notes, service plans, and progress updates in accordance with program, funder, and regulatory standards.
Track and report client outcomes across programs, ensuring consistency in data entry and compliance.
