This position will support the Care Management Resource Pool and may be required to support multiple locations system-wide within the Legacy Health hospitals.
Responsibilities
Coordinates and facilitates interdisciplinary provision of comprehensive, patient-centered, quality health care throughout the continuum for patients with acute and chronic health conditions. Fosters achievement of optimal health care outcomes within accepted standards of care. Serves as an expert resource to the healthcare team regarding the continuum of care, efficient use of resources, Best Practice protocols, team-based care, quality indicators and improvements, and regulatory requirements. Ensures a smooth transition of care between multiple health care environments with planned handoffs. Partners with patients and families in identifying health care issues and barriers to self-care in order to set priorities and engage in appropriate interventions. Demonstrates cultural agility and employs health literacy guidelines to provide education regarding self-management strategies. Utilizes rapid quality improvement cycles to continuously monitor, evaluate, measure, and report progress of interventions and outcomes. Paces the case to assure appropriate and fiscally sound care coordination across the continuum.
Qualifications
Education:
Academic degree in nursing (BSN or higher) preferred.
Experience:
This position requires extensive knowledge of disease management to include diagnostics, treatment and prognosis, community resources and healthcare reimbursement. Minimum 2 years clinical nursing experience required. Relevant experience in one or more of the following healthcare areas preferred:
Coordination of community resources
Care management of diverse patient populations
Ambulatory Care
Knowledge of levels of care throughout the health care continuum to include; inpatient, emergency care, rehab, home health, hospice, long term acute care, SNF, ICF, ALF with an overall understanding of utilization management and resource management.
Working knowledge of Care Management models across the continuum.
Knowledge/Skills:
Knowledge of six core components of case management:
Psychosocial aspects
Healthcare reimbursement
Rehabilitation
Healthcare management and delivery
Principles of practice i.e. CMS guidelines, Interqual criteria
Case Management concepts
Excellent organizational skills
Health literate oral and written communication skills for effective interaction with all members of the patient's health care team
Knowledge of transitional planning to and from all venues
Ability to determine and access appropriate community resources
Ability to engage patient/family in discussion of health care goals and decisions with attention to cultural and health literacy implications
Ability to adhere to and implement regulations in an effective manner. Must serve as a resource to all team members regarding regulatory issues.
Keyboard skills and ability to navigate electronic systems applicable to job functions.

Licensure/Certification:
Both Oregon and Washington state RN licensure required.

Unity Center for Behavioral Health is a one-of-a-kind center for people facing a mental health crisis. Unity Center features a Psychiatric Emergency Service (PES), where patients receive care by specialists in behavioral health in a calming, non-hospital-like setting. Unity also has inpatient care for adults and teens. It is a collaboration between Adventist Health, Kaiser Permanente, Legacy Health and Oregon Health & Science University.
Responsibilities
Provides patient-centered, trauma-informed, short-term urgent and emergent behavioral health services in adult and pediatric emergency settings across the health continuum.
Works independently and collaboratively as part of a multidisciplinary team.
Offers behavioral health consultations to support practitioners, family members, and patients.
Delivers focused, effective, evidence-based behavioral interventions.
Key services include:
Brief psychosocial assessments
Brief drug and alcohol assessments
Crisis intervention and hold assessment
Supportive counseling
Emergency department diversion
Care coordination and communication with medical teams, family, referents, access workers, and community providers
Transitions of care and discharge planning
Guides patients through the continuum of care.

Qualifications
Education:
Master's degree in Social Work, Counseling or other behavioral health field as required by licensure or credential.
Experience:
Crisis Intervention Specialist I: Experience assessing and diagnosing adults experiencing a mental health crisis preferred.
Crisis InterventionSpecialist II: Two years' relevant experience in care management, mental health and/or addictions services, medical social work, or social service organization. 3 years relevant experience preferred.
Knowledge/Skills:
Broad knowledge of the principles of crisis assessments and interventions and the ability to apply them with individuals, families and/or groups integrating knowledge of normal development and the aging process, changes related to illness, and the needs of patients with acute, chronic, and terminal illness in assessment and intervention with attention to cultural and literacy implications.

About the Position
We are looking to hire a Full Time Night shift On Call Mental Health Associate at our programs in the Lane County area, including Eugene, Springfield, and Junction City, Oregon. These position works as a member of a direct care team serving adults with severe and persistent mental illness. Primary responsibilities include supporting the development of life skills and therapeutic interventions, performing housekeeping, preparing meals, administering medication, and keeping accurate clinical documentation. In addition, you will receive crisis de-escalation training, which requires performing various physical, hands-on maneuvers and techniques.
Please keep the following things in mind:
On Call staff must work a minimum of 32 shift hours per month
On Call staff must attend a minimum of one staff meeting per month (and clinical supervision if a QMHA)
On Call staff must stay current on company training
On Call staff must respond to facility requesting coverage within 2 hours of initial call. In some cases, more notice can be provided.

About the Position

We are hiring Residential Associates and Personal Care Associates (PCAs). DESCRIPTION
Provides culturally competent and appropriate behavioral health treatment to people served by Community Counseling Solutions. The position provides the treatment in accordance with the Oregon Administrative Rules (OAR's) and the mission and values of CCS. The position works closely with other involved community partners and is professional in all aspects of service delivery. Additionally, this position assists administration in meeting the behavioral health needs of the community.

RESPONSIBILITIES
Provide case management type services to children, adults, and families.
Actively participate in joint case planning and coordination with agency employees and relevant agency contractors. Act as a liaison between clients and other agencies. Act as a client advocate when appropriate.
Actively participate in group and individual supervision sessions.
Work with other staff and community representatives to initiate new programs, reviews, evaluates and revises existing programs as necessary.
Perform joint case planning and coordination in areas of behavioral health with other community agencies including but not limited to: schools, group homes, police organizations, district attorney's offices, jails, courts, community clinics and hospitals and other agencies in the Department of Human Services.
Provide behavioral health and addiction education, prevention and information to various interest groups.

A Bachelors degree from an accredited college in social work, psychology or other human service related field is required, or a high school diploma and three years of equivalent training and work experience; or equivalent combination of education and work experience.
Certifications

DESCRIPTION
Provides culturally competent and appropriate behavioral health treatment to people served by Community Counseling Solutions. The position provides the treatment in accordance with the Oregon Administrative Rules (OAR's) and the mission and values of CCS. The position works closely with other involved community partners and is professional in all aspects of service delivery. Additionally, this position assists administration in meeting the behavioral health needs of the community.

RESPONSIBILITIES
Provide case management type services to children, adults, and families.
Actively participate in joint case planning and coordination with agency employees and relevant agency contractors. Act as a liaison between clients and other agencies. Act as a client advocate when appropriate.
Actively participate in group and individual supervision sessions.
Work with other staff and community representatives to initiate new programs, reviews, evaluates and revises existing programs as necessary.
Perform joint case planning and coordination in areas of behavioral health with other community agencies including but not limited to: schools, group homes, police organizations, district attorney's offices, jails, courts, community clinics and hospitals and other agencies in the Department of Human Services.
Provide behavioral health and addiction education, prevention and information to various interest groups.
